I walk through those doors with pride,
Who's life will i save today?
Someone is waiting for me,
Someone is alive today because of my duty.
Sometimes we cry cause we can't save them all,
God sometimes won't let us interfere when HE calls.
A baby's first breath when he looks at me,
The joy of my first delivery.
The tear i wipe a way with my own hands,
The life ending of a gentle old man.
The night seems so dark and the morning so bright.
Being a nurse you see life in a different light.
Who will i save tonight?
Who will hold my hand during their last breath with no fright?
Who will enter this world on my shift?
How many mothers will greet their babies with a kiss?
I don't know who these special people are
but i will meet them with every call
I will hold them tight and help the pain
I will hold them up when they feel faint.
I will be strong when i am needed
That is my job, I am a nurse..that is my duty.
Who's life will i save today?

PEDIATRIC WARD
General Objective:
• To gain more knowledge in taking care, and giving meds. to a patient in pediatric ward.
Specific Objective:
• To be expose in different problems of the patient specially in pediatric ward
Observation
Self:
• on the 2nd week of our duty our c.i started to assigned us in a 1 on 1 patient, we started to carry out doctor’s order and do soapie and giving and prescribing the medicine to our patient.
Group mate:
• I am glad that my group mates are very nice, specially in times of trouble in the pedia ward “lalo na pagnatotoxic na ang isat isa” we always help each other us to do the work faster and easier.
Class Instructor:
• our C.I mam nhelia guide us in our 2nd week of duty, she never leave us specially pag madami ang meds. Thank you mam..
Learning:
* in our 2nd week of our duty toxic na syempre so this week dami ko natutunan,taking care of a patient giving meds and “yung pagbibigay ng volume per volume” pero hindi ko gaano nakita…but I’ve learn a lot, then i also give different meds like cefuroxime, hydrocor, and nagpaneb din pala ako…thank you mam for the learnings na lege niyong nashashare sa amin…….
* mam ganun parin objectives ko kasi po priority parin po na maibigay ang best care for the patient.

PEDIATRIC WARD
GENERAL OBJECTIVE
>To promote health in the best way we can in the pediatric ward and to acquire knowledge and improve skills
SPECIFIC OBJECTIVE
>TO identify common problems in pediatric ward and more aware of it
OBSERVATION
SELF
>as of now i observed that im improved a lot not comparing to our first week but since i transfered to NC!! i gained a lot of knowledge more than i expected,preparing meds,prescribibing to dr. order,soapie,and everything!!,im now aware to QPH practices.even not getting nervous
GROUPMATE
> a lot of my groupmates are still coping up with the hospital seeting
some are really good, having them and seeing them makes me proud they are my groupmates,so nice and(walang laglagan,tulungan)
C.I
>Some ask me how is to be under NHELIA PEREZ??their impression makes me lough,they just dont know how is it to have a CI like her everything she teaches to us stays in our mind,yah her strategies of teaching is so effective that we are learning a lot even, shes making joke,there is still the presence of respect
LEARNINGS
>This week i learned a lot,my activities are appreciated,it is so nice that someone says verygood to what i do,and i learned that sometimes(wag padalos dalos,,hahahaah)but im not saying that slow but sure its bettr that fast and sure(di ko makakalimutan ang humabol kung demerit!!wahahahaha)but telling it seriously i take it positively having a lot of demerit will not make my performance poor but rather better(kahit n may demerit n ko di ko pa rin makalimutan na napagsabihan ako ng verygood sa mga nagawa ko,volume per vol,skin test,prolonged life of my pt.during hte shift)haahahaha

I.OBJECTIVE
a.General Objective:
>To be able to know the importance ,responsibility and duties of student nurses in rendering care for pt.
b.Specific Objective:
>To know the different diseases usually occurring to children so that interventions will be provide to pt. for faster recovery.
II.Observation
a. Self
>I am not confident in giving meds especially how to compute but i know how to carry out Dr's order & prescibing meds.I helped my groupmates when they ask question from me.I was able to expose my self in hospital setting esp. at QPH.
b. GroupMates
>Some are to slow in carrying out Dr's order,but as a beginner they still coping up in many ways.
c. CI

III. Learnings
>Being a student nurses, I do hope that our CI will not expect a lot from as .We are just a novice in gaining knowledge and skill. But as i continue to explore I was able to add learning's that I will apply to my work for the future. I learned how to do my SOAPIE well, improved my knowledge & skill like computing drugs and charting,rendering and providing quality care w/ my pt,being patience in whatever challenges that may come,managing my time and lastly I am so glad bec. every pt. that i was handling ordered MGH.HAHAHA..
GENERAL OBJECTIVE:
To gain more knowledge,learning and experience that could help me provide a good nursing care.
SPECIFIC OBJECTIVE:
To make an effective nursing care plan that suits to my patient's problem and to give continues care while patient is staying in the hospital.
OBSERVATION:
SELF-At first I became sad because I handle again the patient I have already handled last week and it came up to my mind that I was not an effective in giving care, so I need to think of another nursing care plan that will be more effective and will suits to my patient's problem. For the second day, I am very happy because within my shift, the doctor ordered that my patient may go home after her 2pm and 4pm medicines. That day, I was so tired because there are many things to be done, behind all those things I can not resist the happiness I felt here in my heart. For the last stay of my patient in the hospital I give my best care and all the health teachings to the mother of my patient so that continues care will be given and instructed the proper dose and time of giving her child's home medicines and its importance as well in completing those medicines.
GROUP MATE-Our group was divided into three clusters. One group is under the supervision of our CI and the other two clusters are under the supervision of our two seniors which is applying their leadership training. All of us enjoyed the second week of duty but some are toxic and tired of making our case abstract. We help each other in times of need, some were supportive and encouraging.
CI-I enjoyed my CI's company she is supportive and easy to approached whenever we got problem or there are things that we can not understand.
LEARNINGS:
This 2nd week of duty, my learning are: Monitor v/s every hour, giving bedside care, receiving IVF and regulating it, carry out doctor's order, writing on the medication sheet and medication card the new ordered drug, giving nebs. drug, giving IV meds., assist the patient's S.O. in the discharged process, the papers to be filled-up before going home, observed the removing of IV of my patient.

In order to know how to carry out meds and think for the intervention that i can apply for my patient..
Specific Objectives
to share my loving care and to know how it feels in handling sensitve patient.
observation
self- not feeling good for the rest of the duty, feel nervous, sometimes happy and confident in handling the patient but still there is a fear inside of me.
groupmate- they are all friendly and "marunong silang mkisama", sometimes they are concerned to me..,and you observed that they are dedicated to help somebody, specifically ma'am ramones.
C.I- She is strictly monitored our work. on how we used the chart of our patient, and she always test the capability of one studtent, she is observant.
learnings
this was my learnings:

-i enhanced my knowledge to make
SOAPIE.
-Decision mking is very important
to me.
-keep clean and avoid "minimal
erasures in the chart.
-you must good in spelling.
I.OBJECTIVES:
A.GENERAL:
To gain proficiency in rendering holistic care to a patient.
B.SPECIFIC:
To be able to function more effectively as a student nurse without the close supervision of our clinical instructor.
II.OBSERVATION:
A.SELF:
This week’s duty was more stressful than the first one although we only had our duty for 2days.
I totally crammed the whole night before we passed our requirements for the following day, and what distressed me much was that I forgot to bring my drug study with me which was supposedly collected in the morning.
Well, so much for that not only did I learn something in the ward, but I also realized that I should be more organized with my things not only in school but also in our house to avoid instances like that again.
B.GROUPMATES:
My group mates and I were divided into 3 clusters to be handled by 2 senior students and our very own clinical instructor.
Nothing unusual happened with our group because most of our clients has been ordered “MGH” compared to the cluster 1 who were more “toxic” because they have lots of meds to be given during our shift and the other cluster who I think had prescribed new meds also.
Overall, I think they all enjoyed our duty last week even though we were all deprived of sleep.
C.CI:
Our CI is Ma’am Nhelia B. Perez, RN,MSN, she’s a good supervisor and she teaches us everything we ought to know while we are in the ward.
III.LEARNINGS:
I learned that as patients, they are still entitled to be given care while they are in the hospital’s premises even though they are supposed to go home already.
Also, I came to know that it’s really hard to auscultate for the CR of a patient who has crackles and I really had a lot of trouble with that one.

General objective:
To gain more knowledge and how to apply the knowledge in caring the patient especially our patient is 0-12 years old.
Specific objective:
To help the patient how to prevent and what is the treatment of the disease.
Observation:
Self:
I observed to my self that every start of our duty I felt nervous especially in doing SOPIE, and computation of drugs and also in prescribing the drugs.
Groupmates:
They are helpful especially in drugs computation .
Class instructor:
She is a good CI because she never leave you if you are toxic especially in giving meds and in doing SOPIE she help you also.
learning:
I learned how to removed IVF in pedia and I help also the mother of my patient in billing and I learned also how to prescribed a home meds because last dec.5.08 in our duty my patient discharged in good condition at 10:00 a.m
Learning Feedback Diary
General Objective: to be able to know what was the appropriate care that we will render to our patient especially pedia patients.
Specific Objective: to be able to acquire knowledge and to practise my skills in caring my patient.
Observation
Self: I feel nervous because it was my first duty in the pedia ward although it was our 2nd week. Im afraid of making mistakes.
Groupmates: They were so supportive and willing to help, they were so friendly.
Clinical Instructor: she is easy and enjoy to be with. She was strict but I know that its Her way For us to learn.
Learnings:
> I learned how to perform a skin test in a actual patient.
> I learned how to compute the right dose that will be given to my patient.
> I learned to know what was the importance of reading.

Pediatric Ward
General Objectives:
-> To learn more knowledge about different cases in pediatric ward that we encounter
Specific Objectives:
-> To render good health service to the patient and to know the importance of the drug we administer.
Observation:
Self: I observe I improve my skill especially in carrying out the doctors’ order.
Classmate: I observe that most of them improve their skill and knowledge.
C.I: Is very strict in checking sample charting and she give her very best to share her knowledge to make us learn what is better in rendering patient and in carrying the doctors order. tnx madame!
Learning:
-> My learning are, i should have to study more about drugs so that I have the knowledge when my patient ask me what drug is for.. And gain a lot of learning especially in carrying out the doctors order..thats all ty ^^
