affiliation 2008

June 9, 2008 (9AM)... The Northeastern College - Nursing Department is now on its third Affiliation in the three specialized Tertiary Hospitals in Manila namely San LAzaro Hospital, National Center for Mental Health and Philippine Orthopedic Center. The fourth year Nursing students were assigned in 2 Batches. Block A and B comprises the 1st Batch while Block C comprises the second batch.
The 85 students were accompanied by 5 CIs and the dean of the college. Ms Maricel Manuzon and Ms Noralyn Ualat are handling students at NCMH, Mr Arthur Garcia and the Dean are handling students at POC and the two Coordinators, Ms Luisa Valencia, Clinical Coordinator and yours truly, Academic Coordinator are handling students at San Lazaro Hospital. To make sure that students are well prepared for their exposure at the 3 hospitals, concepts on CommunicABLE dISEASE nursing were incorporated in their NCM 102 while 4 units of their NCM 104 was offered last summer 20o8 and this includes Psychiatric Nursing and Orthopedic Nursing.

Students are housed at the Good Sheperd LAdies Dorm in Sampaloc. Rules and regulations are imposed to ensure order.

Though the experience is a lot different from QPH, the school's base hospital, still it was noted that the BSNstudents of NC competes with the other students from City schools in terms of skills//

And for that, Im proud...

The first week passed. At the end of the weekend, 40+ students were grounded due to varied violations.

But all in all, it still amazes me how people react to different situations.. how they improve as nurses and as people!!!

This is just the beginning.

OBJECTIVES:
GENERAL- To know the routine at San Lazaro Hospital, to be exposed to different communicable diseases and to know the common communicable diseases common at SLH.

SPECIFIC- To apply the different nursing intervention that we've learned during our discussion and enhance our skills and knowledge in rendering best care and intervention to our patient's.

OBSERVATIONS:
SELF- Giving the best skills and knowledge is the most important thing that i've learned. I'm very excited to be exposed at SLH because it's my first time, hopefully not the last exposure. I really enjoy our stay at SLH. Giving care to my patient is the best experience.

GROUPMATES- As a group we must share our knowledge in order to attain our goal. although we came from different blocks we easily get along together.

CLINICAL INSTRUCTOR- Maam Nhelia is very supportive, she shared her knowledge to us that we needed at clinical area, we enjoyed her company she is really a joker, we enjoyed laughing to her jokes as we travel back to the dormitory, i will be missing that experience. she is very relaxed eventhough sometimes we wre not following instructions. I hope she will never change.

LEARNINGS:
I've learned different intervention that we needed to different communicable diseases, different routines at San Lazaro Hospital that we needed as a independent nurse in the future. thak you to our instructors you are the BEST.

I.OBJECTIVES
A.General
To know the history and foundation of San Lazaro Hospital
To follow their rules and regulations at every wards for better care and effective nsg. Management to all patient’s with communicable diseases.
To develop our skills, knowledge and attitudes in caring a pt’s with different infectious, contagious, communicable diseases.

B. Specific
To apply our knowledge and skills in caring pt’s. with communicable diseases.
To observe a pt. with difference communicable disease and gather more information about communicable diseases.
To Finish and pass all the requirements on time at SLH and review all necessary topics that are needed.

II.OBSERVATIONS
A. Self
I am very blessed and thankful to my family, C.I’s., Friends especially to god because they included me for this batch 2008 affiliation. I don’t know how to describe my feeling, I’m just happy that I’ve done my duty well at SLH even though I’m toxic. I’m proud of myself because I surpassed again this one of the hardest milestone of my study, career and profession someday.

B.Groupmates
My Group mates are blk. A and blk. B, for the first day we were all on the adjustment phase, but we easily get along together. We shared our ideas, attitudes, and perceptions to the different topics in our duty and we’ve learned from each other. They were cooperative, helpful, jolly and joker that made fun and enjoyed our everyday duty at SLH. They did their duties and responsibilities as a student to our C.I. and as a student nurse to their patients. Even though some of them committed mistakes and our CIO’s gave advice to them, they take it positive that can serve as challenge to improve their selves and studies.

C. C. I.
Ma’am Nhelia is our C.I. She’s very jolly. She know how to make her student relax and enjoy their duty even though its toxic. She explains everything that is not clear to us. She’s a good role model to us to be a good C.I. someday, that whoever your patient is gives respect and be patient to them. Thank you for sharing again your skills and knowledge to us, stay cool and happy, God bless!


III.LEARNINGS
For this affiliation at SLH, I’ve learned the history of SLH, the different pavilion and their rules and regulations in rendering care to the pt. I’ve learned their pattern of medication sheet and some doctor’s order. I’ve learned how to get a supply and how to return it. I’ve learned to become patient again everyday by waking up early in the morning and to find our scheduled Pav. Everyday even though we do not know the way going to that Pavillion.

I. OBJECTIVES:
a. General Objectives:
 To be able to gain more knowledge and skills in San Lazaro Hospital.

 To impart our knowledge and skills learned in school to San Lazaro Hospital.

II.OBSERVATIONS:
Self:
I discovered some of my potential when I was exposed to the area that can make my existence a meaningful one. Sometimes I’m a little bit nervous but when I handle my patient, the fears and worries I had was totally lessened. I was so happy because even in small things, I was able to help and I do realized that I have to be mentally alert when rendering health to patient.

Groupmates:
They are all enjoying our duty in San Lazaro Hospital. They are responsible enough in doing their task assigned and more alert when rendering health. And while doing those things, learn more in handling patient with communicable diseases and also they are kind and friendly.

C.I.:
Have patient enough for us even if we are doing some mistake in charting. She is very supportive and approachable to us. And well dedicated to her job. I enjoyed being with Ma’am…. Thank you for your kindness Ma’am.

II. LEARNINGS:
This duty was really a learning experience I’ve learned a lot in San Lazaro Hospital. Especially handling a patient with Dengue Fever, Pneumonia, PTB,Hepa B, and Typhoid Fever. And I’ve learned also the most important virtue in nursing is being patience to the patient so that you can attain your goal.

I. OBJECTIVES:
a. General Objectives:
- To be able to know the different kinds, signs and symptoms of communicable diseases at San Lazaro Hospital.

- To become an efficient student and effective nurse

- To widen my knowledge about CD and enhance my skills in handling patient with CD.
- To enhance my confidence in dealing with people with communicable diseases at different pavilion.
- To render quality care and to assist patient with their needs.
- To help individual to become aware to their health lifestyle.

II. OBSERVATIONS:
Self: It was my first time to be exposed at San Lazaro Hospital I felt so nervous because I didn’t knew how to approach people with communicable diseases, but as the days past, I observed to my self that I improved and I realized that every work I done has a direction. And I’m very happy because even in small things that I’ve done I was able to help other people.

Groupmates: To my groupmates, they are enjoyable and responsible enough because they do their respective work assigned to them and my groupmates are approachable and friendly even this is my first time to be with them.

C. I.: Mrs. Nhelia B. Perez is very understanding and enjoyable C.I. to us. She have self confidence, she is also very creative, she want that all of her things will be neat, and also she is very energetic C.I. But sometimes, she is very strict especially in charting. She want that the charting is neat.
A very heartful thanks to you ma’am for the advise and knowledge that you had shared to us. And you are one excellent C.I. that I had ever met and you are patience enough to us. And ma’am is friendly she always making joke to the group.





III.LEARNINGS:

In our first day at San Lazaro Hospital is our orientation, the staff orient us the different policies of the hospital, different kinds of communicable diseases we encounter at SLH,and also they orient to us what are the responsibilities of the student nurse affiliate in their hospital, with all the days that I was exposed at different pavillion, I’ve so many things that I learned. First, Ma’am Perez teach us to do good soapie and I learned also to become observable and humble to my patient even sometimes the patient and significant others are snob. And the most important that I had learned is how to deal and communicate people especially patient with communicable diseases.

I. OBJECTIVES:

a. General Objectives:

-To establish a great foundation in fulfilling my goals in life.

- To uplift my knowledge and skills regarding the different communicable diseases commonly occurring in our society including its proper managements and interventions to be done to certain diseases.

b. Specific Objectives:

-To develop a good working relationship with my fellow groupmates, patients to be handled, our Clinical Instructor and different staffs in San Lazaro Hospital.

- To be more confident enough in dealing with some situations that I must encounter during our exposure and duty hours in San Lazaro Hospital.

- To practice and put into action the proper approach needed to communicate in different people in San Lazaro Hospital.


II. OBSERVATIONS:

a. Self:

One of the greatest achievements I considered to be an important aspect of my life is being able to experience some things that I will never forget through my entire life. As I think deeply with regards to that thing, one thing that came into my mind is that how lucky I am to be included in the list of students who was able to reach this privilege to have an affiliation in Manila particularly in such hospitals namely: San Lazaro Hospital, National Center for Mental Health and a tour in Philippine Orthopedic Center.

I consider having an affiliation in San Lazaro Hospital a great opportunity for me to learn, to experience new things, to meet new challenges that would develop my sense of well being. It is true, I’m so much happy because I’ve gone this far. But mind me, I can’t be this far without the help of our Dear Lord God. Really God is so good. Through His guiding hands, I have made all our activities in SLH well and good.

A day before we take our duty in SLH, I sincerely talk to God through prayer to ask for His guidance and help us in all our activities that we’re going to do for the coming days of our duties. And everything follows, there I can clearly see the everlasting and unfailing goodness of God to me and to everyone.

Despite of the sadness that all of us students having an affiliation away from our love ones, I realized that it’s a part of it. I should be strong enough and do my best as exchange for the sacrifices my parents have just done for me to go in this kind of affiliation. I know all the sacrifices they have than for me and I’m very thankful for their love, care and for everything they have done.

During our duty hours and exposure in San Lazaro Hospital,
I quietly felt nervous but not that nervous. The activities everyday in the hospital is very challenging. And I do hope that I have given the appropriate care needed by the assigned patients to me for the past two weeks of our duty in SLH. I do believe that I’ve done my part as a responsible student nurse.

b.Groupmates:

They were all very kind and enjoyable companion. Eventhough half of them are first time to be our groupmates, they are not hard to mingle with. They were also very serious in terms of doing their responsibilities as student nurses. Some also are very cheerful which makes us happy and laugh all the time.




b.Clinical Instructor:

Our C.I., Ma’am Nhelia B. Perez is a very kind person. She do have a one of a kind personality. Ma’am always makes us happy, makes us smile, and laugh. Smile is the best medicine, isn’t it? Yet, she always give us opportunities to experience new things. For the past two weeks that we have Ma’am as our C.I., we’ve loved Ma’am for everything. Eventhough it’s our first time to have her as our C.I., we have known her better and we have developed a good relationship to her as a group. I admire Ma’am for being a kind hearted and religious person. We thank you so much Ma’am for everything you have shared to us, your students. You are very approachable Ma’am. Thank you for the knowledge, for the goodness, opportunity and for the unconditional love you have given us. We also do appreciate all the encouragements that you have told to us Ma’am. I do believe that God will bless you more. We’re looking forward for having you again as one of our C.I. as we go through life’s journey.


III. LEARNINGS:

For our entire duty in San Lazaro Hospital for two weeks, I Have learned a lot and all the memories that had came my way will remain in me forever. One of the most important thing that I’ve learned is to trust God for everything and always. The same as being faithful to Him. I also learned how to be very careful in doing such things. How to be responsible enough in reviewing my lessons and study very well and I have learned how to value every important things that comes into my life. I do thank our C.I., Ma’am Nhelia for everything and most especially to God. Lastly, I have learned more not only during our exposure in different areas in the premises of San Lazaro hospital rather we also learned from the lectures done by our two coordinators during our pre and post conferences in the hospital land at the dorm. Especially the prioritize nursing interventions to be done in such diseases that we have encountered including its treatments and managements.

SELF:
 I have the eagerness or desire and courage to learn and practice what I have learned in our lecture in communicable disease.
 I’m humble enough to accept my mistake and give others chance to prove their worth.
 Spend time with my patients and watchers ( S.O.) to express my ideas, knowledge about health

GROUPMATES:

 I observed in my groupmates that there is a intimacy, caring, sharing, getting to know each other and most of all there is openness of problems
 They try their best to work efficiently and effectively
 We work as one( teamwork) with concerted effect to build harmonious relationship in work group
 They show initiative and alertness in performing their duty in the ward.
 They show respect in the hospital staff and personnel and to other people they meet.

 She provides the student with clear and relevant information during orientation.

 I’ve learned a lot in caring patients with communicable diseases
 I’ve learned to be flexible, knowing adult and children varied characteristics and behavior enabled me to reach to their level which my interventions became more effective
 Take every unfavorable experience in the ward as a challenge and opportunity to learn rather than discouragement

Gain more knowledge, information about the different services offered and the organizational chart flow in admitting patient and also patients for check-up / follow-up.

- To help individual to become aware to their state of health and to carry on those activities they must under take from themselves in order to realized the importance of a good healthy lifestytle.
B. SPECIFIC OBJECTIVES:
- To promote the development and proper used of health services to those who have a communicable diseases and to rendered effective nursing interventions, and be competent health provider.

11. OBSERVATIONS & LEARNINGS:
- Here we are San Lazaro Hospital; we are as a student nurse from NC and we are here to prove our self that we can be a effective and be a competent nurse with regards to handling a patients who have a communicable disease. Its now our previledge to prove to prove our self that we can make it our affiliation success. I my self, wanted to prove that I can be a effective nurse that’s why during my duty I always give my best shot to apply my all learnings, skills and knowledge so I take it this as a big challenge to my profession. Our 1st day at SLH was the orientation for as a affliates school will be aware to hospital settings, to be familiar to all health team member and to find out when SLH built. On the following day, we are now handling patient who suffered different communicable diseases so as a health provider needs a protection via using double mask, lab gowns, and when it highly contagious we use gloving. Now as I go on to my career I realize that our profession was at risk because all the patients need, waste we are responsible for that. But I promise to my self who ever I handle even rich or poor I give my proper care that they need. But of course I can’t never ever do that thru the help of GOD that’s why in every procedure, decision I made in the area I ask GOD to BLESS the work of my hands and help me, guide me as I go on because I do believe that he is the only one who can help me. I always pray that he keep me more patience, kind, effective and competent nurse so that my goal to promote a health y lifestyle will reach.
- To stay for 2 weeks at SLH was a big help for me because I learned a lot and I do really enjoy to have duty at SLH. With regards to my group mates they do enjoying their duty. The team work of the group is always present, they also do their task very responsible to their patients and very cooperative to each other.
111. CLINICAL INSTRUCTOR:
- Aaahhh aaahhh.... of course the high light of all my learnings is to give a lot of THANKS to my ever dearest, beautiful, kind, intelligent, smart, jolly clinical instructor none other than MRS. NHELIA B. PEREZ RN M.S.N. Ma’am, thank you so much for all the skills, learnings, knowledge you shared to us and be approachable always because of that we are now proudly say that we can be effective, competent NURSES in the future. You are a good mentor, model to us because you are not just there as our clinical instructor nor you act also as our good friend, sister and mother to us; your one of a kind. Willing to help and guide us all through out our duty at SLH. GENERAL OBJECTIVES:
In order to expose ourselves as well as be familiariazed with the different comunicable diseases in San Lazaro Hospital; moreover, to equipped with knowledge, skills and values in oredr to render holistic care and emphathy to those who need it most.

SPECIFIC OBJECTIVES:
In order to render care to the patients in the different pavillions.

In order to establish rapport with the patients as well as to establish harmonious relationship with my groupmates, CI, staffs and other fellow student nurses

OBSERVATIONS:

SELF - 2 weeks have quickly passed, and i couldnt explain the gratitude of being part of the patients lives during the 2 week duty at SLH. I enjoyed every hour of our stay there, i learned a lot and I appreciate more the nursing profession.Sharing immeasurable care and affection to the patients is such a big help to them not only physically but also emotionally and spiritually. THe experince itself couldt be replaced by anything else because its one of a kind.

GROUPMATES:

I have come to know my groupmates and I thank God for giving me the chance to know them and build a good teamwork and friendship with one another. I appreciate our own weaknessess and strengths,but I appreciate more our common goal to help our pateints the best way we can and aquire skills, knowledge and values that we all know we can use in the future. LEARNINGS:

Patience is very important when we are caring for our patients. No matter how stubborn they are, no matter how they hate and embarrass you, no matter how hard and toxic, no matter how messy and dirty they are, no matter what kind of life they belong......still you have to be patient with them.One more thing, relationship with the health care team and with the patient is very important in order to work better because we can never achieve our best goal if one canot cooperate.

For my obsevation of my self started my first and last duty in the hospital i can managed my time to rendered care and making SOAPIE and also the charting, Im very careful in charting because i dont like got demiritI'm not toxic but in reporting I failed i dnt know what happened to my self Im nervously when reporting so i can't answered the question, i know the answer but icnt express. for my groupmates they are happy and enjoyed while in the duty on the hospital, some others are toxic. for my CI, she is very responsible of us she guided us allways specially when the Dr's round and charting. She is not strictly but still she demerit in charting.
III LEARNINGS
I learned time management is very important. I learned the strategies in the hospital. I learned the screening before admiting the pt. I learned the history of the hospital. Thank you mam for the guidance and teaching us.

Hello guys!
I apologize for interfere in ur blog posting coz i hardly figure it out were can i post a shout out message in this blogger site....anyway i read some post and i end up thinking that this people are very intelligent in a way i vision them very competitive in working in nursing field soon.....as of now the craft that was made form me as Northeastern College Student is a big Break for me as in Hella Bad, I stunned because, im not showing off but im telling the truth in actuality that as being student of NC, im being aware that people who work with me right now is amazed, eventhough im not license yet as a nurse and working as Certifeid Nurse Assistant here in U.S..Some Licensed Vocational Nurse (LVN)who graduated here (US) That im work with always ask me some information in regards in medical term or doing a procedure, i know its a western country and they are more advance in technolgy or so whatever, but we are more brainer than them i guess. there is one license nurse who approach me seek question that i can help her reading the laboratory result coz it does not print on the paper what is the normal value on that specific lab request, unboastfully i answer her the normal value and she made a big smile and compliment on me thats make my confidence boost.the other nurse ask me what do i call or part of the body, in between of the leg near to perineal area, Grion i told her. another example is im with a license nurse answering the patient call from the room because the patient wanted to know why she is recieving that medication (but i forgot the brand name and generic name of the drug), the license nurse cant comprehensively answer the question but definitely answer back here is the drug book and look for it (i mean the nurse is so mean and rude), just to make sense bout the inquiry, i help the nurse by telling what i know bout drug litereture on that specific drug, at the end i encourage the patiente to ask her doctor for more info coz they know more way better than us and the patient thank me togeter with the nurse coz i save her ass....hahahaha!
i mean as u can see some nurses here doesnt know the body part( anatomically) also does not know how to read lab result also answering simple question about drug literiture.........im not racist but the nurses im telling you guys are gradute here as in graduate here in US.......Pilipino like me finish in the philippine make a way more flying Colors....hehehehe(Charing)
im not showing off guys bout just to reflect what ive been and from through the help of NC and ma'am nhelia, ma'am luisa, ma'am gladys,etc.,they just nail me to be a good student and do my assignment well. Without them i cant able to help and function as a good samaritan to the nurse or patient.
just piece of advice always concentrate to ur study and stick what ci telling you just to improve yourself as well to ur grades and well prepare to be a nurse.....for me wish me happiness and better life also sana I can pass the Nclex exam.....

SPECIFIC: To apply the different skills i've learned in the school and in the hospital.

OBSERVATIONS:
SELF: I am proud to myself because I gave my best shot in caring my patient's with communicable diseases with less mistake.

GROUPMATES: At first, i am not comfortable with them because some of my groupmates came from different block. But few days after, i've been comfortable with them because i found out that they are friendly and easy to approach.

CLINICAL INSTRUCTOR: Maam Nhelia is still the same C.I. that i have before...... approachable and joker. She taught us on the different nursing interventions in caring our patients.

LEARNINGS: I have learned the different infos about San Lazaro Hospital and i also learned on how to care patient's with communicable diseases.

OBSERVATION:

Self:
Of course, we acquired some ideas from the 4th student who had affiliation in San Lazaro Hospital to prepare myself. I felt nervous because ma’am Perez will guide us in the area and it was my first time to work with her in clinical area. But as we go on our duties, the nervous subsided into a relaxing feeling. I am glad that I saw the beauty of San Lazaro Church. It was a great opportunity for me to sight and hear the story about the place. I am also proud that we showed to everyone how effective nursing we are.

Group mate:
I knew half of them because they became my group mates before and the rest are new to me. I met different personalities. They were all good and cooperative. I saw a little nervous but at the end they were just enjoying their moment in San Lazaro Hospital. They proved that they can show their talent and abilities to serve what they had learned.

Clinical instructor:
Our Clinical instructor was Mrs. Nhelia B. Perez. She guided us in our duties. She removed away our nervous towards the hospital by making as laughing throughout the duties. She was also our walking NANDA book. She discouraged us to use it for us to be smart and talented in making our SOPIE. My C.I is very strict regarding the charting but she has purpose. She and Mrs. Luisa Y. Valencia made an activity that will help us to be familiar on different diseases.

LEARNINGS:
I’ve learned the history of San Lazaro Hospital which is one of the oldest and excellent hospitals. It symbolizes the real hospital where sick people have a good recovery. From what happened to my group mate, it emphasized the importance of the 5 rights in administering a drug to the patient. My C.I is very strict regarding the charting but from that regimen we’ve learned to be more careful and have focus on what we are doing.
We are all mastered in caring a patient with diagnosis of dengue because this is the most common disease in SLH. We had experienced to have a duty to the patients who have tuberculosis. Although it was very scary for us, we’ve overcome it. We use to do all the universal precaution such as wearing a mask, have an alcohol, take high dose of Vitamin C and hand washing.
We had also our pre and post conference where everyone will has to report their respective topics and case abstract that we have to past every other day, from that we have been familiarized in many communicable diseases.

II. Observations:
a. Self: During my first day of exposure to the area, i was so nervous because it was my first time to be with Maam Perez on the area, but as the days goes by i felt free because she is very approachable person. And i felt sorry becuse my CI doesn't feel good to my report.

b. Groupmates: Almost all are approachable one except to one persom whom i felt insecure bacause of his attitude during our first day of exposure,may be because i am not as intelligent as he is but GOD knows that i did my best.

c. Clinical Instructor: Our CI is very intellegent and approachable person. She does not make us toxic on the area and she is very supportive to her student. I learned to her.

III. Learnings: I really improve a lot, NPI,charting and making of SOPIE without seeing a nurses pocket guide book. I also learn how to manage and how to approach a patient with communicable diseas. My knowledge about the different communicable diseases was added like on how to prevent it.

 San Lazaro Hospital is not familiar to me. At first. I felt nervous and afraid because all I know that, SLH have patient with communicable diseases. The place also not familiar to me because it was my first time to have duty at SLH. When we had our orientation, we didn’t know the different pavilion of the hospital, but when the day’s pass by, I felt more comfortable and it become familiar to some pavilion of the hospital. I enjoyed having a duty their and a lot of experience, I’ve experienced.

B. GROUPMATES:
 My group mates not yet also familiar in the hospital. And some of my group mates got lost during our second day and that is a part of our experienced. They are cooperative, and supportive. Some are funny and some are moody, but we always make our group happy.


C. CLICAL INSTRUCTOR:
 Our clinical instructor was so kind. She always gave jokes to make our group happy. We felt enjoyed together with her. She is very effective clinical Instructor. Before we handle our patients she gave first an idea what is the disease of our patient. We felt comfortable having duty with her. She was very approachable, disciplined, understanding and she cares about us.


III: LEARNINGS

 I acquired a lot of experienced and learning’s when we had our duty at SLH. The hospital where we can found the different communicable diseases. It was my first time to saw a Herman’s sign, which is the pathognomonic sign of Dengue fever. It was my first time also to handle a patient with Tuberculosis. It was very happy to render care with all your hearts, gave moral and spiritual support to patient with communicable disease, because we felt and see their happiness when there is somebody who care’s them. It is hard because their diseases are communicable. So we need to perform the proper assessment and management to protect the patient and our self.

I. OBJECTIVES:

General Objective:
>To gain learnings through actual experiences and hands-on exercises on how to handle patients with communicable diseases

>To know the different concept on how to address the needs of every patient's having different communicable diseases;
>To enumerate some of the communicable diseases to be encountered at the floor;
>To render the proper nursing care towards the client with specific communicalbe diseases;
>To develop a good client-nurse relationship to promote faster recovery especially in performing our nursing care management.

II. OBSERVATIONS:

-SELF:
Assessing one self is hard for me to do however, with the given duration of our hospital exposure I've proved to myself that I can able to comprehend and able to met the deadlines at a given time. I've also realized that eventhough we've came from the province, we are more competent compared to some student nurses coming from prestigiousand well known universities at the metropolitan manila.
In this case, I could say that "student nurses from the provinces has the guts and glory to rich the top...go NCnian!!!

-GROUPMATES:
Realizing the fact that this was our first exposure to especialized hospital like SAN LAZARO HOSPITAL, my groupmates and I has the feeling of being behind compared to other nursing schools. But this feeling turned into a positive outlook when we were able to render our nursing care on time without any problem arise and unfinished task within the shift.

-CLINICAL INSTRUCTOR:
Who could say that Ma'am Nhelz is a type of universal person...she could act accordingly to want kind of people she'd been encountered. I was so amazed by the attitude she'd shared unto us, I thought she's the type of person of being feared off but then this observation turn into opposite side and i'd realized that she's the type of person to whom you can find refuge and comfort...She's one of a kind...who could tell that Ma'am Nhelia can throw a joke...Of course nobody can believe me?!! but that's the truth...She see to it that we learned from every cases we've handled...

III. LEARNINGS
Prior to affiliation, we've been experiencing a great financial constraints @ home. In fact, i'd made a decision of not joining the 2008 affiliation due to some factors. But this epistles of mine didn't happened and as expected i was able to be with the 1st batch of affiliates of 2008.
Ah, so much for that....After 2 weeks exposure to San Lazaro Hospital and have had the opportunity to learned the different routines in the floor especially in dealing with the patients having with communicable diseases I've learned that it is important to know the history and the right nursing care management for certain diseases.
It is also an opportunity to know how San Lazaro Hospital started and how long it exist. Imagine after a hundred decades of existence, still SLH could still render the finest medical care to those patients with CD cases it its optimum level of care.
This experience is a great venue for grwth and development to us student nurses who aims to become a GREAT NURSES in the field of nursing...Not only to serve and cater the needs of people abroad but most espeacially to serve our Co-FILIPINO people.
Lastly, i know that it is a fact that we are all loking forward for a greener pastures, but come to think of it, Pinoy's population are really longing for the right and optimum care. As the saying goes "sino pa ba ang magtutulungan kundi kapwa PILIPINO mo din"...Magiging masaya ka kaya kahit gaanong yaman ka, kung ang nasa paligid mo ay walang-wala sila..???

OBSERVATION:

Self
I experience a little bit of nervous because it is a new affiliation hospital &it is my first time to expose here at SLH. But it is not hard to adjust because it is most similar to QPH but there is some policy only that they are different , in terms of universal precaution SLH is very strict, when in terms in rendering my patient I can say that I
perform we3ll I do morning care & I easily get the trust of my patient .

Groupmates
Some of my groupmates came from the different section but we easily jive each other, they are cooperative helpful & some of them have a sense of humor & in terms of rendering of health care to our patient they do there responsibilities as s nurse.

Clinical Instructor
When I heard that she is my clinical instructor I felt a little bit nervous it because it is my first time to be my clinical instructor I’ve never experience to be my clinical instructor so my impression to her is terror & strict. But all this impression erase she is kind, intelligent, sense of humor. I enjoyed so much to have my clinical instructor. Thank you very much ma’am to all the knowledge that you shared with us. Learnings:
Day I
It is our orientation at the same time tour to the hospital they discuss a brief history of SLH.

Day II
It is our first duty to expose to SLH at the Phil. Health ward my patient is DHF.

Day III
Our next exposure to SLH is at Respiratory Female so all of them is PTB so we need universal precaution I enjoyed rendering my patient at the sane time I learned.

Day IV
Adolescence ward, AGE case my patient is cooperative in terms of treatment.

Day V
Adult male tetanus case it is my first time to handle a patient with a tetanus but he is already on the recovery stage he is already cure but he complain is he need assistance when he move, he is already 1 months on the hospital.

Day VI
Pedia main pneumonia case my patient is a 3 years old it is hard to get his trust because he has fear to the nurse every time I get the vital signs & touch her she always crying.

Day VII
Adult female DHF she is cooperative & I get already his trust.

Day VIII
Pedia extension DHF again so it is not hard to do some intervention because I already handle patient with DHF

GENERAL OBJECTIVES: To develop the essential skills, knowledge and attitudes in the care and management of patient with infectious and contagious diseases.
SPECIFIC OBJECTIVES:
>To develop our skill in assessing patient; making appropriate nsg. Dx; formulating SMART plan of care; implementing proper nsg. interventions and giving health teachings; and evaluating the outcome or effectiveness of care.
>To observe and learn the routine at the San Lazaro Hospital; learn and familiarize ourselves with the different communicable diseases.
>To develop the right attitude when working at the clinical area; develop/enhance our interpersonal skill – in dealing with the patients, significant others, staff nurses, clinical instructors, fellow student nurses and other personnel of the hospital.
OBSERVATIONS:
Self: During our duty At SLH, I felt nervous because we were at a different setting but I am also excited and happy because we were able to take care of patients with communicable disease & able to observe some cases that we don’t usually see at the province. Some/Most of the time I commit mistakes at the ward ... I also experienced having my duty at the OPD of SLH…and I administer rabies vaccine, but I was a little upset and disappointed with my self because I haven’t done the ID injection properly, but may be some failure or “kapalpakan” are part of learning. I tried to do what I can—in doing my responsibilities & in finishing my requirements like daily case abstract and individual reporting.
Group mates: My group mates are kind, friendly and cooperative. During our 1st week we were all adjusting to our new environment although we have almost the same routine in the province. They are willing to learn but some the time we were demerit because of some mistakes in charting. Every body tried to give their best during our duty & reporting. I also learned from them.
Clinical instructor: Ma’am Nhelia is our CI. She shares her knowledge to us, the information or the things that we might need on the area. She is supportive, she guides us around the area; she taught us the proper charting & disciplines us. She is also friendly and most of the time she makes us laugh with her stories and jokes.

LEARNINGS: I learned different communicable diseases and some appropriate nursing interventions for pt. with CD. I was able to see Herman’s sign & patient with Steven Johnson’s syndrome. I learned to improve my SOAPIE charting; I learned that it is better to work when things are organized & It is important to have enough self confidence because it affects the SN’s working ability. I learned to work independently, and to be alert when working, to be able to accomplish the necessary task to be done. I learned to have more patience & to improve the way I deal with patients & other people.

OBJECTIVES:

SPECIFIC OBJECTIVE: To practice dependent and independent nursing care to our patients.

OBSERVATIONS:
TO MYSELF: During our first day I’m a little bit afraid to the reason that I will be taking care of a patient having communicable disease. But throughout the duty I already developed self confidence, to the fact that I know how to protect myself, and so that our c.i is always there for us to guide us. I’ve done mistakes but I learned through it.

TO MY GROUPMATES: It’s my first time to be with some of my group mates but I observed that they are all good cooperative and easy to deal with.

TO OUR C.I: madam Nelia is so good, whenever there is a new experience she’ll let us observe it or she’ll let us do it with her assistance, she’s so strict yet so nice. One thing I learned from her was to have focus in everything you do and just do it by heart. She’s so jolly that’s why I didn’t felt boredom during our stay at san lazaro.

LEARNINGS:
>During our first day that was June 25, 2008, we had a general orientation, we’ve been oriented about the history of san lazaro, the staffs its objective and also the rules and regulations. We also had a tour on the different pavilions.
>During our 2nd day(June 26,2008)we are assigned at the female adult ward, my patient was a 22 y/o woman and her case was typhoid fever, throughout our NPI, I’ve traced when did the signs and symptoms first started, how did she managed it at home until she seek consultation when the symptoms persisted. Our time is very limited we only had two hours for our patient so I just rendered health teachings like how to manage the disease and ways to prevent it. On our third day (June 27, 2008) we are assigned at male adult ward. My patient was suffering from hepatitis. This day I learned about being careful with all the food we eat and how we prepare it.
>June 30 20o8 was our 4rth day, and we were assigned at pavilion 10 male respiratory. I earned how to take care patient having such kind of disease. Today I’ve rendered health teachings like disposing contaminated materials properly and the importance o taking the medicines regularly.
>July 1 2008 was our 5th day and we are assigned at the OPD. We’ve learned about the different vaccines given to patients suffering from dog bite. We also experience injecting the anti rabies through intra dermal injection on the deltoid.
>July 2 2008 this was our 6th day, and we are assigned at adolescent ward my patient was a mentally retarded with a diagnosis of pneumonia and TB meningitis. I’ve learned how to assess patient having such defect, he had no verbal response, so all I have to do that time was to observe, and monitor v/s then referred the abnormal findings.
>July3, 2008 this is our 6th day and we were assigned at the Phil health ward, my patient is suffering from dengue fever. Today I’ve learned how to assess presence of Herman’s sign. >July 4 2008 this is the last day of our duty and everybody is excited to go home. Today we are assigned at the pedia ward in where we had encountered different communicable diseases that were not familiar to us like herpes zoster, measles and Steven Johnson syndrome. We actually handled patients suffering from this disease.

To earn more learnings regarding communicable diseases and practice skills through actual exercises at SLH

Specific Objectives:
To be familiarize with the staff, policies, procedures and San Lazaro hospital itself
To observe, practice and use learned academic theories on real hands-on activities
To develop and render effective nursing interventions on patients
To establish good relationship with my group mates, CI, and patients

Observations:
Self:
I felt so lucky, spent almost a month of beautiful days and just can’t thank God and my mother enough for having me as a part of this Affiliation 2008. Having so much doubt in my mind at first for thousands of reasons, but as days passes by, I found myself enjoying and learning in this affiliation. I faced all my worries and fears on handling patients with such diseases. In addition, I know in myself that I did my jobs and responsibilities great and made every single day a very meaningful day.
Group mates:
Honestly to say, I really didn’t like my group mates at first especially seeing on the list the names of my past enemies. I had a bad first day on our duty and annoyed every minute of it. But I am crazy to say that I love my group now. I made a good relationship with them and I could say that they are so cooperative and very responsible.
C.I.:
I was happy knowing ma’am Nhelia as our clinical instructor. I have known her as a strict yet fun-to-be-with C.I. She helped me realize that I could be a great student on my simple ways. She’s very approachable and gave me learnings regarding academics as well as lessons in life.

Learnings:
I really had a great time living in our dorm for couple of weeks. In that span of time, I learned so many things especially on how to interact with my CI’s. They stand as my mother and helped me discipline myself. I learned how to fix and organize my things, to wake up in the morning myself only, wash my underwears, and many more. In our duty at SLH, I learned the policies and some of the routines and procedures done there. I was also able to make my own pathophysiology of my postcon disease. This affiliation was really a great fulfillment on my part and I can’t stand on the fact that I really learned so many things in this activity.

OBJECTIVES:
a.) General Objectives: To develop & enhance the appropriate & essential attitudes, skill and knowledge in rendering quality health care service & management for patients with communicable & infectious diseases.
b.) Specific Objectives:
 To plan and organize appropriate nursing care plan for patients with communicable diseases.
 To perform activities that are suited for every patient I am going to handle.
 To provide emotional support and appropriate health teachings for the significant others and visitors of the patients.

OBSERVATIONS:
a.) SELF- It is our affiliation in San Lazaro Hospital & as a student nurse having my duty in San Lazaro Hospital I’ve felt mixed emotions like excitement, happiness and of course nervousness. On our two weeks duty on the San Lazaro hospital I have made appropriate nursing care plans and performed such appropriate activities for every patients I have handled. I did my best to do my duty turned out well even for the fact that all our patients have communicable diseases. I have handled patients with typhoid fever, pneumonia, pulmonary tuberculosis, dengue fever and the first time I’ve ever heard the Steven’s-Johnson-Syndrome. As I am handling those patients I have rendered quality health care for them. I have also dealt with the significant others in a nice manner and I established rapport to gain their trust and cooperation. I discovered that to have a better outcome to meet the needs of the patient even in just three to four hours of duty cooperation of the health care team and the S.O. should be always there.
b.) GROUPMATES- Upon arriving from the SLH I can see on my group mates face the excitement for having our duty there at SLH and some are anxious because we don’t know on how to approach those patients with communicable diseases. Before the duty at SLH my group mates and I were talking about what will going to happen in our duty at SLH because it is very different from the other institutions that we have affiliated and what we are somewhat afraid because were going to handle patients with communicable diseases and as clinical instructors always telling us that we must be extra careful in handling them. In our two weeks duty my group mates really participated well. Even though the settings in the hospital are really new to us we tried our best to adjust with the rules and regulations. Even we have made some mistakes, our C.I. is there to correct us and inculcated in our mind that we should be aware on what we going to do or before we do such thing we should think first especially when there are abnormalities on our patients vital signs.
c.) CLINICAL INSTRUCTOR-It is my first time to have ma’am Nhelia as my clinical instructor and the good thing is at SLH. I have heard from the other students that she’s strict on the area, have a sense of humor and not boring to be with. And as I have my duty at SLH, I’ve found out that she is a good one, not that strict because she just want that our duty will be organized and as much as possible no mistakes at all. She made our duty easy by always smiling and giving a joke which we felt that learning is not just being too serious on what we are doing but rather it is the thought that rendering service with a good heart and good feeling. I have really enjoyed my duty at SLH with the guidance of ma’am Nhelia. I know that being strict is just her way to make us(nursing students) a competent nurse someday like her. Ma’am nhel, thank you for being such a nice one, I know there’s a lot of blessings you are going to receive from our Dear Lord. May you continue to guide as we are taking our journey on this profession (nursing). Even if it is only two weeks that I have you as my C.I. I have discovered and learned a lot from you. LEARNIGS:
I have learned a lot from San Lazaro Hospital. Being a student nurse of the patients with communicable diseases is really a great challenge of our dear Lord to us and it’s my honor to have given the chance. I know being a nurse is not just because of wanting to earn more money but the fact that we are saving lives which our God’s master piece. I’ve learned how to appreciate God’s gift to us, our LIFE! We must learn to take good care the God’s gift to us for us to be able to do such great things that are pleasing to him.
At SLH I have handled different cases. On our first day duty in the PAV II ma’am Nhelia assigned me the patient with typhoid fever, on the second day of duty I have handled patient who has Bronchopneumonia, on the third day the patient with pulmonary tuberculosis, on the fourth day we have our at the OPD and it is a really nice experience because I have tried to administer a anti-rabies drug, what a wonderful learning! I have enjoyed doing the Intradermal procedure in actual with my patient. On the fifth day our duty is in the Phil Health and my patient has pneumonia, for our sixth day I have handled a pt. with dengue fever in the adolescent ward, and on the last day I have handled a patient with Steven’s-Johnson-Syndrome that is new to me. I have learned appropriate nursing care plans for the patients I have handled and performed appropriate skills for every case that I have handled. My duty turned out well, I have improved my skills, knowledge and attitudes and I have learned new things.

Generel objective:
to be able to gain more knowledge and skills at San Lazaro Hospital.
Specific objective:
to promote wellness,prevent complications and aggravations of the disease by proper assessment,management,diagnosis and evaluation of the effectivity of care provided.
Observations:
Self:
It is a new experience for me having my duty at San Lazaro Hospital. I'm so eager and very excited but quite nervous because of a new task that I must finish. To render service is a very fulfilling and I'm glad that this will be the start of my new challenge so I must be careful and give my best in rendering service.
In this two weeks of duty improve more my self confident in dealing with the patient with communicable disease.
Groupments:
In this duty they are all exited and nerrvous. they are cooperative and intarested about in every topics we discussed.
Clinical Incstructor:
Maam Nhelia Perez is very supportive and approachable .She teach us everything we need especially to the different disease that we encounterd.
Learnings:
This duty was really learning experienced. Ilearned more on the different communicable diseas,the nursing process that must do in every disease we encountered.

OBSERVATION:
Self:In my first duty i feel nervous and scared because i will care with the patient with a communicable diseases.But after few days i accept because its my own learnings on how to handle/ or render care to patient with communicable diseases...

Group mates:Even though some of my groupmates are not familiar to me,they are very active,friendly and they help each others and willing to learn a new things."for short we have cooperation for each others".

Clinical instructor:
It is my first time to be handle by ma'am Nhelia as my C.I. my first impression to her is she is very strict and moody but as days goes by i found out that she's very kind,at my pagka2log pala" as verbalized by me.she give all her best to make all things clear to as . tnx ma'am for all the knowledge, skills that you sharedd to us.and wish that i will have duty with you again as my C.I..korona"

Learnings:On my first duty i learned the different pavillon in the SLH and the hx of that hospital. on my 2nd day i was toxic on my pt. at pav10. i learned also the different cases that they handle on the hospital like typhoid fever, dandy fever,rabbies,AIDS,TB all that are admitted to give their appropriate care.and i learned also the proper way to make a good SOAPIER. .

GENERAL:To be able to enhance our knowledge in taking good care of the patient in SLH with communicable disease.And to more familiarize us those different diseases in SLH.

SPECIFIC: To implement our knowledge in caring those patient with communicable disease and to gain more learning's on the diffent diseases that it was our first time to encounter.

OBSERVATION:
MY SELF:I was very happy to be part of this affiliation 2008,first i want to thanks my parents who is very supportive to me,because of them i join the affiliation i treasure these and i take this as my inspiration to improve my self especially on my studies.Having my duty in SLH i always think the sacrifices of my parents to improve all the things that i have done in taking good care of my patients.And i do all my best to implement my knowledge in caring of those patient with communicable diseases.

GROUPMATES:We have a good bonding with each other although they are not my original classmates.We are sharing some funny stories,I enjoy their company even though it was our first time to be a group mates.We are cooperative with each other we know our obligations and we do all our best to rendered care in our patient.

CLINICAL INSTRUCTOR:I dont know what to say about my C.I,but im very thank full to her because she is very supportive to us very approachable and very jolly person.Thank you maam because your always there to improve us in our studies, and i enjoy your company.Thank you all the learnings that i gain to you..I salute you for being a good C.I to us we love you maam

LEARNINGS: In SLH I have many things that I learned especially in dealing with those patient ,on how to gain their cooperation and their trust to us in just 3-4 hours.I have learned some diseases that is new to us to handle that we never encountered in QPH.I learned a lot some diseases that shared and discussed by my groupmates and C.I.

SELF: during our first day of exposure in SLH,i felt nervous with mix filling of excitement because this is my first day of duty and encountered different communicable diseases.

GROUPMATES: my groupmates are very cooperative and they are willing to rendered good service to our patient.We help each other in every activities that we do they are also very indulgence

CLINICAL INSTRUCTOR: our C.I maam nhelia were made not to be bored because she is very jolly,to make as always happy all the time although we are all tired in doing in our assigned task,we are always happy no matter there are problems arises,She is also very virtues and coperative,she is always there to guide us in our activities,always willing to explain the topics that we don't understand,thank you maan for your indulgence with us.
