Self:On our first week,Maam Lutrania oriented us in male ward and I'm excited because its my first time here and I do cooperated to them
On our second week,we handled client.And I checked the meds before I gave to my client.I cooperated to my C.I's,staff's,groupmates,and to my clients.
C.I.:Our C.I's who handled us is kind and supportive.Always accompanied us every time we give meds.
Learnings:
I learned the policies in male ward.Improving my self confidence also.And putting always my self on their situation

MYSELF:
as a student nurse, i did my best to become more responsible on the duty to know my obligation and work with my heart and soul and always mentally present to avoid any complications that can result a risk to my client.
GROUP MATES:
Same as through with me all of us are participating, we did our obligations specially in taking good care of our patient. We have our bonding in sharing some stories giving jokes, and also sharing of ideas and knowledge regarding on the cases of our patient.
CLINICAL INSTRUCTOR:
my impression to her is very strict so difficult to approach, but this impression is wrong sher is approachable kind and very supportive C.I
LEARNINGS:
More knowledge that i got from my groupmate and C.I on sharing their knowledge,ideas and opinions,that can help for my improvement as a student nurse.And I learned specially on how the proper way of taking good care of the patient.In the male ward i experience to have skin test with my patient.And i practiced to become more alert in the duty thats why I got two merit to our C.I

general>this is my first duty at the male ward so need to gain more knolage and expirience.

Observations:
self> i observed that in my duty in the male ward I gain alot of knowledge; my skills improved and mature but still needs alot of learnings and expirience to ba able to master the best health care a can give to my patients.
groupmates> to my groupmates observed that they are all hard-working they enjoy our first two weeks in the duty and I've laerned more with them.
C.I.Maam Perez is nice, I always learn new things in her. and she also helps us in our duty.Us for maam lutrania and maam chit. I've also learn many things with them. and they are also nice to us.

First Rotation

To conduct our orientation at male ward about process of rendering care to the patient's.
B.Specific
To Know the rules and regulations in doing the charting of patient's especially in giving medication and performing treatments to the patient's.
II.OBSERVATIONS
A.SELF
Eventhough its my first time to take an duty at male ward, I did my work well for the first rotation,I felt nervous because there were many patient's and I'm thingking that they were toxic,but in the end I felt happy because I've done my work and the first week of duty was finished again.
B.GROUPMATES
They were cooperative,helpful to other's that has a toxic patient.they did their responsibilities and work well especially their duties to their patient's.
C.I.
Ma'am lutrania oriented us,and handled us for two days,and Ma'am Chit for Saturday,they were good C.I.,they taught us when we don't know what to do, they also explained some about the diagnosis of our patient's, and of course they also strict,and I know that it is important to us as a student so that we will learn.
III. LEARNINGS
I've learned on how to make a soapie again with revision, I've learned how to charting, to give medication especially in computing the medicines.I've learned how to admit patient.
Second Rotation

To acquire knowledge and learnings through observations and by applying it appropriately.
B.Specific
To render effective care to the patient's and to apply appropriate nursing interventions relevant to the needs of our patient's.

I became improved know,I know how to make my work easier than before,I am more faster and more organize in my work compare for the first rotation.
B. Groupmates
They were better for this week, they were more organize in their work and they were know also what will they going to do for some instances that happens to their patient's.

Ma'am Nhelia is our C.I. for thursday only,eventhough its one day only I've learned more from her in our duty,she has a sense of humor,she know how to make her student cool and relax with their work eventhough its hard and tiredy.
III. Learnings

OBJECTIVES:
1. General Objective:To familiarized and exposed ourselves in the Male Ward setting and gain knowledge and skills in order to effectively render care and service to our patient.
2. Specific Objective: To render an unbiased care to patient suffering from cardiac arryhthmia and a patient with Benign Prostatic Hypertrophy in the second week of our duty.
OBSERVATIONS
1. SELF - I enjoyed my duty in the Male ward, and i gained more terms, skills and knowledge as days went on. Mistakes arte just part of our duty but these make us more persistent and patient enough to withstand all difficulties that come along. I enjoyed being with my patients, with my groupmates and clinical instructors as well as with the staffs.
2. GROUPMATES - I can also see the gleam in their eyes as they render care and attend to the needs of their patients. Some were toxic, but we see to it that we help each other whenever possible. I know they also learned a lot of things.
3. CLINICAL INSTRUCTORS - Actually, we were supervised by three CI's this week. Maam Perez is a skilled and intelligent CI. She constantly guide and help us in the duty. She see to it that we learn and acquire a skill. Shes friendly and easy to deal with. Mam Lutie is also equally kind and knowledgeable CI in her own way. She also support and teach us the right and best way. Maam Chit is a vocal and straight forward CI. She is frank and strict in her own good way but we are learning from her. Shes friendly and approachable too. All of them deserves to be emulated. and Im proud to be one of their students.
LEARNINGS:
Patience, perseverance, care and dedication accompanied with good motivation are best way I think to be successful in everyday duty as well as towards a long term goal. I learned to be extra careful in everything I do in the duty especially when administering medications. I learned other medical terms and diagnosis, skills and knowledge in the male ward. Harmonious relationship with your companions is also very important in order to perform well. Establishing rapport and trust with the patient, and with a positive outlook of life as a student nurse help a lot. Thanks for the CI that incessantly teaching and sharing with us their skills and gifts of wisdom.

Self:first i feel nervous because this is our first time to expose in the male ward, but i tell myself that i would be more responsible in rendering services to our patients.

LEARNINGS:
I learned new things during our duty in the male ward, learned how to do discharge process,and learned to be more active and alwayrs ask your C.I if you have some concerns about the medicines that you our giving.

General:to learn more about the different cases and how to handle the health problems of the patients.
Specific:to be able to follow all the policies in the male ward and how to handle the different cases of male patients.

Groupmates:they have more confidence when our c.i maam nhelia and feel toxic to maam chit.
C.I's:they approaches students in different ways and have different strategies in teaching students.
LEARNINGS:
during our duty i have known that different C.I have different approaches to there students.I have also learn how to discharge a pt.accordingly.and to refer to a doctor.

To improve more my skills towards handling a patient because they are our primary or priority to be care.

During our first day Ma'am Lutrania oriented us about the routine at male ward. I feel nervous because its my fist time.

They are helpful and very cooperative.

She's very kind and approachable every time we need her help.
Learning’s:
I learned allot during our first three days of duty at male ward on how to handle a CVA patient regarding on his need and how to cope up with his condition.
1st Week
I. Objectives:
A. General Objective
To be able to know what are the different things to be done in the Male Ward.
B. Specific Objective:
To be able to familiarize with the different drugs that are usually given in the male ward.
II. Observations
A. In our first week of duty in the male ward I felt like it is my first duty because in our past 2 rotations we are not to make our SOPIE and write it in the chart.
B. Group mates
Most of them did they responsibilities well, although most of us struggled in making our SOPIE.
C. Clinical Instructor
In our first week of duty Maam Lutrania and Maam Chit serves as our CI, both of them have their own strategies in making SOPIE, but we enjoyed each companies. Maam Lutrania oriented us about the format of discharging,writing in the medication card.
III. Learning
I have learned about the different kind of drugs given in the male ward and their stock.And got familiarized with the different policies in the male ward.
2nd Week
I. Objectives:
A. General Objective
To be able to expand more our knowledge and to improve our skills.
B. Specific Objective:
To be able to give our holistic care to our male pt. young and adult.
II. Observations
A. Self
In our 2nd week of duty i kept on praying that God will guide me, so that I can give the appropriate care for our pt. and to avoid mistakes.
B. Group mates:
Most of them are confident in what they are doing, some are showing their interest in skin testing. Most of us are shares our ideas if one of our group mate seek for help specially in formulating a SOPIE.
C. Clinical Instructor
I can't say any negative comment about them because they did their part as our C.I. they guided us.
III. Learnings
I have learned about the format of carrying out a new Dr. order.
LFD 1st week
General objectives:To determine and assess pt.'s health needs and to collaborate efficiently with the health care team.
Specific:To effectively apply nursing care through implementation of nursing process.
Learnings:
Orientation was conducted to give us the overview of the institution's rules and policies as well as the different wards in the male section and had our first quiz in the afternoon which served to be part of evaluation. On the second day, respective pt.'s were assigned to us and we started on with our duty proper where we rendered morning care, monitored v/s,regulated IVF, and administered meds after which we made our soapier charting.Our duty ended well with some of us getting merits and demerits but somehow relieved that another day had passed.
LFD Second Week
General Objectives: To organize and implement pt.'s plan of care that will greatly contribute to their early recovery.
Specific: To involve pt.'s in their health care process and at the same time educate them with our health teachings.
Learnings:
I had looked forward to another fruitful week so we could prepare for the grand case.On the 1st day of duty, we managed to worked on our routine job with our pts. and started looking on possible case which will serve us another achievement to end our rotation. Luckily, there were 2 cases of CVA and everybody had their fair share of work.On the second day, we attended to our patient's needs and we learned the procedures of admitting pts. and discharging them from the hospital.We filled out forms needed for admission as well as those scheduled for discharge. On the third day, we were given the same sets of patients and we had initiated and done nursing interventions with the guide of our supportive C.I's.
We are hopeful that we will always learn from our mistakes and have the initiative to develop more of our skills through the endless efforts of our C.I's. Thank you !
1.Who were the persons involve in the case?
The persons involved in the case were Lorenza Somera the Head Nurse, two student nurses Valentina Andaya and Consolacion Montinola. The assistant surgeon Dr. Bartolome. and the two Doctors during the operation, Dr. Favis Dr. Bartholome.
2. In your own words, relate what happened.
Several days prior to May 26, 1929, Pedro Clemente took his daughter, Anastacia Clemente, to Dr. Gregorio Favis at Manila. Dr. Favis decided to perform a tonsillectomy and instructed the father and daughter to go to St. Paul’s Hospital where he would perform the operation at 7am on May 26, 1929.
Assisting Lorenza Somera, a Head Nurse, were student nurses Valentina Andaya and Consolacion Montinola. The assistant surgeon was Dr. Bartolome. During the operation, Dr. Favis asked Dr. Bartholome for Novocain solution. Ms. Montinola handed Dr. Bartolome a syringe of solution which was handed in turn to Dr. Favis who injected the same to the patient .After a few minutes, Dr. Bartolome noticed that the patient was becoming pale and acting as if dying. He called the attention of Dr. Favis to this but the latter said it was not unusual. A third syringe of solution was injected and a few minutes later, the patient died in a few minutes. Dr. Favis asked if the Novocain was fresh. Ms. Somera replied that the solution was not Novocain but 10% cocaine.
In 1929, in Manila, Lorenza Somera, was found guilty of manslaughter, sentenced to a year in prison, and fined one thousand pesos for following a physician's orders and not asking why there is a need to inject cocaine to the patient.
.3.Identify the errors committed by the persons involve in the case.
Wrong orders given by the Doctors to Lorenza.
Not questioning the Dr.’s order to inject the patient with cocaine.
Injecting 10% cocaine instead of Novocain to Anastacia Clemente that cause her to death.
Still injecting the patient with syringe even though she is suffering from lymphaticus and that such patients were known to die even with so slight an injury as a needle-prick.
4. If you were in the place of Miss Somer, how would you act in the situation at hand?
If I were Miss Somera I’ll question the Dr.’s order of why he's giving the patient cocaine instead of the right medication. I will let them remember about the things to do and not to do in handling cases such as lymphaticus.
When in doubt I’ll obtain confirmation and use cautions.
I also won’t just depend on what the doctor says, I have to know if the doctor's orders are correct.
5. If faced with the same situation, what would you do in order to defend yourself?
When faced with the same situation , I can’t never defend myself, nor complain because it's my mistake of not questioning the Dr’s order and not confirming if it's the right medication for the patient . There is a clear negligence on my part.
