I walk through those doors with pride,
Who's life will i save today?
Someone is waiting for me,
Someone is alive today because of my duty.
Sometimes we cry cause we can't save them all,
God sometimes won't let us interfere when HE calls.
A baby's first breath when he looks at me,
The joy of my first delivery.
The tear i wipe a way with my own hands,
The life ending of a gentle old man.
The night seems so dark and the morning so bright.
Being a nurse you see life in a different light.
Who will i save tonight?
Who will hold my hand during their last breath with no fright?
Who will enter this world on my shift?
How many mothers will greet their babies with a kiss?
I don't know who these special people are
but i will meet them with every call
I will hold them tight and help the pain
I will hold them up when they feel faint.
I will be strong when i am needed
That is my job, I am a nurse..that is my duty.

GENERAL OBJECTIVES:
-To focuses on pedeiatric care and their families to achieve their optimum health potentials.
SPECIFIC OBJECTIVES:
-To acquires and maintains knowledge for pediatric care.
-to identify and analize the assessment data to determining diagnosis.
-to implements the nursing interventions and identify in the plan of care.
OBSERVATION:
SELF: Doing all activities for pediatrics care, happily for handled pedia ward, friendly and enjotable especially more learn.
GROUPMATES:Friendly and they wanted to cooperation to each other for uncrease performance.
C.I:i learned more about my C.I she teach and give though before we prociding to our patient for welfare the outcomes of nursing cae and she is also friendly with thier students.
LEARNINGS:I've learned more about giving nursing care on how to approach them to be comfortable for my patient and on how to interacts, provide safety, interviewing to make assessment and also to improve my knowledge and skills as a nursing rrole for patient care.

During the lecture of our C.I, I was found out to myself that I have to improve my knowledge with regards to her lecture about Case abstract and to be much particular in doing our case.
B.)Groupmates:
My groupmates are sharing knoledge to each other, giving idea and suggestion whenever there was a question specifically with regards to health.
C.)C.I:
Our C.I is very strict in terms of charting, wearing of proper uniforms especially in giving medication to the pediatrict patient.
IV.lEARNINGS:
I have learned about the case of my patient which is a pneumonia, like in giving proper nursing management to prevent the disease back.

General objectives:
To be able to learned and get information about the routine in pedia ward,daily activities done after the NPI and also the requirements.
Specific Objectives:
>to be able to learn and to be oriented about the requirements after the duty.
>to be able to assess what are the problems.
OBSERVATION:
>SELF:
I feel nervous at our first basic duty because im alone compared to my groupmate's and with that i learned to be dependent in my own self.
>GROUPMATES
They are cooperative and active in doing their respective responsibilities.
>CLINICAL INSTRUCTOR:
Shes always observes our work, also support us to the things we are doing and gives us more knowledge and learnings.(MAAM nawindang aq s pagiging mg isa!!!)
LEARNINGS:
I learned to become more independent, to approach also the possible attitude of my patient, and to understand what are the right and good things to do.
General objectives:
- To give care on the patients especially on the pediatric ward

Observation:
Self: A little bit nervous and uncomfortable, I feel that its like my first time to duty at the hospital.
Groupmates: They are responsible in doing their duty as a student nurse. They are willing to improve their knowledge and skills.
CI:Eventhough she is strict, she taught and guide us what to do very well, she makes us confient with ourselves.
Learnings:
I had been more confident and comfortable in dealing a patient,improving my assessment, knowledge and skills in giving care as a student nurse.
LFD- Learning Feedback Diary
I. Objectives
a. General Objectives:
-To improve my knowledge and skills in caring pediatric patients
-To be familiar with the diffirent diseases usually present in children.
b. Specific Objectives:
-To take and plot the Vital signs of the patient.
-To be familiar with the medicines we give and to enhance my skill in drug computation.
-To give health teachings to our patient or SO regarding to his/her disease.
II. Observations
a. Self:I was nervous, conscious and slightly serious because it is my first time duty at the pedia ward.
b. Groupmates:They were kind, cooperative, helpful and approachable.
c. C.I.: She's kind, approachable, nice to be with and understanding.
III. Learnings
I learned how to make an ncp, soapie and on how to take an assessment and physical examination, but before doing anything we must introduce first our name into our patients and we must gain their trust first to have a good history takings. I also experienced to TSB a patient with fever. And the last I learned how to discharged a patient.

General Objective:to know more about the nursing process particularly in the pediatric ward.
Specific Objective:to familiarize and apply the theoretical learnings and techniques of nursing we have studied in school at the pediatric ward.
Observation:
Self: Willingness on my part to learn more about hospital work, particularly in the pediatric ward because i have a one year old grandson.
Groupmates: All were very excited to resume our hospital duty and experience new challenges.
CI: She is willing to share her knowledge with us. She has a good sense of humor and she motivate us to be more responsible nurses in the future.
Learnings: The orientation lecture given by our CI was very informative. We were able to reacquaint with hospital terms, learn more about charting, case abstract,gordon eleven and the do's and don'ts in the pediatric ward which will help us in our hospital work.

GENERAL OBJECTIVES:
-To learn and develop new skills and techniques, to enhance and implement learnings from theories learned for the purpose of nursing intervention.

-To experience and to learn first hand how to handle and to work with cases of different proportions and to render learned techniques from nursing care theories.
OBSERVATION
SELF:
-I was amazed on how much things change I assumed that things would still be the same, but as Madam Nhelz literated in her orientation things need to be a more close encounter one of a kind nursing intervention, which i enjoyed and erudite much.
GROUPMATES:
-I could see my classmates determination and thirst for new knowledge and the transition of their theories to actual realization of nursing intervention i see in them are wonders.
CI:
-As i have mentioned earlier, she did orient us to the possible circumstances and new learnings that concerns our rotation in the Pedia ward, she's very valuable, accomodating and strict, but thats Mam Nhelz just being cautious.
LEARNINGS:
-As much as i wanted to say something im only bounded to what i have experienced first hand, and that i need to practice more in my rapport matter which i think i still lack.

>I felt nervous because it was my first time being handled by ma'am Perez,but i felt excited because it was also my first time at the QPH PEDIA WARD.
GROUPMATES:
>They cooperate well.i observed that they are eager to learn more.And on the first day of our duty each of us gained knowledge.
C.I:
>She guides us in everything that we do.When we had our NPI to the patient,she's their to observed us.She taught us things that we dont know.She taught us things that are proper and not proper in regards in our profession.
LEARNINGS:
>I learned that i should be alert in everything that i do.I should be relax to overcome my nervousness while preparing the medicine of the patient.I also learned how to cooperate with my groupmates well.

-To facilitate a smooth and suitable care, practical and technically feasible ideas of nursing interventions.
OBSERVATION
SELF:
-My first rapport of the day was a success, well basically its a team effort but i think ive done my part of the bargain, and that im beginning to be part of something new and excitingly helpful in my life.
GROUPMATES::
-I could see that my partner in crime, errr what i mean partner in nursing care was diligent and provides necessary interventions that is appropriate, and the whole group are very accomodating and they remind one another if something is at miss.
CI:
-Since Mam Nhelz wasnt around last saturday, still she left us in good hands in the presence of Mam Chix who expanded our knowledge in the horizons of practical nursing interventions.
LEARNINGS:
-Ived learned something new and something novel and thats how to make a admitting and discharge plan, well not perfectly but still acceptable in level.
GENERAL OBJECTIVES:
-To render our service in hospital settings esp. in Pedia Ward.
SPECIFIC OBJECTIVES:
-To be able to take care of the patient and give health teachings as well both patient and SO esp.in Pedia Ward.
OBSERVATION;
SELF;Doing our task as a student nurse esp.pediatric patient is not easy because children are very sensitive and they are affraid of nurses and doctors esp they had already encountered trauma about injections.But I've observed that it depends on the students nurse on how to gain their trust.
CI:Ma'am Nhelia is very inspiring CI and instructor as well.She's very strict and frank when it comes to policies and regulations but inspite of that she's approachable and willing to teach us and guide us to do our duty a very effective one and to make ourself independent too.
LEARNINGS:On the first day of our 2nd rotation in QPH which is in Pedia Ward as usual is our orientation and we had lectures too.I've learned on how to do my physical assessment to my patient and make a graph of the patients family history of illness.

-To obtain knowledge and direct information about the illness and to create more appropriate assessment, diagnosis, planning and interventions.
SPECIFIC OBJECTIVE:
-To render good quality service to our patient in accordance to what we had learned from our C.I. and to work collaboratively with the staff and work independently as a student nurse.
OBSERVATION:
SELF
-I got nervous because it’s my first time to be assigned at the pediatric ward and my first time to do the skin testing.
GROUPMATES
-We work as a team, they are kind, and they are not selfish to share their knowledge.
C.I.
-She is kind, industrious in explaining procedures that we must and should learn.
LEARNINGS
-I learned how to apply appropriate interventions, diagnosis and planning. I also learned how to make “BURADOR” which comprises of name of the drug, dose, and time.
-I experienced how to perform skin testing.

OBJECTIVES:
A.General Objectives
To be oriented and be exposed to the pediatric ward of QPH.
B.Specific Objectives
To know how to deal with the problems and how to do the necessary intervention in these special area,the pediatric ward.
OBSERVATIONS;
A.SELF: I have added a new knowledge during our orientation these past week at the QPH pediatric dept. Hopefully i can use it during our stint here. May the knowledge imparted to me be of a big help to my patient.
B.GROUP MATES: With the mixing of the old and the young in our group i think we are all very comfortable with everyone because it is balanced. We are sharing ideas and also helping each other.
C. C.I- To our mentor, may our stay with you on our second rotation be a fruitful one for all of us. Be a good motivation for us, to do good when serving our patient, be a guided by our C.I, specially the things that we haven`t done or accomplished before.
III. LEARNINGS:
During our first week of our second rotation I have been given a patient to be discharged. I have learned the different steps to discharged a patient, a very enlightening start for me because I have never done these thing in any of our rotation. With these additional learning Im very happy that I have applied what our C.I`s had taught us.Thanks for the guidance MADAM.

Hi ma'am gud am po! just finished our interview about the OR at CVSH together with Ms Tacang & Ms Cayabyab , we are thankful bec. they are very accomodating and cooperative.(Just passing by kasi wala pang nagpo post ng mga LFD,like me cguro very busy last weekend)
