kagaya ng naunang mga batch, ang orientation sa SLH at tunay na inaabangan. Hindi dahil sa mga experiences na kanilang makukuha kundi dhil sa 2 CI na makakasama nila ng isang linggo.
Ang araw ay nagsimulang maganda, first batch poh kme at napaaga ang aming pag-uwi sa dorm.
nakapagpahinga kme ng maayos, at nakakain ng fud na luto ni danggit.
kinagabihan, matapos nmin kumain at mabusog, kami ay naupo na upang mapakinggan ang hnandang report ng 2 studnt nurse. ang PTB.
bilang paghahanda sa kanilang duty sa TB ward, xla ay nabgyan ng advance info.
dumating ang araw sa PTB ward.. kmi ay di gaanong nahirapan. mababait ang aming mga pasyente at di kme natoxic. subalit ng matatapos na ang aming duty, kami ay pinakitaan ng balde baldeng hemoptysis. hay buhay!
ngunit subalit datapwat sa aming pag uwi, kme ay nagpasalamat at nakaraos kme ng isang araw muli.

OBJECTIVES:
A. General Objective
To be able to gain more knowledge and improve our nursing skills.
To be able to know how to manage or prevent the other types of diseases that we have encountered.
B. Specific Objective
To be able to get more new nursing techniques.
OBSERVATIONS:
A. Self- In the first day of duty, I admit to my self that I have a little bit nervous. Because it's my first time to have a duty at SLH. And as I observed, there is a big differences between QPH and SLH. And since this was my first time that Ma'am Nhelia was my C.I.in the ward,I felt nervous and excited.
B. Groupmates- They have cooperation and unity. They exercised Bayanihan System. They are all sincere with each other. And as I observed, they are all serious and willing to learn new things just what I feel.
C. Clinical Instructor- To our C.I. Mrs. Nhelia B. Perez, all I can say is that, she is a responsible C.I. She is very flexible and well-organized with her students. She never leave her students in any procedure that must be done especially in terms with our patients, in rendering care with them.
Learnings: I have learned to become more flexible just like our C.I.. I become more enthusiastic with my studies and also have presence of mind and become active.

OBJECTIVES:
GENERAL:To be able to enhance more our skills,knowledge and attitude.

OBSERVATIONS:
1. SELF: upon entering at SLH i have my mixed emotion, im nervous and excited. nervous because that is a hospital of communicable disease and excited because its my first time to enter on that hospital. i also did my responsibility as a student nurse to my patients i render them my care. and i enjoy my stay at SLH because i learned there.
2.GROUPMATES: they are also nervous and axcited. they are also doing their responsiblity to their patients.and they enjoy their dtay at SLH also.
3.CLINICAL INSTRUCTOR: she's so very suppotive to us, she always helps and guide us in everything we do. she also impart knowledge to us and that is a big help for us.she is also approachable and nice to us.
LEARNINGS: during my stay at SLH i learned a lot. and its a great apportunity to have a duty there because you van see different cases of communicable diseases there that we cannot see at isabela. during our first day i handle a pt. w/ PTB, there i do my health teachings with my patient, I apply wa\hat i have learned, I learned also to do my interventions dependently and independently. second, i handle a pt. with pneumonia, i also give him my health teachings. third, i handle a patient with PTB, a feel pitty on her becuaseat age og !$ years old she already acquire that disease and she's to young, as a student nurse assigned to her i give her my best care and my health teachings also, and lastly i handle a patient with snake bite, i only give him my health teachings.
although we have only given 5 days to have our duty at SLH, i learned a lot there and i will never forget what i have learned on that hospital.=)

I. Objective
a. General Objective:
-to gain more knowledge about communicable disease

-to expose ourself in the hospital
-to apply my knowledge in the best way that i can be.
II. Observation
a. Self- when i was heared that we will have an duty at SLH, many thought that comes in my mind like how is the pt. there, is they are very contagious, how if I myself are contaminated with them what should i do? Is there staff are are friendly or approachable? i expected also that there are so many school doing their affiliation wants to learn like us. First day of our duty our respected Clinical Instructor toured us in the San Lazaro Hospital and i can say that the place are very confusing.
b. Group mates- they are very friendly and approachable, i think that they did all there best to do their task correctly. As what I've felt, they are nervous too, because all of us is our first time to have an duty at San Lazaro Hospital.
C. Clinical Instructor- when i was heared that our respected clinical Instructor is our Clinical and Academic coordinator, I felt so very nervous because i think that there are very strict. But during our actual day of our duty, I've noticed that all my interpretation about our respected C.I are wrong because Ma'am Nhelia is very kind to us, approachable and very friendly person, she is strict in the way that we are learn, she guided us all the things that we do.
III. Learnings:
During my exposure at San Lazaro Hospital i learned a lot of things, and i experienced an unforgettable moments during our duty. In PTB ward, all I've seen there are very yucky, a container that full of hemoptysis but i really enjoyed, we also seen the actual pathognomonic sign of Dengue which is the Herman's sign, I've also handled a person with TB meningitis i felt nervous because this is my first time to handled this kind of case , I enjoyed the last duty in SLH because I experienced how to inject Intra Muscular, I assisted in doing Bed Bathing, Bed Making and I assisted in Osteorized Feeding. I learned the different cases that we handled and i learned how to manage my time carefully. I gain more knowledge about Communicable Disease and i apply my skills in the best way that i can be and my skills are more developed. I learned how to chart carefully and I've met a different kinds of people and different schools, some of them are kind and some of them are boastful. I am so glad that i experienced all of this and I'm thankful to God because all the trials that i experienced in SLH, i can survived and i can make it in my own way.

During our first day that was orientation, the speaker oriented us about the history of SLH and Ma'am Perez tour us in different pavilions.
Our second day, we are assigned in Tb male ward, for me, it is an experience because I handled patient who is suffering from hemoptysis.
Our third day is on pedia ward, I handled patient with AGE, I give meds and assessed for some signs of dehydration.
Our fourth day is on adolescence ward and I handled patient with DHF(II), we saw a patient with a empyema thoracis and it add our learnings about that case.
Our last day of duty is on adult ward, I handled patient with a case of snake bite but there is no signs of envonimation.

OBSERVATION:
Self: At first, I felt very nervous in the fact that San Lazaro Hospital is known for the patient to have communicable diseases. But I overcome this nervousness coz we have maam Nhelia always telling us to take our vitamins for us to strengthen our immune system.
Groupmate: As I observed, some of my groupmates are nervous, some are confident in doing our own task. But for sure, all of us enjoyed and learned a lot from our 1 week duty at SAn Lazaro Hospital.
CI: Our CI, maam Nelia, is always beside us, always watching our move and correct us from our mistakes. She is so kind to us eventhough she is strict.
LEARNINGS:
Last july 21-25, we have duty at San Lazaro Hospital from 7:00 in the morning to 3:00 in the afternoon. we are 9 in our group and we are given our own report to be reported everyday. From there, I've learned the different kind of diseases, its signs and symptoms, and its coarse of action. And from our 1 week duty in San Lazaro Hospital, we have handled PTB patient, patient with DHF, patient with meningitis,and patient with CTT (close thoracostomy Tube). due to empyema. We've seen herman's sign to our patient with DHF. Our Ci also allow us to give medications to our patient. At first, I was afraid and was so nervous, but I keep in my mind that we have our CI to guide and support us.
our main objective in having our duty at San Lazaro Hospital is to experienced handling patients with communicable diseases, the proper approached care to render and also to apply the knowledge that we gain from our lectures since San Lazaro hospital is an institution for patients with communicable diseases such as PTB, meningitis, pneumonia and etc.during our first duty at SLH which is the day of orientation i'm very attentive in listening every do's and don'ts in the institution to know our limitation as a student nurse in caring our patients. emotion is unexplainable because there are other schools at the institution so it is like a competition in our parts as a student because we're representing our school.
On the second day of duty we were assigned at the pav 10 which is commonly known as RM PTB ward. i feel so anxious looking at patients in a terminal stage "what if" are put on my mind that what if we will be infected with such diseases, but i've realized that it was my chosen profession so i have to face any consequenses that comes.
at the third day of our duty, we were assigned at pav 8 which is the pedia main ward. i've handled a 6 month old patient with AGE but the worst thing that i heared from the S.O is they are only residing at "BANgKETA" were everybody slep to and walk ed on too.
on the fourth day of our duty at SLH we were assigned at adolescent ward, i've handled patient with empyema thoracis a disease which is new to my ear thats why i don't know what intevention and care to render but still i made it through the help of our C.I
at the last day of our duty, our C.I didn'y gave us our patiebt together with one of my groupmate because i'm so sick that time. we were just assigned to do the plotting of vital signs but still i've done nothing.
Mrs. Nhelia Perez RN, MSN, is our Clinical Instructor and i can't say anything about her she's very considerate, she helps us whenever we need her help and very approacheble. Now i myself is strongly dis agree to the saying first impression last. MAAM NHEL thank you for all the knowlegde you've shared for us, all the support words of encouragements and also your time. even if we just spent short time with you as our C.I it is very memorable.
Our objectives in having our duty in san lazaro hospital is to apply our knowledge that we've learn in school about communicable disease and also to experinced caring patient with communicable disease that is not present in our base hospital.
In our first day we did an orientation about the rules and what to expect in SLH. i have a mixed feelings about our duty, i am anxious because there are many schools who are having their affiliation in the SLH and also because i don't have any idea of what kind of patient is in the hospital, i afraid of being infected with the disease that is highly communicable like PTB. meningitis and some other communicable disease. i am also excited to care the patient in the hospital and also to be socialized with other student nurse in the different schools.
On second day of our duty we are assign in pavilion 10 or the so called ptb pavilion, all of the cases here is PTB. i observe that the pavilion is over loaded because some patient is sharing bed with other patient. i did my best to take care of my patient even though we have ony limited time to do it.
On third day of our duty we are assign in pavilion 8 or the pedia main. the cases here are typhoid, AGE, dengue and some other common disease for pedia. we only have 4 patient in our group so we are not that toxic that day.
On the 4rth day we are assign in adolescent pavilion. i handled a TB Meningitis patient. in that day i was nervous because my patient is unconscious i did the feeding via NGT and give meds via NGT also. i assist in giving good hygiene to the patient.
In the last day of our duty our CI did not gave me a patient but i was the one who did the ploting of cital signs of the patient.
My CI is Mrs. Nhelia Perez, she is also my teacher in pharmacology. at first i was afraid because of what i've heard about her, but as we go on of our duty i fiund that she is not that bad if you are doing your part correctly. she taught us something that we do not know, she is strict in checking our charting, i also thank her in correcting me if i did something wrong. i also thank my groupmates who was very cooperative, we are helping each other in making our requirements
I've learn a lot in our affiliation, i experienced caring the patient with TB meningitis, i did apply my learned knowledge. i also learn to socialized with other people.
To be a part of the 2nd batch of affiliates at San Lazaro Hospital is an honor and opportunity to widen my knowledge and observe on the different signs and symptoms occurring on the different diseases and to appreciate what I've learn on my communicable disease lecture
During our first day at SLH, we've orient regarding the different rules and regulation of the institution
2nd day is the proper duty wherein we were assign at pavilion 10 or the TB ward, handling TB patients and as a student nurse we do the morning care, NPI and assist the needs of the patient and give some health teachings and charting.
3rd day of our duty, we do the same routine of nursing care but on different pavilion
4rth day, after giving the meds of the patient as ordered by the physician and done with our charting we do the ward class.
5th day, last day of our duty, after we've through with our NPI, giving meds and charting, we went to gavino hall for our lunch and afterward we had our case analysis regarding communicable disease.

OBSERVATION:
SELFf: At first I’m nervous because I will care for a patient with infectious diseases but in the middle I accept that because it’s my own learning on how to render to my patient’s with communicable diseases. I gave my best performance to my patient’s with all my wills & knowledge to help them to overcome their illness & help them to feel comfortable. I have a wholesome relationship to my group mates &to staff of San Lazaro Hospital & most specially to all patient’s that we have handled.
GROUPMATES: Even though some of my group mates are not that familiar to me, they are very active, friendly & they help each other & they are willing to learn new things.
CLINICAL INSTRUCTOR: Madam Nhelia teach well she love her profession as a R.N. & as a clinical instructor. Madam Nhelia teach well everything she knows that will help us to be a successful nurse someday. Madam always guide us in everything we do at the area she has a wholesome relationship to all staff of San Lazaro Hospital & to us his students.
LEARNINGS:
DAY 1. To give the appropriate nursing care to patient suffering from dengue hemorrhagic fever & to know the sign’s of bleeding like narrowed blood pressure, pale looking & boundy pulse.
DAY 2. To apply proper care to patient suffering from PTB & learned how to protect my self from droplet nuclei coming from the patient & to practice to universal precaution to prevent the transfer of mycobacterium tuberculosis & learned the other name of PTB drugs which is APAT NA SIKAT.
DAY 3. Rendering the proper nursing care to patient suffering from malaria like close monitoring of v/s specially blood pressure because narrowed blood pressure may indicate
Bleeding. And applying external heat during chills & increase oral fluid intake & strict monitoring of I & O to prevent pulmonary edema.
DAY 4. Patient suffering from snake bite should close monitor of v/s & watch out of dropping of eyelids because it my indicate positive venom from snake.
DAY 5. In patient suffering from pneumonia should properly disposal of secretion to prevent the spread of infection. And in patient with pneumonia the first nursing management is maintain patent airway & adequate oxygenation.
DAY 6. In caring a patient suffering from malaria I also learned that if patient temperature falls & sweating begins warm sponge bath may be given.
DAY 7. A patient suffering from dengue should close monitoring because of possible bleeding internally & externally if there is a sign’s of shock you can place the patient in trendelenburg position to provide better blood circulation reaching the head part
