Objectives:
Specific Objective: To prove everyone that i am capable in rendering my nursing care as a student nurse.

General Objective: To complete the hours of duty required these sem.

Observations:
Self: i was nervous of course, it is because male ward again. Being in male ward takes alot of responsibility and there must be no room for mistakes. Naninibago nanaman,hehehe!

Groupmates: almost all of us had same feelings because the first 2 rotations was in O.R. Call but the tension is there.

C.I. : I think She is already used of us that is why she just let us do our job.

Learnings: I learned that in everything that you do you must keep it because as a nurse you nust be flexible. It is not an excuse whether you were in different rotations before,whats important is you must always have something preserved in your mind.

I. Objectives
a. General
* To reoirient myself on the setting of the ward in order for me to perform accordingly.

b. Specific
* To take care of my patient holistically by following all the guidelines and proper procedures in administering medications as well as rendering nursing interventions. Furthermore, to gain new ideas and additional insights with the supervision of my clinical instructor that could help me become a competitive nursing student.

II. Observations
a. Self
* On this day, I am active doing my tasks but due to the fact that I'm reorienting myself on the routines of the ward, there are few things that I've done wrong like prescribing the right amount or number of drug but I did my best to accomplish my duties this day.

b. Groupmates
* Like me, they are also on the period of adjustment. They are all cooperative and active performing their tasks. We're teaching each other just to avoid mistakes. Though some of them are slow foot, they are still happy to be with.

c. Clinical Instructor
* Ma'am Nhelz is indeed a very good CI. She taught us most of the things she knew in order for us not to perform at our worst instead at our best. She stayed with us all the time. She is also approachable, open-minded, considerate and especially with a sense of humor. She is always there to guide us and to advise us on what to do. Because of these, I really enjoy being with her. Thanks ma'am!!!

III. Learnings
* During this day, Im so lucky that I've encountered Glimepiride, Fibrafen, Meloxicam and Cefotaxime for the first time. I've learned also that there are patients that are impatient dealing or interacting with their assigned student nurse and there are some patients even S/O who are hesitant to accept or follow the health teachings to them but it's their right to refuse so I can't do anything with that. Nevertheless. it's a great day for me.

Learning Feedback Diary

Objectives:
General Objectives:To be able to render effective nursing care to the patient.

Specific Objective:To be able to assess the patient regarding their condition.

Observation:
Self: at first,of course i feel nervous because its my first time again to had a duty at the ward, but i will that i will do all my responsibilities because in wale ward their is no room for mistake and most of all we are dealing with life.

Groupmates:like me at first i noticed that they feel nervous also.but the presence of being an student nurse to their patient is there.

Clinical Instructor: shes i\a nice CI, because she makes as a good students nurse to our patient. she is always their to assess us in doing some procedure, but sometime she let us do the procedure alone.

Learnings: during my first 3 days of having duty at male ward sames through in other duties in everything that you must do we must have our presence of mind and be alert and active all the time and always put in our mind that their is no room for any mistakes.

OBJECTIVES:

GENERAL OBJECTIVES:

As a student nurse,is to be able to rendered a service in the hospital to give care, support on the patient, for them to restored health, to prevent diseases especially to treat them.

SPECIFIC OBJECTIVES:
to be able to give care on the regardless n their creed,races, color specially on their inancial status, giving the patient in their needs in terms os giving them moral support,love and care

as a sudent, in giving or rendering a service to the patients it must be able to established a plan on the patient,for as the patient to cooperate in coping with the illnes.

OBSERVATION:

SELF

On the first day of my duty, i feel so very hapy and a little bit of nervous it becouse in male ward in every time i entered at may patint room i felt in toxic,very happy at because i've done my ork good.
GROUPMATES:

They are very cooperative and easily to deal with.

CLINICAL INSTRUCTOR:

She is a very supportive one and a smart one. she alwys reminds us to study hard and giving an advice on how to do our job good.

LEARNINGS:

i've learned in the searies of being cooperative, it because the patient will not follow you if you your self is not a cooperative one. and as a nurse must be respectful, because i belive that " respect be gets respect" and every thing what the clinical instructor says you must follow it.

MALE WARD

I. Objectives:
General Objective: To be able to perform in correct manner all the nursing procedures to be rendered.

Specific Objective: To be able to master the routine on the said area, male ward.

II. Observations:
Self: I feel nervous and I'm still coping with the routines at the male ward and I keep on discovering new ideas and learnings.

Groupmates: I am happy and so blessed to my groupmates because they are so supportive and cooperative to one another.

Clinical Instructor: She is very helpful and effective instructor regarding the procedures most especially during giving medications. THANKS Ma'am.

III. Learnings
- during our duty, I observed that time management is very important and I keep on observing and assessing on the side effects of the medications which I have given or administered to the patient.

I. Objectives
A. General Objective:
To be able to give a quality care for the pts in the male ward and to be able to acquire knowledge by having an experience in handling pt with different illnesses.
B. Specific Objective:
To be able to apply what we have learned in our lectures.

II. Observations:
A. Self
In our first day of duty, I did my responsibilities as a student nurse, I didnt let my companion to perfoem all the task I did help her for us to have a good outcome.

B. Groupmates
Most of my groupmates forgot about our regular routine in the ward, but they still perform their duties well, we all have a good relationship.

C. Clinical Instructor
Ma'am Perez as always did her job as a clinical instructor, she guided us in our 8 hours of duty.

III. Learnings
I have in our first day of duty a new term for me, the Wernicks Korsakoff Syndrome which is a personality disorder characterized by psychosis, disorientation, delirium, insomnia, and hallucinations.

Objectives:
General: To broaden one's knowledge in clinical placement through application of desired treatment, procedure and drug administration.
Specific:
To be part of the health care team in rendering care to patients, health maintenance and management of diseases.

Observations:
Self: I always look forward for fruitful experience in the ward and hope to learn more
Groupmates: Everyone had their fair share of work in the conduct of their duty
C.I.: Mam Nhelia had her equal share of responsibility and we always look up to her as our kind and supportive C.I.

Learnings:
Working our way out of the O.R rotation for 6 weeks is difficult but facing a new phase of experience in the ward is another thing. The hangover from the O.R will always linger in our senses and we felt the same thing about our male ward.But with all the difficulties we had encountered, the consolation comes handy because of our energetic and competent C.I.'s. However things maybe similar in some ways, Male ward will always be different.Goodluck to us all. Remember the moment of truth has finally came.

OBJECTIVE:
General: To train our skills in rendering service to medical and surgical patients.
Specific: To be able to perform the usual routine in the ward and do some procedures done in medical and surgery patients. And to expose ourselves in doing our duty as student nurse and it would be a great experience for us in the near future of being registered nurses if it’s God’s will.

OBSERVATIONS:
Self: On the first week, I felt so nervous (again). But as usual, as times goes by, I learned to deal with the routine although I had an error on the very first day of duty on my patient’s chart. I noticed that even though I had a mistake , I still enjoy the duty because of that mistake I learned…
Group mates: This group is really funny and all of us are very cooperative. We help each other in all activities.
C.I. : Ma’am Nhelia is still very funny. Even though we are all toxic with the patients or any procedures, she’s still calm and makes some jokes for us mot to feel toxic.

LEARNINGS:

On the first week, I had a MGH pt. already. I experienced doing discharge charting or the SOAPIE, making home med. Prescriptions and some teachings for the patient to maintain good health condition even at home.

Objectives:
General objectives:
to start anather day of bieng with the patient. and thop give care for my patient.
Specific objecvtives:
To gain more knowledge obout the patient disease. And to assess the patient condition.

Observetion:
Self: I was able to to do thing carefully with the suppervition of my clinical instructor
Groupmates: they do there job carefully and help one another in assessing there patient. They do there assessment carefully.
C.I: She give us information on how to assess our patient carefully. She allows us to do all the work in area.

Learnings:
I have learn how to discherge a pateint by following the procedures. And it feels so good that my patient has been discharge during our duty.

>To enhance our knowledge and skills.

Specific objective:
>To be able to enhance our knowledge and skills during our rotation at QPH male ward.
>To improve our self confidence in rendering care to our patients.

B.Observations:
Self:
>I was nervous on our first day at the male ward because inevitably we’re going to deal with medications again and we had to do a lot of computations.hehehe
Groupmates:
>my groupmates are all nice and cooperative and they’re all doing a great job.
Clinical instructor:
>Ma’am Nhelia is kinda strict on the clinical area but she is very approachable and fun to be with. She taught us a lot even in the clinical area.

C.Learnings:
>I learned that we should always remember that assessment on patients V/S is really very important before & after administration of medication.

OBJECTIVES:
General: To have an additional knowledge and skills in rendering my nursing care to patients with different kinds of illnesses.
To be more familiarized and be an expert in performing my nursing skills and other procedures.

Specific: To meet the needs of my patients.
To achieve my goals and to make the most out of it.

OBSERVATION:
Self: I can say that i am now more responsible with my work,unlike before that i an not that serious. I think I am now more skilled in doing my job.

Groupmates: Same as i, they are now more responsible. Sometimes some are still out of their mind but still can follow.

Clinical Instructor: Comparing before, I observed that she already develop her trust to us, it is manifested by entrusting to us our patients by making us functional nurses.

LEARNINGS:
This week is very challengingweek.It is because I experience how to become a medicine nurse and a procedure nurse.

As a medicine nurse is exciting. You are the one responsible in all your patients medication from oral to parenteral, and i find it more easier than having 1 patient and doing everything.

Being a procedure nurse take alot of responsibility. You have to attend all your patient needs and off course you have to know everything that is happening to your patient.
