OBJECTIVES

GENERAL:
To increase our understanding about a certain disease. and to concretize the theories that we have learned. Learn the value of caring others especially our clients.

SPECIFIC:
• To broaden our concepts about communicable diseases.
• Enhance our skills through rendering quality service to our client to become an effective nurse someday.
• Learn how to mingle to other people with different personalities.


OBSERVATIONS:


SELF: I felt excited but a little anxious maybe because to my new environment and to my clinical instructor. It is my first time to have MA’am Nhelia as my CI so I really don’t have any idea on how she approaches her students. I knew that I need more knowledge and experiences in order to become an effective nurse someday. I’ve learned many things and help me a lot in our 3 days of duty at San Lazaro Hospital.


GROUPMATES: I observed that they also felt the same emotions, but I am happy because through this affiliation we learned to help each other, and work as a group. It also increase our understanding about the significance of” oneness”.


CLINICAL COORDINATOR:

As what I have said, it is my first time to have Ma’am Nhelia as my CI so I don’t have any idea on how she approaches her students. We are very thankful that she is considerate and jolly. We learned to laugh to our mistakes, but inculcated to our mind in order to avoid committing errors again in the same situation.


LEARNINGS:
“Learning does not take only in one place”. To be a competitive nurse someday we should have the knowledge, skills and attitude so there is a big impact of related learning experience (RLE) to us nursing students. We need to concretize the theories that we have learned not only in the community but to the hospital setting.
Exposure to San Lazaro Hospital gave us a great opportunity for us nursing students, to experience and handle clients have highly contagious diseases such as PTB and PTB Meningitis. Through the theories that we have learned in our CD subject about the mode of transmission, incubation period and period of communicability of a certain disease, we applied the safety precaution like wearing our mask, gowns and surgical mask and the proper hand washing in order to avoid cross contamination.
There are many negative criticisms that we have heard from our Clinical Instructors but we accepted it positively and considered it as a challenge to study hard because we know that it is for our own benefits.

II. OBSERVATIONS:

a. Self
My first exposure to San Lazaro Hospital, I felt mixed emotions. I am excited yet a part of me was nervous. I am curious but a part of me was afraid that I could acquire a disease once I've entered the hospital. Also I felt happy that I made it to the level where I will now have my affiliation.

b. Group mates
I could see the excitement on their faces but I know they were also nervous inside like me. They were fun to be with and very cooperative in everything that we do during our duty.

c. Clinical Instructor
She was very supportive and jolly to be with. All through out our duty, she was there to assist us in formal teaching through our pre-conferences and supervise us in our clinical practice. Being with her makes my duty very enjoyable and memorable one.

III. LEARNINGS:
As we enter the institution, my first reactions was.. whooaah! Is there an outbreak in here? What I can see all around were people wearing those laboratory gowns and face masks. Then I realized, this is a hospital of communicable diseases., silly of me.. I almost forgot.
Although I already have foundations on communicable diseases from our past lectures, I cannot say that I am ready and confident enough to face patient's with communicable diseases. Listening have a big difference from experiencing. That's why my exposure to S.L.H. was very important 'coz I believe that clinical experience is the core of the nurse education.
Our five days experience to the institution, I have learned and observe a lot of things. I was so glad that the patient's and the S.O. were so very kind and cooperative. Some patient's still manage to smile despite on their conditions. They were open to answer our queries so we did not encounter any problems regarding with our data gathering.
I have learned that nurses exposed to this kind of institution was not that very easy for they are in contact each day with patients who are in incubation period of development of communicable diseases and were closely associated with people who are carriers of disease-producing-microorganisms. For this they have to protect themselves and safeguard other patients by observing principles of aseptic technique in practically all nursing procedures.
This learning experience is something that I would carry wherever God will send me to apply my chosen career in the future.

Thank you ma'am for everything, for your convincing words of wisdom & encouragement. May you have more students to help in reaching their goals.

To have a knowledge regarding to the different cases of non communicable and communicable at San Lazaro Hospital.

> To be able to know the leading cases of communicable disease at San Lazaro Hospital.

> How to communicate or socialize with other people especially the patient.


OBSERVATION:

SELF:
Im glad that i have an duty or expose to those patient diagnose communicable disease,because i can apply what im aquired during our CD lectured especially the diff. or immediate nsg. intervention in its cases.

GROUP MATES:
They are all very cooperated during our duty,and they maintain discipline in the clinical area.

CLINICAL INSTRUCTOR:
>She is helpful to us,and have sence of humor.
>Good lecturer in the clinical duty.
>At last she train us how to become a confident and good nurse someday.


LEARNING'S:
San Lazaro Hospital is the only one institution in the Philippines admitted with the diff. kind of communicable and non communicable disease.As i entry and conduct an duty i feel so nervous because a lot of communicable disease admitted in these hospital.At last i learn a lot during our affiliation at San Lazaro hospital, i saw a pt. with PTB and i learned how to render or give immediate nsg. intervention with these kind of patient,and i saw a pt. with a petechial rash that is the patient dx. a dengue,and at last how to make a good pathophysiology.

I. OBJECTIVE

GENERAL:
To broaden up my knowledge about communicable diseases and to evaluate how far my understanding is.

SPECIFIC:
.To learn the proper way of rendering a quality care to my patient
.To teach my patient some ways how to avoid transfering the disease to another person.

II. OBSERVATION

SELF:I felt exited at first but a little bit nervous also.It was my first time to be on ma'am nelia's supervision.Even though its very tiring because of the requirements,im still happy because i've experienced having a duty in San Lazaro Hospital.

GROUPMATES:Some of them felt the same as i do but some are just relaxed.Most of them were very cooperative and helpful but some of them also were not that helpful.

CLINICAL INSTRUCTOR:
She's very nice and cool.We are very thankful because we have given the oppurtunity to be on her supervision.Sometimes we commit mistakes but she's not beeing angry with us instead were just laughing on it but we're putting it on our mind as a lesson and to avoid commiting same mistake again.She's a strict but jolly CI.

III.LEARNINGS
Exposure to San Lazaro Hospital gave me a lot of learnings.Learnings about the different communicable diseases,their mode of transmission,incubation period and even the period wherein this certain diseases is highly contagious.I,ve learned also how to protect my self during my exposure to different kind of patient with differenr diseases by using the different parameters like using mask,gowns and etc.I've learned also how to communicate with my patient without making them feel that they have this certain disease that is highly contagious.I,ve learned to be a good student nurse.

General:

>To be to familiarize in the hospital setting

OBSERVATION:

Self:
It was my first time of having duty at San Lazaro Hospital in Manila last July 27-31,2009,i felt nervous because all patients confining in this institution was very infectious.During the duty proper i was anxious to their charting and i want to avoid some mistakes.I have self-confidence in dealing with my patients even some was irritable and sensitive to talk with them.

Groupmates:
They felt also nervous and very avoidant in dealing with their handled patients.I observed their initiative,being responsible and cooperative.I also observed their eagerness to learn in the area.

Clinical Instructor:
To our dear instructor maam nhels,thank you for your spending your little time to us at San Lazaro Hospital.Thank you for your sharing your knowledge,your kindness and encouragement to learn more.I appreciate your being with sense of humor,you made us happy everyday by sharing your funny jokes.

Learnings:
For 3 days we've stayed in San Lazaro Hospital,i learned how to handle a patient with an infectious disease and how to precautious.I appreciate the actual view of patient having a TB disease and the patient with sypmptoms of petechial rashes and urticaraial rashes of having a dengue hemorrhagic fever disease.I learned fastly in charting even it was new to me.I learned how to manage my time and i built up more my self-confidence in dealing with other people.

To apply the theories I have learned in the school (communicable Diseases)

To provide appropriate information and health teachings about the present condition of the patient, and to render quality nursing care.

Observations:

Self:
I felt nervous as well as excited because it’s my first time to deal with the patients having communicable diseases. I noticed that I have to be more attentive and knowledgeable enough on handling patient at San Lazaro Hospital. I know its not easy for me to adjust from the duty I had experienced in our affiliating schools because we deal with communicable patients.

Group mates:

All of them are willing to learn and they are all eager to become skilled nurse someday. Every body helps each everyone for the improvement of case presentation.

C.I
She’s a skilled and intelligent nurse. She taught us things that can help us to improve our abilities in this field of nursing. I noticed that she knows how to put a gap between student and being a professional. She’s a jolly clinical instructor.

Learning:
I am thankful because I’ve reached this level as a senior student nurse and be one of the affiliates. I am lucky ‘coz I have experienced this affiliation because it’s different as compared to our affiliating hospitals.
Through this affiliation, I have developed my self confidence and also my communication skills. I am able to see and handle patients with different communicable diseases. (PTB. Meningitis, DHF, pneumonia) and know the proper treatment and its management. I also learned how to protect my self, co- students, and my patient from cross contamination of infections. We should be sincere on what we are doing because we are dealing with lives……….
Our duty at San Lazaro Hospital is a memorable experienced because I’ve learned a lot of things with the help of our C.I. And I hope that this would help me in my career.

I.OBJECTIVES

II.OBSERVATIONS
a. Self-Since this is our first time to have a duty in San Lazaro Hospital, I cannot explained what I really felt because I felt too nervous, but
I’m also exited. During our duty I did all my best to render proper care to my patient, and even though I have some mistakes I’m so thankful because I did it, I done my responsibility to my client.
b. Group mates-they are also curious about different communicable diseases that we handled in San Lazaro Hospital, they are cooperative and very nice group mates, and even though I came from block b they treat me as their own classmates and also group mates.
C. Clinical Instructor- ma’am Nhelia is too nice and very helpful C.I, she always helps us, so that we can render proper care to our patient. Ma’am Nhelia is very jolly person, she is too different C.I she make our affiliation more memorable. Thanks ma’am for being C.I to us.

III.LEARNING'S
During our affiliation in San Lazaro Hospital I learned different things. I learned the different diseases that in countered by the hospital; I learned how to manage the different diseases. I learned the proper nsg. Interventions to my client who have communicable diseases. Because of our affiliation in San Lazaro Hospital I experienced handling patient with tuberculosis, I’m so thankful for that.

Learning Feedback Diary

I. Objectives
a. General:
To increase the level of my awareness and understanding regarding communicable diseases and to render nursing skills applicable to the cases of the patients.

b. Specific:
- To apply prior nursing interventions to the identified potential and actual problems of the patient.
- To uplift my knowledge and skills regarding handling patient with communicable disease.
- To practice my communication skills that will lead me to an accurate assessment.
- To learn how to interact with different patients regardless of their different personalities and illness and considering the proper approach to be used.

II. Observations:
a. Self
I have a mixed feelings of being excited and anxious on my first exposure at San Lazaro Hospital. And that time i found my self with lack of knowledge regarding communicable diseases and i need to study harder.I did my best to do my assigned task.

b. Groupmates:
I noticed that some of my groupmates are easy go lucky and depending on the work of others.But most of us know the significance of being a team.

c. Clinical Instructress:
Ma'am Perez is very approachable C.I. that is very contradicted on the impressions of some students. She Motivated us to study harder and to learn how to manage our time. She taught us in a way that is very easy to absorbed in our mind. She know how to mingle with her students in a way that she gave us positive reinforces to pursue our dreams. She is a strict and firm C.I.

III. Learnings:

Nursing profession is not an easy as 1,2,3.....it takes a lot of efforts, time, money and of course knowledge and experience to be competitive one. It was a wonderful experience to become a part of health care team at San Lazaro Hospital. We are very lucky to have an opportunity to have our affiliation there. I learned how to apply appropriate nursing interventions to the patient with TB Meningitis even it was my first time to handle such case. I also experienced to handle a patient with PTB and Pneumothorax. These diseases are highly contagious as i have learned from our lectures but as a nursing student I have to face them as they are not communicable to gain their cooperation.
Nursing for me is also a sacrifice because we are trying to get in touch in such kinds of patient just to render nursing interventions to promote their wellness but in return we are also exposed to the disease they have. I also learned during our conferences that it is not enough to just read and listen from lectures and books. Experience still the best source of knowledge and skills. "Criticisms are good when taken positively" When Ma'am Nelia criticize my pathophysiology I took it positively and I considered it a challenge....Infact, I don't really know how to make a pathophysiology but in that instance I was motivated to try to do it and I felt proud to myself even it was not perfect.

Observations

Self:
I'm glad that I belong to the students who undergo affilation at manila. I felt excited but a little bit nervous because of the set up of the hospital. I knew its not easy to adjust in new environment bec. of the pt. had an CD, I help myself to be attentive and confidenced.

Groupmates:
They felt the same emotion also. They work as a group, even though there is a little bit misunderstanding in our last day of duty. They maintain discipline in the area.

Clinical Coordinator:
It is my first time to be with ma'am nelia in clinical area. I don't have any idea on how she approach students, she is not that strict if you know what you are doing. She is very approchable, She has a good skills and a broaden knowledge that she impart to us.

Learnings
In 3 days being at SLH, I gain a lot of learnings. Learnings that we can apply in the future. I am so lucky because i've seen a PTB pt. with CTT and a DHF pt. with a s/sx of herman's sign and urticarial rash. i've lerned on how to apply safety precautions to our pt.

OBSERVATION:

SELF:
"UNEXPLAINABLE FEELINGS" THAT THE RAEL FEELING THAT I OBSERVE TO MY SELF, IT'S MY FIRST TO HANDLE A PT. HAVING WITH THE COMMUNICALBLE DISEASE.MORE KNOWLEGDE AND SKILL HOW TO PREVENT IT THAT COMES TO MY MIND.I ENJOYED MY DUTIES AT SAN LAZARO HOSPITAL.

GROUPMATES:
THEY ARE ALL NERVOUS AT FIRST BUT THEN THEY ENJOYED TO RENDERED CARE, THEY DO TEY BEST TO GET THE TRUST OF THEIR PATIENT.

CLINICAL INSTRUCTOR:
MAAM NELIA PEREZ OUR CLINICAL INSTRUCTOR IS VERY RESPONSIBLE TO US, SHE IS A GOOD MOTHER TO US GIVING ADVICES AND OPINION HAVING WITH THE GUIDE IN ALL WE DO.IT IS NOT MYFIRST TIME TO BECOME A PART OF ITS CHILD,2ND TIME TIME MAYBE AT THE SUBJECT OF PHARMA.SHE ENJYED TO US EVEN THOUGH WE ARE "PASAWAY" SOMETIMES.SHE SHARED ALL THE KNOWLEGDE AND SKILL THAT SHE'D BEEN THROUGH.

LEARNING:
'LEARNING IS A PROCESS THE WAY YOU ACT" AND THE WAY YOU APPLY" AS I EXPOSE TO THE COMMUNICABLE PATIENT RENDERING AND PROVIDING
CARE IS VERY IMPORTANT TO COPE UP WITH THE PATIENT. I LEARNED HOW TO MANAGED AND BE WITH THE PATIENT, UNDERSTAND THE SITUATION AND GET THE TRUST AND MORE INFORMATION HAVING THEIR THEIR SICKNESS.AND I ALSO LEARNED HOW IMPORTANT THE SUBJECT COMMUNICABLE DISEASE.LEARNING TAKES NOT JUST ONLY IN ONE PLACE,HOW TO SOCIALIZED WITH THE PATIENT ANT TO ENCOURAGED WIHT THE PATIENT HAVING A LOW SELFSTEEM.

OBJECTIVES
General:
To have firsthand experience being in the San Lazaro Hospital, the center for communicable diseases in the Philippines.
Specific:
1. To apply what I have learned from lecture to the clinical area.
2. To learn more about and have personal interaction with patients who have communicable diseases.
3. To see the clinical picture of a patient afflicted with a communicable disease.
4. To learn about methods of treatment and ways of preventing the spread of communicable diseases.

OBSERVATIONS
SELF: I was curious and afraid
during our affiliation at San Lazaro Hospital. Curious because I wanted to know how the hospital can handle such a big number of patients with communicable disease. Afraid because I might contact a disease and pass it to my family.So before and during our affiliation there I took a lot of vitamin supplements and observed precautions, not only for myself but also for the patient I was to.
GROUPMATES:
Although I was with a different group during our affiliation in Manila, I did not have a hard time adjusting with them because we all belong to only one block. I think that my groupmates felt as I felt, excited, nervous, and anxious all at the same time. But inspite of all the pressures and stress, I think that we cooperated and performed well as a group.
CLINICAL INSTRUCTOR:
As with any new clinical instructor, I was nervous but with Ma'am Nhelia I was doubly so. First, because this is my first time to be under her supervision. Second, because I know that she is knowledgeable and that I might make mistakes. I had heard that she was strict but I found out that her strictness is reasonable. I think she was frustrated with us and that she would like us to learn more. I also found out that she has a nice sense of humor, knows how to get along well with us students, and has a huge collection of jokes to share which makes the students feel at ease with her.

LEARNINGS:
During our affiliation at San Lazaro, I was able to appreciate fully the things we studied during lecture because seeing is much better than just reading. I gained firsthand experience and had personal interaction with different patients. Also, I think that the stress and pressure, the fun and even the conflicts we had as a result of putting a lot of people under one roof, all these shared experiences will be an enduring memory that will stay with us for a long time.
