Eto ang nangyari sa last week nmin sa San Lazaro...
Hay buhay, kung kylan ba nman huling week ska pa panay ang ulan at baha.. kakaloka...
Pero, masaya na dn. Kz at last, wala kme rotation sa PTB Ward. O d b???
Eto pa isang masaya. Nakakita xla ng 2 rabies patients. Imagine that??? Hay swerte tlga.
Masaya din ang inyong abang lingkod sapagkat sa aqn natapat ang grupo na marunong makipagcooperate. Nag ward class sila. At ang suggestion q ay knilang sinunod (pero palpak lang ang handout kz baliktad,,, Hahahaha)... pero okey na dn. at least xla may meryenda. Ung grupo ni mam luisa, both 1 and 3, ala!!! Mantakin nyo un. at muntik pa xla d na kapag ward class e kz nman... basta...
To cut the long story short, Affiliation 2008 has ended. Not with a Bang but with rain.. lots of rain... mabuti na lang mabait ang mamang driver ng Baliwag... at xempre ang mga boys ang girls n tumulong... (slamat)...

I. Objectives
General: To learn more about communicable diseases.
Specific: To be able to develop our skills and knowledge to handle patient with a communicable disease as part of our being caring for them no matter they have.
II. Observations:
Self: I've been do my part as evenly I'm only a student nurse.On the first day of duty in San Lazaro Hospital. I'm aware that all patients there are highly communicable so I'm painstaking to myself, I wear mask and gown or else it will be transmitted with in me.
Groupmates: They are approachable sometimes because there are things we cannot to be able to cooperate. I've been observed that someone each of us are doubted with our patients. So we are the boys most us usual to do anticipate the needs of the patient.
C.I.: Our C.I. is approachable such as if were asking some we're not understsand on the patient's chart like the Doctor's order, the meds. And of course are next guardian who take advises on us or reminds what is being to do our patients.
Learnings:
I was happy within the days of our duty in San Lazaro Hospital because there are some different schools we've met.And I'll try my best to report with our case. And especially we give our service dealing with a communicable disease patients.
I. Objectives:
a. General objective- our general objective in having a duty in San Lazaro Hospital is to learn more knowledge and more skills; to apply the knowledge and skills that we've learned in the school about communicable disease and to experienced handling a patient with certain communicable disease.
b. Specific Objectives- to know more skills in handling patient with communicable disease.
- to give our care to a patient with C.D., to help them in their faster recovery
II. Observations:
a. Self- I have a mixed feelings during our first day of our duty because i don't know what approach will i give to patient to be assign to me by our clinical instructor. But it my pleasure to have a duty in a hospital like this kind of hospital(San Lazaro hospital).
- I've done my part as a responsible student nurse to my patient and to my fellow student nurse.
b. Groupmates- i think they are nice, because since in the beginning that they became my groupmates, sometimes we didn't helped each other, sometimes we do but they are good, because they've done also their part.
c. Clinical Instructor- Our C.I. ma'am Nhelia B. Perez, RN, MSN is so nice to me and to everybody, she always thought us many ides of hers in caring patient with C.D. She is strict in "charting", she always correct ones wrongness, and give "demerit" for those who committed error in "charting" like me, hehehe. And i thank also because she becdame my C.I.
III. Learnings:
- in the first day of our duty in San Lazaro Hospital, I've learned how then the institution established, the personnels involved in establishing.
- and on the remaining four days of our duty, we've handled patient, I've learned more techniques, more knowlege and attitude and I've also learned how important "groupwork and partnership" is.
Objectives:

To be able to aaply my skills in rendering nursing care to patient with different diseases.
To gain more knowledge.
To observe and appreciate what I have learned in my lecture.
Specific Objectives:
To established rapport and to gain trust and cooperation.
To render Health care.
To give medication as ordered by the physician.
Observations:
I - I am more confident to talked to my patient but have fear that I might acquire the disease because i know that most cases at SLH are transferable via airborne
Groupmates:
They are all nice, cooperative and friendly.
C.I
She is approachable and nice she is not selfish to share what she know.
Learnings:
During my stay at SLH I've learned that we must protect ourself or wear precautionary measures in handling our patient because as a nurse we must protect ourselves first for us to be efficiet in rendiring care to patient.

For us to experience handling patients with communicable diseases, and to apply the proper nursing intervention to a patient with communicable diseases.
Specific objective:
The specific objective to broaden our knowledge and skills about the different communicable diseases. To be familiarized and to see the actual appearance of the different pathognomonic signs of a communicable diseases.
Observation:
Self:
I need to have a presence of mind all the time and be alert.
On the first day of our duty at San Lazaro Hospital, I feel that I need to be more careful in terms of handling a patient with communicable disease. Fear to be infected with such diseases like PTB, hepatitis, and etc is a big NO for us.
Luckily as a routine in every institution having a general orientation about the history and the mission visions of each Hospital and others.
On the second day of our duty we were assigned on the pediatric ward. We handled a 1 year old patient with pneumonia t/c pertussis. We finished our first duty with out any problem.
On the third day of our duty we were assigned on the adolescent male. I’ve handled a patient with Schistosomiasis a 13 years old boy with distended abdomen caused by his enlarged spleen and liver. I felt sad because of his condition he is too skinny and very young to be in a hospital confined because of a disease. I advised him to wash his hands before and after eating and after using the bathroom to avoid any other complications.
Group mates:
We as a group we have cooperation in each activities we do.
Clinical Instructor:
Maam Nhelia is approachable and shared what she knows. She is strict but for the sake of our learning.
Learning’s:
I learned that when you feel that there’s something wrong with your health don’t hesitate to consult a doctor. I learned that be careful with our body because it is a gift from God that need to be treasured. I learned to
Socialized with others. And I learned what to do when there’s an earthquake…hahahaha………….

I felt nervous during our first day of duty at san lazaro hospital because it seems so hard to render care with those patient who had communicable diseases. but during the following day i felt more comfortable because our patient was very cooperative. i still tried my best to do or finish the task that was assigned to me. eventhough there are lots of challenges, i still did my best to render the quality of care to my patient in that 5 days of duty at san lazaro hospital.
groupmates:
They also felt nervous during our first day at san lazaro hospital. they were all cooperative and nice. they know how to accept others idea. they also did their part to finish their assigned task.

Ma'am Nhelia was very nice to all of us she's very approachable and always there for us. she always correct us when we did something wrong and she also shared some of her knowledge.we enjoyed being with you Ma'am thank you for all the knowledge that you had imparted. i really appreciated all of this and we really enjoyed being with you ma'am even if its only for 1 week
Learnings:
During our duty we encountered diff. cases of communicable diseases. we learn the causes, and the diff.medications that was given and the proper management or nursing interventions to be rendered to meet the quality of care of those patient who have comunicable diseases. we also gain knowledge during the presentations of our cases or case analysis. our clinical instructors shared their knowledge about the case we presented they gave suggestions and additions with regards to the case we presented. we enjoyed our 1week duty at san lazaro hospital. we are all lucky for having this kind of affliation.

Learning Feedback Diary
OBJECTIVES:
General: To enhance my knowledge, skills, & attitude in caring of patient ‘s with infectious disease.
Specific: To learn more about contagious / infectious disease at San Lazaro Hospital.
To enhance my skills in rendering care to patient’ s with infectious disease.
To know the proper attitude in communicating to patient’s with infectious diseases.
OBSERVATION:
SELFf: At first I’m nervous because I will care for a patient with infectious diseases but in the middle I accept that because it’s my own learning on how to render to my patient’s with communicable diseases. I gave my best performance to my patient’s with all my wills & knowledge to help them to overcome their illness & help them to feel comfortable. I have a wholesome relationship to my group mates &to staff of San Lazaro Hospital & most specially to all patient’s that we have handled.
GROUPMATES: Even though some of my group mates are not that familiar to me, they are very active, friendly & they help each other & they are willing to learn new things.
CLINICAL INSTRUCTOR: Madam Nhelia teach well she love her profession as a R.N. & as a clinical instructor. Madam Nhelia teach well everything she knows that will help us to be a successful nurse someday. Madam always guide us in everything we do at the area she has a wholesome relationship to all staff of San Lazaro Hospital & to us his students.
LEARNINGS:
DAY 1. To give the appropriate nursing care to patient suffering from dengue hemorrhagic fever & to know the sign’s of bleeding like narrowed blood pressure, pale looking & boundy pulse.
DAY 2. To apply proper care to patient suffering from PTB & learned how to protect my self from droplet nuclei coming from the patient & to practice to universal precaution to prevent the transfer of mycobacterium tuberculosis & learned the other name of PTB drugs which is APAT NA SIKAT.
DAY 3. Rendering the proper nursing care to patient suffering from malaria like close monitoring of v/s specially blood pressure because narrowed blood pressure may indicate
Bleeding. And applying external heat during chills & increase oral fluid intake & strict monitoring of I & O to prevent pulmonary edema.
DAY 4. Patient suffering from snake bite should close monitor of v/s & watch out of dropping of eyelids because it my indicate positive venom from snake.
DAY 5. In patient suffering from pneumonia should properly disposal of secretion to prevent the spread of infection. And in patient with pneumonia the first nursing management is maintain patent airway & adequate oxygenation.
DAY 6. In caring a patient suffering from malaria I also learned that if patient temperature falls & sweating begins warm sponge bath may be given.
DAY 7. A patient suffering from dengue should close monitoring because of possible bleeding internally & externally if there is a sign’s of shock you can place the patient in trendelenburg position to provide better blood circulation reaching the head part.
