general objective-to know that pedia is one of the most difficult pert of the rotation of duty.

specific objective-to learn more about infants.to gain knowledge how to handle them.to be aware that infants are hard to handle.


observation-

classmate-they are so hesitant to make the move,some are confident enough,some cant just adjust to the new environment.

self-im afraid in doing my actions but i can see im improving a lot because of my fear to our clinical instructor that makes our duty more interesting.

CI-she is so strict,every time we see her our knees shakes on fear hence shes so jocund that through our fear we become more confident to learn

learnings-from the beginning i learned to fix my things accordingly.have proper grooming and to learn a lot like changing vol per vol.improve charting,and i know for the next days i will still have time to learn more.

Objectives

General Objectives:
To be able to familiarize the
different protocol of the hospital.

Specific Objectives:

To be able to render health care to the patient in the pedia ward and to gain more knowledge and skill in duty.

OBSERVATIONS

Classmate: I observed that all my groupmates are very nervous because we are now in the climax of 3rd year level on duty and also because of our C.I, however we are all confident on our duty.

Myself: I am curious and nervous because i expect new learnings and challanges in my duty.

C.I: She is very stict in her student nurse especially in cheking parafernalias and those student who came late. She instruct her student very nice and clear.

LEARNINGS

My learnings is I should always be ready in rendering health care and always in complete parafernalias. I learn a lot compare to my past duty. I gain more knowledge and skill.. ^^

PEDIATRIC WARD

General Objective:
• To gain more knowledge in taking care to a patient in pediatric ward..

Specific Objective:
• To be expose in different problems of the patient specially in pediatric ward

Observation

Self:
• on the 1st day of our duty I was nervous specially when our c.i oriented us and saying that (pagdating sa ward bawal na mgtanong ah)..
on the 2nd day of our duty our c.i says that Basic muna tayo…..then we were required to passed a case abstract at the end of the day.
On the 3rd day of our duty mam nhelia give us 1 on 1 patient we do documentation and charting.
Group mate:
• I am so happy with my group mates, because we help each others in the ward and teach each other with the thing that we don’t now.

Class Instructor:
• our C.I mam nhelia help us with the things that we don’t know,she explain to us different things that could help us.

Learning:
* in our 1st week of duty I’ve learned a lot specially with the experienced that our C.I is giving to us. Ang sarap ng feeling when you give your best to your patien.

LEARNING FEEDBACK DIARY

I. OBJECTIVES

GENERAL OBJECTIVE: To know new learning and to apply the things we already knew in caring pediatric client.

SPECIFIC OBJECTIVE: To establish rapport not only with the significant others but specially the patient as well, whatever age bracket and to give the best care they needed.

II. OBSERVATION

SELF- During the first day, I felt nervous and excited, Luckily we did not have yet our duty, but we had our orientation to orient us the things we need to know before handling patient. I was shocked with our surprising quiz after our discussion, and I find it quite difficult but I enjoyed our first day. During the second day, it is a tiring day for me, because we already handled a client, and I observed that it is really a different thing when handling pediatric patient. the third day, I handled a 1 month old baby, and i will be honest that I got nervous and tense while handling her.

GROUP MATES- We enjoyed our first day of duty because our CI give us the privileged to do the things that we our not supposed to do during the proper duty, though we are all shocked with the surprising quiz we had. Some of us, almost got many demerits because of not bringing the complete paraphernalia, but it is alright, for us to learn and follow the rules. On the third day, we already did carry out the doctor's order and prescribing medicine with our client, some of us got toxic, but we are happy because we pass it through.

CI- I like it when our break a funny jokes, because it lessen the tension we had in our minds, though she is really strict, the way we wear our uniform, in checking the paraphernalia and an passing our requirements in exact time, I think, all she wanted is for us to learn.

III. LEARNING

I learned the parts of patients chart, where and how to document the data s taken from the client, the proper steps to follow when we carry out the doctor's order. I also learned how to order a laboratory request like serology, urinalysis, ultrasound and x-ray, I also learned the proper drug computation and IV computation and regulation and I learned that proper use of time is really important thing every seconds we waste is a useful time in doing important things.

OBJECTIVE:
General Objective:
To be able to know and identify the real responsibility of an student nurse in handling & providing adequate care to the sick especially to the children 0-12 yrs(pedia).

Specific Objective:
To be able to know the diff procedure performed to pt. gettine acurate v/s,regulating IVF, computation of drugs, charting & most of all assessing the health of the pt.

OBSERVATION:
A.Self
Im afraid to our CI but as time goes by everything is getting ok.I asked question whenever i am doubtful.I think i can surpassed all the challenges.
B.Classmate
They are helpful, they share their knowledge & recommends whenever the group were discussing.
C.CI
Our CI is knowledgeable enough to share her experiences w/ us,strict & fair to our group.She supervise us in everything we do,she"s so nice & joker that"t why i like her very much.

LEARNINGS

My learning's for the whole week are:
1. changing vol. per vol.
2. improve charting
3. computing drugs
4. I learned the steps on how to make lab. req. & to carry out Dr"s order.
5.sentence for the week "Paging IW on duty to pedia ward please."
6. I learned how to manage my time

I.OBJECTIVES:
A.GENERAL OBJECTIVE :
To become a responsible and efficient student nurse with a minimal supervision from our clinical instructor.

B.SPECIFIC OBJECTIVE:
To be accustomed to the nursing procedures at Quirino Provincial Hospital’s Pedia Ward.


II.OBSERVATIONS:

A.SELF:
During our first day, I was glad to know that the whole day was scheduled for our orientation only, and the second day was a good start for me since I was assigned to a 4 year old girl who can only understand Tagalog unlike the other kids in the ward who were all Ilocano including their guardians.
Overall, I was getting along with our duty pretty well for the first week, even though I got a lot of demerits because of my incomplete paraphernalia during the second day and I wasn’t able to complete my requirement particularly the PA during the third day.

B.GROUPMATES:
My group mates are all amiable and I know most of them already so I really don’t have any problems regarding my relationship with them. It’s been great having them around especially during the last day of our first week where we’ve been all busy doing our best to address each other with Miss/ Mr. since we are not used to calling each other with much formality.
Also, I think they were all doing well during our first week of duty, although some of them were a little agitated because of the condition of their patients who happened to be a little bit serious for us novice to handle.


C.CLINICAL INSTRUCTOR:
Our clinical instructor is well-organized and I know well that it’s a good example to follow especially for us aspiring nurses as she had told us on the first day of our duty.
She is also strict especially with our uniforms and conduct but it’s all right with me because I know it’s for our own good also.

III.LEARNINGS:
I learned that taking care of a patient, especially those under the toddler stage could be very difficult as they are more inclined to throw temper tantrums especially around an unfamiliar environment.
They could be a little bit annoying at times too, but despite this, they are still cute little children who only like to play and don’t want to get hurt so as their care provider, we should do our best to understand their behavior.

A. I have observed to mt self that i am nervous because its the first tome that i will be handled by maam nhelia

B. Some of my groupmates are not prepared because there are things that they did not bring

C. My C.I. prepare as before going to our patient. she have shared information that we can use

OBSERVATION

Self: during my first day of duty, i feel nervous because it was my first time to have a duty on pedia ward.


Groupmates: on our first day, my groupmates too were very nervous, they were so friendly and they were approachable.


C.I.: she is strict but we know that in her way, we will learn. she is approachable and friendly.

LEARNINGS
> i am able to know what are the new parts of a case abstract that were able to pass during friday.
> I learned also that being late is a bad....hehehe but it serve as a challenge to me..

I.OBJECTIVES:

General Objectives:
• To recognize the importance of the nursing process in rendering care for the patient having different illnesses regardless of their race,beliefs,principles lifestyle and status they belong.

Specific Objectives:
• To gain more knowledge in the care management
• To enhance more my skills in providing quality care
• To become more aware in the medical setting where I was assigned

II.OBSERVATION
a. Self:
As I enter QPH,it is an extraordinary feeling is in me,a little bit anxious but excited in the area specially to the the challenges that I may encounter which is really contribute to the field I’ve choosen.

b.Groupmates:
Honestly,I feel uneasy to my groupmates because it is my first time to be meet them and being with them.But as I mengle with them, I really appreciate their being friendly and approachable.

c.Clinical Instructor
Having our C.I. is a great previledge in guiding and assisting us in the said area and for the first three days of our duty, I learned a lot of things from her.Her
sencirity in teaching us really motivates me to do my part as a future health care provider and that is to excell more and more in every medical exposure.

III.LEARNINGS
During our duties,I’ve learned how to make properly the so called case abstract by means of actual assessment and interacting to the patients being assign to me,though it was my first time to work as individual,I consider it as a challenge which brought me into another level of my learnings in the medical field.

My observation to my classmate they are all kind and generous and they are helpful because if you need help they help you especially in doing sample charting

Self
I nervous and afraid because we have a new CI and she is strict.

CI
She is strict especially in uniform and also in charting.

learnings:
I learned how to change the IV fluid of the patient. At first I felt nervous because it is my first time to change IV bottle and I was also afraid.

EXPECTATION / OBSERVATION COZ I DID'nt attend the duty last week mam..,, and i trIED to make LFD IN ORDER TO HAVE A GRADE MA'AM AND I HOPE FOR YOUR CONSIDERATION MA'AM...

SELF- still friendly and trying hard to cope up those i missed

clssmte-i expect that they are more frendly and happy.

C.I- she is unique because she knows how to make a bonding between student.. obsrevant and she is always there to guide us...

general objectives:
To render effective and efficient care services through applying various intervention.

specific objective:
To observe and learn different procedure in the pediatric ward.

observation:
Self:
Being not on focused and does not anticipating my client needs,that was exactly I am last time we had our duty.I feel so embarrassed but i learned a lot of things.

Groupmates:
my groupmates are great help for me to learned and performed in the ward.Though they have their own priority during our shift i can still approach them whenever i need their assistance.

Clinical Instructor:
my C.I wants as to learned in every every exposure that we had our duties,she makes sure that were doing well our assigned task,she kept on reminding us in the area so that we could function effectively.


Learnings:
Our 2 days duties,i experienced being corrected by my superior in such unpleasant way for the mistake i've committed but then i realized that she just want me to be attentive enough and learned more about my responsibilities during our shift.In addition,i learned to obtained accurate information through communicating to the S.O to trace why they have such illness or diseases.
