I. Objective
a. General Objective
To appreciate the different duties and responsibilities of nursing student in term on handling pt at Quirino Provincial Hospital (QPH), Medical extention Ward.
b. Specific Objective
To provide quality care to my pt.
To gain knowledge and skills at hospital setting

II. Observation
a. Self
In the 3 weeks of our last rotation, medical extention ward….in every rotation i learning something new, that would help me to improve more my skill in the field. Its our 1st week and all boys are the head nurse, and I have encountered new cases, like benign positional vertigo, error in refraction…on the 2nd week of our duty im am the procedure, my patient has a liver cirrhosis and he’s on BT it ismy 1st time to have a patient with BT and I thought it was hard but them it was so exciting and challenging, and by other patient has AGE and his IV was dislodged and I was ordered to reinsert the IV of my patient unluckily it was not a 1 shot but I did my best…on the 3rd week and last week of our duty we are going to have a case analysis and evaluation exam and a food trip (dinner), although we still had a patient to take good care of and luckily my patient has a TPN so now I have seen what is TPN looks like, and on the last day of our duty my patient passed away who has liver cirrhosis…we’ve done post mortem care to our patient….
b. Groupmates
I really appreciated the attitude of my groupmates. Even sometimes there are kill joy they always go on w/ the diff topic. Now, they are my close friend w/c I can lean on my problems when troubles has come. They share their knowledge and skills, w/c I gain knowledge from it.
c. CI
She always begins the duty so nice and exciting. She carried the responsibility of being our clinical Instructor and I appreciate madam the way she is. On then second hand she is also a keen observer and she share her knowledge w/ us. She work so hard, and im thankful we have this kind of C.I

I. Objectives:

II. Observations:
a. Self
> this is my last rotation in our RLE and my first time to have my duty in that area. at first day, I feel nervous because I dont know what is the protocol in the Medical Extension. Our clinical Instructor said that we will be functional. so my first task is to be a procedure nurse. and our first patient is on going BT, so I monitored him every 30 mins. because im afraid to commit mistakes and to avoid demerits...
in my three weeks of affiliating every week is may new learnings. habang tumatagal ang duty, i felt comfortable and hindi ako natotoxic as my other groupmates do... I learned a lot in this rotation because mam nhelz thought us in every new diseases we have encountered. happy because its our last and I will cherish those moments that I am affiliating in that ward.
b. Groupmates
> first time ko silang mkagroup except Mr. Guinto and Mr. Balisi.. i thouhght I will not be enjoying this group pero nagkamali ako cause kahit papano i felt special and cared by this group just like my recent groups. They're easy to approach and willing to help if you are needy, they're also approachable.. meron akong naging ka-close meron din namang medyo lang, pero kahit ganun, atleast nakilala ko din sila ng lubusan...

c. Clinical Instructor
> She begins our every duty in a smile and it occurs it smoothly to do what are the task that we were going to do. she is able to answer in every part of our mind that has question mark ? ... she is approachable and willing to help if you her help...
III. Learnings
> I learned what are the dangerous adverse reaction in a patient who has on-going BT.
> I learned to be more efficient in constructing my SOAPIE.
> I learned to know what are the new diseases.
> I improved my skills as a Head, Medication and Procedure Nurse.
> I learned that I will Always be reading my meds many times.

a. Self
> In our last 3 weeks and last rotation in the Medical Extension.....I learned new things that may help me to guide my way in this field. In this 3weeks, I have been assigned in different clusters.Sometimes toxic but I enjoy what am I doing.In our last night of duty, the patient that we handled died with a diagnosis of Severe Liver Cirrhosis. So sad because this is my first time that my patient was die within our shift. In my part, we gave all the care needed with our patient but still life is in the hand of God.After that sad incident I and my 2 group mates who are assigned with that patient do the Post Mortem.It is my first time again to do that thing, so scared but we need to give respect and a peaceful death of the patient. This is a meaningful last rotation for me.
b. Groupmates
> Im happy and comfortable to be with them. They are funny and helpful with each other.
c. C.I
>She is one of the C.I that I will be miss. Very jolly though we are toxic. Guide us in the proper way of everything we do. And share knowledge.
III. Learnings

A.Myself:
I never imagine, I’ve gone this far..Wow drama..hehe..When I first step nuersing, I felt the feeling of moving forward and backward..In short urong sulong..=).A part of me says ‘will I make it?’But now im almost there, I’m almost fulfilling the dreams I’ve wished for, the dreams,my parents wanted me to be. Now as a 4th year graduating student , I can really say that I’ve learned so much in my 4 years stay in my dear alma mater and in the hospitals where we affiliated.
B.grOupmates
Ofcourse, my life will not be completed without my everdearest groupmates. My source of happiness during our clinical duties.And I can really say they really made my life more meaningless (ay meaningful pala.)hehe..I thank them for the laughters, the anger and a little bit of depression they gave me.=).I will never forget that one day in my life theirs someone like them who came into my life and brought me joy.

She is very kind and nice instructor from the beginning until the end. And were really blessed to have such C.I like ma’am nhelia. What I will not forget about her is that she’s the one who taught us the proper addressing of our co student nurses. Whenever she caught us calling our groupmates with their names only, she will alwayssay “ano?sino siya?sino siya?”and as a result to that she will give us a corresponding demerit. She’s not only a C.i to us but she’s been a mother, a sister and a friend to us. Thank you so much for all the learnings ma’am. :)
III. LEarnings
I’ve learned so much from our entire rotation. Learnings that I will forever kept in mind. Learning coming not only to our c.i but also to our patients. I will never forget the knowledge regarding different diseases shared by ma’am nhelia to us.i will also not forget what had ma’am nhelia told us,that when we became RN’s we should bear in our mind a very important goal and that is tO “keep our patient alive”.we should take good care of our patients no maTTER how less is their chance to live. We should give the best care they desrve to. During our our entire rotation, I’ve encounter different pt’s with different characters, some of them treated us nice but some just ignore us and showed irritability to us. I appreciate those pt’s who say ‘thank you’whenever you give care to them, it’sa nice and unexplainable feeling to hear something like that from them. It’s also a nice feeling when a pt appreciates what you do to them. And I will not forget that one of my pt. hold my hand, she has difficulty in speaking but understand what she really wants to say.i’m so happy she accepted me as her nurse. And to see a pt smiles at you whenever you render care to them. It lessens the stress you feel. And it really breaks your heart into pieces whenever you see your pt. suffering to much pain from their illness. It gives me the eagerness to help by offering the best care to them. I also pity those pt’s who can’t afford to buy medicines for their treatment. Sometimes in cases like that the only help that I can offer is to donate syringes to them and pray for them.
In our every day encounter, I can really say that we have touched their lives and they definitely touches ours too. THANK YOU SO MUCH.!!
We must "keep our pt. alive" this is our ultimate goal for our last duty being a student nurse at Northeastern College. We were trained by our clinical instructor ma'am Perez to help our patient by promoting health, prevent illness and alleviate sufferings.
In my 3 weeks under ma'am Perez I've learned how to smile and keep my integrity for every poor performance I had.I was really disappointed to myself because of different trials i had, but like ma'am Perez says "do not allow trials stop you from dreaming", thus for now on I'll take trials as my stepping stone to success.
In my last week of duty, it was purely self esteem and trust to God that made me strong to comply with the requirement in RLE.
I would like to thank all of my group mates who made me strong, but sometimes it was hard for me to work with them because it was my first time to be with them, knowing they already established strong relationship with each other.
Moreover to my instructor, ma'am Nhelia Perez who really cared for us, thus it was proven when I had problem on administering vial( i accidentally broked it) she was there for me with "Tender Loving Care", HINDI KO PO KAYO MAKAKALINUTAN MA'AM, SORRY PO SA MGA KAPALPAKAN KO..:-(.Thannk you po sa guidance and patience, with the additional knowledge and skills you've given to us I will be guided in the future as a REGISTERED NURSE!

She cares and supervised properly. She really had a "TENDER LOVING CARE" not only to patient but also to us .

III.LEARNING
I was goal oriented, thus our ultimate goal is to "KEEP OUR PATIENT ALIVE". Although to some it was goal unmet, but it serves as their experienced in doing post mortem.
I've also learned to have integrity and self-esteem in times of trials, thus I will always remember what Ma'am Perez say's "Do not allow trials stop you from dreaming".

Hi maam.. It’s been almost 2 and ½ years mula nung naging CI namin kayo,, Si maam Nhelz eh bihira ko makita magalit.. nagagalit pero hindi naman nag tatagal.. and kung meron man kami magawa kasalanan or pag kakamali,, hinde sya ung tipong,, “yan kasi,, sinabi ko kasi ganito ganyan”. Hindi tulad ng ibang CI, wrong plotting lang na pwede naman burahin pinapalaki pa. pero maam ayuko sana isipin ng iba na nag papasipsip ako pero mabait si maam.. sa inio ko nakita yung sinasabi nilang “Your teacher is your second mother”.

Oi Groupmates.. hows life? Yung iba sa kanila toxic,, pero kitang kita naman.. nag eendorse on time and minsan pa earlier than the said time to endorse.. Hindi ko nga ba alam kung bakit pinagsama sama kami d2 sa group na 2. ESP these guys

Self : I’ve enjoy and appreciate my last duty. We’ve been at Pedia ward. Patients are very hard to approach and even the clinical instructor. I can say there is a big effect if clinical instructor is very approachable, less toxic, less nervousness, less wrong performance
Groupmates : Almost of them are confident of what there are doing. They finished their works on time even they have a toxic patient
Clinical Instructor : I can say you are my favorite CI, our second mother, our true adviser. Although I’m only a transferee, I appreciate you as a teacher and CI because we can ask and approach you anytime unlike others, others will not teach or lift you up when you did something like accidentally open new meds where in there is available in the fridge,instead they will dump the student and ask “may pambayad ka dyan?” inspite it is only a generic cefuroxime. Thank You maam for being very very patient to us. Thank you for being a good teacher, adviser, clinical instructor and our second mother. God Bless!
Learnings: I’ve given an insight in the case of one patient which is Benign Positional Vertigo. Which is rare case.

Objectives:
General-to render maximum care and aleviate suffering of my patient, lastly i keep my patient alive that is my goal in our last duty in medical extension ward.
Specific-to improve the condition, to give the needs and to establish camaraderie among patient. and to develop my weaknesses in the different fields.
Observations:
Self- 1st im very lucky and happy to have this group, because in the entire hours of our duty they make me laugh, a keen observer daw ako, pero minsan nangto2xic daw but the near facts is I don’t want to commit error or mistake in the chart, I want also to make easy everything,.maybe naoobserbahan niyo n lagi akung pumupunta sa patient ko its because ayaw ko kasing msubukang maoverload ng IVF or ma underload. Sad ako when CAMILO died because I felt that I am not effective student nurse, honestly speaking takot ako when I and Ms. GALAPIA do the post mortem care to my patient, feeling ko kasi ha2wakan niya ako…Naging confident ako for the whole rotation in the extension kasi may tiwala ang CI nmin. Kaya I don’t want to break that TIWALA. RHODGE is still Rhodge what would be my career after my college ako parin ang nkila2 niyo n a rhodge ma’am promise poh. Jejejejejee Love u ma’am
Groupmates- I want to give my heartful thanks to my Groupmates. Because they are soo sweet and very understanding to me kasi when ever I need them andiyan sila, mapa parapernalias man, Procedure aanjan sila para tumulong, now I know with proven that all my groupmates are very responsible and therapeutic because they always give a sweet smile to their patient before, within and after NPI.
CI- I want to thanks you ma’am as being part of our life, being nice and nontoxic CI, as we observe your unique kasi iba ang feeling at bonding pag kayo poh ang kasama namin ma’am. Nasubukan nmin ang maging confident at mag-isa sa pagbi2gay ng meds ma’am at masaya kayong kasama mam. Again and again thank you ma’am and May you be blessed a hundred folds
