Learning Feedback Diary
General Objective:
To gain more knowledge in rendering care most especially to the patient's of Pedia Ward.
Specific Objective:
To do appropriate nursing diagnosis and interventions.
Observation:
a. Self- Im still quite nervous but I can do the task that was given to me.
b. Groupmates- they are helpful, and some are responsible enough to do their task with minimal suprevision of our C.I.
c. Clinical Instructor- she's nice everytime we ask something, she always answer our question and explain it well.
Learning:
In our second week of duty, I have learned how to compute the remaining IVF and how many IVF or meds we need for 24 hours. I have also learned how to deal with clients/patients in Pedia Ward.
I.Objective:
A.General Objective-to be able to perform the right care especially to Pedia.
B.Specific Objective-to be able to acquire knowledge to any diseases especially to Pedia cases.
II.Observation:
A.Self-the "nervous effect" lessen,not like last duty.I gain slight confidence to my self especially when approaching the SO of the patient.
B.Groupmates-some of them knows what their doing and some still nervous.
C.CI-shes always there to guide us.
III.Learnings:
i learned the processes of discharging a patient and recalling of proper computation of IVF and my communication to children was improved.

OBJECTIVES:
General Objectives;
To be able to study the client's conditions, attitude as well as the health status.
To be able to provide care in the patient under pedia.
Specific objectives;
To be able to provide plan and formulate proper care, and gain trust to the client, and S.O.
OBSERVATION:
A.Self-i was quiet nervous to handle a patient but i overcome it.i ws very careful how to call my groupmates to avoid having demerit.
B.Groupmates-they are energetic,funny,joyful in providing care.
C.CI-she was a joker thus for us.She guided us in the area however,she was carefully watching us in how we call our groupmates.
LEARNINGS
-how to gain trust in pediatric clients
-set and give appropriate intervention in pediatric client who having AGE.
general objective:
to provide care to pediatric client.
specific objective:
to gain knowledge and experience in hospital settings
to give appropriate intervention to patient who suffer disease of UTI
Observation;
Self-i watch all my words before i will call their name to avoid demerit.
Groupmates-their good and nice because they are real person,no one is plastic and we help each other.if there is one needs help.
CI-she guiede us in every procedure.she gave us some idea on how things work in hospital especially on pedia ward.
Learnings
i learned how to assess client in pedia through the help of our CI.
GENERAL OBJECTIVE
to gain knowledge about my patient disease.
to perform proper nsg.care.
SPECIFIC OBJECTIVE
to know what the real cause of yhe disease of my patient.
to perform proper intervention and proper giving of drugs
to perform proper discharging of patient.
OBSERVATION
SELF-slightly nervous and confused because its my first time to discharged a patient.
GROUP MATES-some also was nervous and very busy in carrying out of doctors order.
CI-approachable and always there to guide us.
LEARNING
i learned that even if you have a partner that is in higher level you should not always rely to her but just follow your instinc.

OBSERVATION
SELF-Im easily adjust myself in PEDIA WARD,including the routine and also our plan of activity.At first I thought it was so very hard to handle children,but now it is easier for me to handle patient in PEDIA than other ward that I have been experience in my past rotation
GROUPMATES-all my groupmates are aware to our time,didnt waste time for things that are irrelevant.they are easy to approach,cooperative,very jolly and most of all they are very kind.Our group is intact with each other,as in were close to each other.
CI-very smart,Im very thakful because youre unique to other CI.unique in terms of strategies to the patient,even handling youre student,even youre approach to other staff.Youre so kind and happy to be with Ma'am.
LEARNING
Im so happy to have an experience in doing the ward class. I have learned that before we leave the ward be sure that we document what we have done.Learned properlyon how to reffered lab.result.
Objectives:
General: > To obtain knowledge about actual procedures in handling the stability of patients and to gain more experience and skills.
Specific: > to continue rendering quality nursing care and to be able to share our knowledge by conducting a ward class.
Observations:
Self: I'm still nervous at first but still i do my job and presence of mine is still there.
Groupmates: they are good listeners and happy to be with.
C.I: she always share his knowledge and answer our question very well
Learnings:
I've learned on how to compute the IVF and being patience.

>to be able to know how to do a Case Studies and Case abstract properly. To know the meaning of Urinary tract Infections, and still to do our duties and responsibilities in Pedia ward.
B. Specific Objective
>To know the management and interventions for our patients. to do things equally partners.
II. OBSERVATION
SELF
> when Mrs. candelario handled us. there's one problem and situation that until we go home still bothering my mind, I admit that i am too weak in terms of computation and analyzing one situation. when i compute the rate flow i got wrong answer. in taking Traditional chart procedures and interventions i forgot to put it in.
GROUPMATES
>They are cooperative especially in doing our different cases and studies. they share new definitions new meaning of there cases. Through join forces the assignment and tasks of partners became more easier and presentable.
Clinical Instructor
> She stiil guiding us in everything we do. She help us to improve our skills and knowledge to be use someday in our future. Mrs. Candelario gave us a new ideas and knowledge too in pedia routine like in doing tradional in charts.
III.LEARNING
> The right definitions and meaning of Urinary Tract Infection, the pathophysiology, the risk factors that affect Individuals, the procedure and diagnostic to be use and the interventions that we need to prioritize. I can say that im improving in Pedia ward because of doing the routine properly and accurately. i have no demerits in these week.

> To Establish rapport
> to render morning care
> To Carry out Dr.s Order and get v/s on 10am and 2pm
Observations:
Self: I am excited to learn new things in pedia ward.
Groupmates: They are easy to be with and very cooperative.
C.I.: Our C.I. is fair in all aspects and we learn a lot from her.
Learnings:
> I learn how to deliver a report in a ward class. And I have gain self confidence in having a duty in pedia ward.
OBJECTIVES
General Objective:
To master the nursing routines at Pedia ward as we are having our duty at QPH Pedia Section
Specific Objectives:
• To know the drugs(its action, side and adverse effect, and the nursing responsibilities) usually recommended by the Pediatrician at QPH
• To gain more clinical experiences and learning in such a way that we may used or applied these interventions even outside the clinical premises
• To maintain hydration of the client by regulating the IVF according to the doctor’s order and by computing IVF that will be consumed within the shift
• To do documentations correctly
• To carry doctor’s order consequently
• To assess client through history taking, P.E., and refer to lab. Results
• To conduct a ward class and provide accurate answers from their questions
OBSERVATIONS
Myself: I was stressed during this week. I did case abstract and we conducted a ward class wherein we have prepared good visual aids and prepared ourselves in order to deliver it successfully.
Group mates: They are doing their works respectively and are participating in all group activities and discussions. They have minimized their voices when laughing or talking each other.
CI: Ma’am Perez has sense of humor. She always shares jokes with us to evaporate the madness on ourselves. When she speaks, all the things she wants to express are clearly absorbed by us. She is intelligent and a good C.I. Ma’am Candelaria is a strict one. She wants all the actions we are doing is almost perfect and should be organized. She is cheerful and very professional.
LEARNING
• I had discharged a patient again. It should be organized and well did.
• In order to maintain the hydration of the patient, I have learned that I should compute the IVF to be consumed within the shift and regulate it very well.
• I have to be patient to my client and gain his trust in order that the whole shift will be fine for us to have good relationship and works will be working smoothly for me.
• I have to empathize the clients and not to sympathize with them.
• “Worked documented is worked done. Work undocumented is worked undone. “
Learning Feedback Diary
General Objective:
To be able to provide care management,teaching and health promotion to the pedia ward.
Specific Objective:
To be able to continue performing assessments and nursing interventions to a pedia patient.
Observations:
a.Self:
Still adjusting myself but I'm enjoying any work given to me.
b.Groupmates:
They are jolly fellows and willing to learn too.
c.Clinical Instructor:
I love her style.If theres something we do not understand,she explains it well to us.
Learning:
Within 2 weeks duty in the pedia ward, I improved in charting and
rendering proper care to my pedia patient.
