LFD...MALE WARD LAST ROTATION..1ST WEEK..
its a symbolic rotation actually.. . y? well, i wont be handling fourth year block A RLE anymore next semester. should i be happy... or sad?
the 1st week proved to be ok...though there are still mistakes committed by some members of the group.. the same mistakes were committed.... error in charting..failure to follow instructions...
but hey.. you still have 2 more weeks to hone your skills... Good luck..LOLZ...

I. Objective
a. General Objective
To appreciate the different duties and responsibilities of nursing student in term on handling pt at Quirino Provincial Hospital (QPH),Male Ward.
b. Specific Objective
To provide quality care to my pt.
To gain knowledge and skills at hospital setting

II. Observation
a. Self
Its our 1st week of our last rotation, male ward….in every rotation i learning something new, that would help me to improve more my skill in the field. Its our 1st week and our 1st week was so great, im glad, happy and accomplished something. It is my 1st time to insert IV im so nervous and excited to have my 1st insertion and im glad it was successful (1 shot lang eh)hehehehe…
b. Groupmates
I really appreciated the attitude of my groupmates. Even sometimes there are kill joy they always go on w/ the diff topic. Now, they are my close friend w/c I can lean on my problems when troubles has come. They share their knowledge and skills, w/c I gain knowledge from it.
c. CI
She always begins the duty so nice and exciting. She carried the responsibility of being our clinical Instructor and I appreciate madam the way she is. On then second hand she is also a keen observer and she share her knowledge w/ us. She work so hard,

I. Objectives

a. Self- Im a little bit nervous because its been too long when we had our duty at male ward, but I am also excited to gain more knowledge.
b. Groupmates- They are kond and cooperative, we help each other if anyone needs help.
c. Clinical Instructor- she's happy to be with and she's always there to help us if we need help.
III. Learnings
On this week of duty, I have gained more confidence in doing the hospital routines, charting, v/s taking and etchetera. I have also gained knowledge about the different nursing interventions needed for specific types of cases in the male ward.

a. Self- I feel that its like my first time to have a duty at male ward, im nervous.
b. Groupmates- they are still the same, happy to be with, helpful and cooperative.

III. Learnings
On the first week of our duty, i had able again handle a patient, i had improved my skills in rendering good nursing care such as morning care, vital signs taking, providing interventions that is appropriate to my client, giving medication and improved my charting.

To anticipate the doctor’s needs and cater to the necessities of the patients.

a. Self- I was so happy on the first day because I don’t have any demerits but as days came,, I felt everything seems not to be fine, I’ am guilty with what ma’am told at the beginning of the blog, commenting about the same mistake thing…. I’ am quite disappointed in my 1st week of duty at the ward again...:c
b.groupmates – They are good and nice to me.
c.CI – she is very meticulous about our job, well I understand her being a part of health care team there is no space for mistakes that’s why I really understand her.
IV. learnings
Being in the ward after 1 month isolation at OR is very overwhelming feeling, but its nice to be at the ward again, I handled 2 patients in the first week, they are cooperative and knows how to deal with me, that’s why I became a therapeutic nurse to them, I think…. I improved my skills in making SOAPIE.

a) SELF: i observed that, it is different compare in 3rd yr level be coz i learned different things and i think improve my skills and finally i am a good and effective nurse..^^
b)GROUPMATES: they are all busy rendering care to pt. and they do their best to achieve pt good condition.
c)C.I: she acts like a second mother to her student and always keeping her student neat and behave. she teach very well and i am bow to her...tnx mam nhelz! ur d bez..

Week No. 1
OBJECTIVES
General Objective
To improve my skills, knowledge and practice as to provide appropriate nursing care to the client having medical- surgical disorders/ diseases in standard way of nursing practice
Specific Objectives
To gain knowledge about specific disease that causes illness to the patients through proper history taking, assessment and interdependent treatment of illness
To provide health teachings about prevention of the disease to avoid reoccurrence and to promote health wellness
To establish good relationship to the client
To render nursing care not beyond to the usual practice
OBSERVATION
Self: On the first day, I have discharged my patient accordingly. On the second day, I handled new patient who has been diagnosed to have PTB/ Dengue Fever. They are both cooperative. I feel bit of nervous but I need to become confident in my undertakings so that I could render good nursing care.
Group mates: They are doing their respective duties. Some of them commit error especially in charting. Some are confident enough and ready to become a nurse respectively.
Clinical Instructor: She answers our queries and provides information related to it. I think she enjoys our company.
LEARNINGS
The following are things that I have learned during the first week:
Diabetic patient should take care of his or her feet to prevent occurrence of wound/ lesions that could lead into severe infection.
DM can not be treated hence complication should be prevented.
Surgery/ Operation should be done immediately if gangrene occurs and if not treated with antibiotics to the patient who has infected wound and known to be diabetic.
DM type 1 is insulin dependent while DM type 2 is non-dominant dependent but may undergo insulin therapy due to decrease production of insulin.
Treatment for dengue fever is supportive and symptomatic.
Shock should be prevented to dengue patient. Hydration through increasing the oral fluid intake and IVF therapy must be initiated.
If there are signs of bleeding, aspirin should not be given.
BT(FFP/Platelet concentrate) is the most curative way to treat DF having decrease platelet count.

LEARNINGS:
we have to take good care of ourselves always, for us not to get sick and stay in the hospital because I observe to my pt. that they cant afford to buy meds that was prescribed by the physician which are branded meds.

LEARNING:
CONTUSION-injury of a brain tissue
asthma-narrowing of air passages,the bradykinins and histamin that is being release during anti inflamatory reaction causes the constriction of the bronchi.

i learned more to handle MGH patient
I was suffering from a severe headache and diabetes and even the potent pain killers only gave me slide relief. I went for a CT Scan plus MRI and doctors found a ‘blood clot’ in the brain.
