Objectives:
GENERAL: To gain more knowledge about taking care of the patient inside the ward and be familiarize with thw different nursing responsibilities for the improvement of every students' skills.
SPECIFIC: To expose ourselves specifically on the medical and surgical ward, to know the different problems of patients and their interventions and other responsibilities as a student nurse.

Observations:
SELF: The second week of our duty was really an interesting experience for me. I really had a good time to be a head nurse on the very first day ( though we were all toxic on that day).
GROUP MATES: This group is really funny and kind. We are all helping each other specially during the duty.
C.I.: This week, we were all toxic but Ma'am Nhelia stayed cool and funny. She doesn't want us to feel the that way. Even if she's tired, she makes us all happy.

LEARNING:
This week, we are all work as functional. Everyone has its own assignment-the medicine nurse,the head nurse and the procedure nurse. I was assigned to be the head nurse on the first day. I learned to look out or monitor the different patients assigned to us. Though it was really really hard because they were all toxic, I had a great time dealing with them. On the second day, I was then assigned to be the procedure nurse and we bathe our patients, we did the morning care and some procedures that the patients need. It is an overwhelming feeling to do this job.

General Objective:

To learn how to handle patient according to nurses function in the ward like head nurse, medication nurse and procedure nurse.
Specific objectives:

To gain more experience in handling patient functionally.

Observations:

Self: on our first day to handle our patient functionally, it is so difficult for me to do things right because I have to assess or care for 3 patients at the same time and I have to render care for them accordingly. On the second day of our functional nursing thing have been more essay to do because i have learn new thing on our first functional work, and my group mates also help me to do my job in the area.

Group mates: We as group, works together to make all things in order especially in assessing the patients need to be comfortable.

Clinical instructor : Our C.I help us to manage the condition of our patient, especially when there are new orders of doctors that are new to as, she also help us read the doctors order that we as a student could not understand.

Learning’s:
I have learned that it is more essayer to work in a group than to work by my own. Because of we are working together we can correct the work of one another. But there are times that we commit mistakes in our works. That is why, our clinical instructor give us demerits.

General:To advance our knowledge in performing functional nursing
Specific:To effectively apply nursing process as part of our nursing care to patients in QPH

Observations:

Group mates: The group participated well in the conduct of their assigned task.
Self: I enjoyed working with the group because they have shown their cooperation as well as their dedication to their duties.

C.I.:We look forward to another fruitful rotation with you Maam.Thank you for your unending support to us.

Learnings:
I consider last week's duty as another exciting and challenging work for me. Functional nursing is a learning technique for student nurses like me and this opens new ways to enhance and develop our skills in the clinical area. As part of learning, our strenghts and weaknesses will always appear as we go on with our duties.However we hope to acquire more learnings and learn from whatever mistakes we commit along the process.

OBJCTIVES:

GENERAL OBJECTIVE
To be able to know what are the different techniques in rendering health care to the patient. and especially on how to interact with the patient, giving more information to the patient about their condition. and also help them for the rehibilitation of the patien.

SPECIFIC OBJECTIVES

In giving nursing care to the patient, the nurse must establised a plan for the recovery of the patient, like giving moral support , tender love and care.

OBSERVATIONS:

SELF
On the second week of our duty im very confident in doing my work it because i was asign as a procedure nurse to our patent. i do bed bath to my patient and cutting finger nails.and also i observed the effeciveness of my intervention to my patient and that was good, i done my work well.

GROUPMATES:

To my groupmates they are very supportive and kind.they were tryinf their best to be an effective nurse to the patient,we as a group did well.

CLINICAL INSTRUCTOR:

Maam Nhelia is a very strict person, she always teach us on how to do the proper techniques in giving the medication to our patien even on proper preparation of medication. a very kind clinical instructor, she makes us happy during our stays on the ward.

LEARNINGS:

I've learne a lot during my stays on the ward.it wasmy first time to be a head nurse, everyone of us was asign to be a head nurse, procedure nurse and medication nurse. it is a test on how we are gouing to establish a plan on how to the wprk more easier. i was very happy it because the cooperatio was thier,.

I. OBJECTIVES:
A. General Objective:
To be able to apply effective nursing interventions prior to pt's needs and complains.

B. Specific Objective
To be able to apply what we have learned from our lectures and to learn more from the different cases we have handled.

II. Observations:
A. Self:
In our second week of duty,I enjoyed the functional type of grouping it seems that we are like Registered nurses having our own duties.

B. Groupmates
My groupmates enjoyed our duty because it is more easier for them. Most of us have a good relationship and are participating.

C. Clinical Instructor:
Ma'am Nhelia as always did her part as our clinical instructor, she did guided us throughout our duty eventhough we are toxic during those time.

III. Learning
In this week i've learned to be independent from doing my responsibilities,.This week I was assigned as the head nurse and as the procedure nurse, I learned to be calm eventhough there are many things to be accomplished, I've learned to work with others.

Objectives:
General: to be able to practice my profession and gain more knowledge on how to care patient regarding different diseases.

Specific: to be able to render service to my pt. with tender loving care. and gain more experience in handling pt. functionally.

Observation:
Self: during our functional group at firs it is very hard to handle many pt. specially when you are the headnurse.but i enjoy working together with my group mates.

Group mate: as i observe to my group mate we have the same filling during the first day of functional group. They were to toxic but as i observe to my co group, were lucky because were helping to one another as a team.
C.I.: Ma'am Nhelia is there to help us always in handling our patient specially when giving some medication, she’s also kind C. and she makes us all happy everyday in the ward.
Learning’s: during our functional group, i feel exciting and happy all the time and i also learned a lot because we’ve been test on how we are responsible to our designated assignment. On how to be resourceful and active all the time.

to gain knowledge & more information & enhance our skills in exposing to the communicable disease hospital at San Lazaro Hospital

b. Specific Objectives:
>to render care
>to apply our skills & our knowledge to what I've learned about communicable disease
>to apply our interventions as a student nurse to our patient at San Lazaro Hospital
>to share information to our patient about preventions, treatment & about where & how they get those communicable diseases.

II. Observations:

a. Self:
At my first exposure to San Lazaro Hospital, I felt nervous & afraid to those patients at San Lazaro Hospital especially to those patient who have TB disease. But at the next day of my exposure I felt I'm confident. And not yet afraid to my patients.

b. Group mates:
they are also like me,they are afraid and nervous,but at our 2nd day exposure to other pavilion, they are also confident. they are responsible in doing their responsibilities as a student nurse to their patient.

c. Clinical Instructor:
She is understanding, kind, and a good enhancer, a good Clinical Instructor & a good adviser too.She widen our understanding and our knowledge about those communicable diseases. She help us to become a good nurse,and make us become flexible nurses.She make us also happy even we are toxic.

III.Learning:
I've learned that if you handled patients who have communicable disease, you don't need to be afraid with them,rather you need to pray them to treat their illnesses, and pray to have a good health especially to those patients who have a critical illnesses.

>to be able to apply the skills and the knowledge that we've learned in school to the clinical settings

II. Observation:
a. Self:
I observed to myself that I am excited because its my first time to step in San Lazaro Hospital

b. Group mates:
to my group mates they are supportive and they share their ideas about the disease that they can handle.

c. Clinical Instructor:
to our Clinical Instructor she is kind, & supportive. And sometimes she taught us if what we will do in our duty.

III. L earnings:
Our duty in San Lazaro Hospital, I've learned how to mingle and cooperate in our patient with communicable disease. And we need to treat them as a normal.
