OBJECTIVES:
a.General Objective- to provide a comprehensive knowledge regarding communicable diseases, to describe their manifestations, management as well as the etiology and the role of the communicable disease nurse.
b.Specific Objective- to consider the importance of rendering quality care for patients with communicable diseases.
OBSERVATIONS:
a.To myself- I felt excited,uncertain and even little anxious during our affiliation in manila,but in the end I really appreciated my stay and I learned many lessons which I can use to be a successful nurse someday.
b.Group mates- I am happy that one benefit we acquired during our affiliation is that we strengthen our group and fixed previous misunderstanding bet each members and learned the importance of group cohesiveness in order to make our affiliation purposeful.
c.C.I- were thankful for having her as our mentor,that despite of the many problems we caused them,they have still the open mind and heart that forgive.We appreciated also their strong wants to make every students learned from them, to be a competitive and well prepared nurse someday.
LEARNING'S:
During my exposure at San Lazaro,It started quite a little stressful in my side, for I lost my wallet and I got sick, so I felt sad and I almost thought I will be sent home already, but luckily God answered my prayer miraculously,from being bed ridden, I got well the first morning of our RLE duty at San Lazaro,so I am very thankful that I had given a chance to fulfill my roles and carry out my responsibilities in the said institution.
Before, the field of communicable disease health nursing seems a little unfamiliar or mysterious for me,making it hard to imagine what the experiences will be like or what nurses do on this area,but provided having our C.I who diligently exposed us to hospital's clinical feature, and to every cases which we can practice our skills well,our affiliation became and truthfully became the highlights of nursing related experience.
Despite of the pressures,sleepless and sickness moments we encountered during our stay for affiliation,I have lot of unforgettable experiences still to consider;first this opportunity let us get along well with each other, it makes us settle disputes,and most importantly as I went home, one very special learning remains on my mind, that indeed.., "It really takes a lot of brain to be a nurse"

SAN LAZARO HOSPITAL
“LEARNING FEEDBACK DIARY”
I. OBJECTIVES
A. GENERAL OBJECTIVES:
To conduct duty at SLH guided by our CI and acquire knowledge and skills through our experience and be able to impart our nursing care to the patients.
B. SPECIFIC OBJECTIVES:
- To develop skills in providing clinically-related activities as may be require or instructed on us to do.
- Impart our care to our patient through his/her wellness with the collaboration of other care provider in the hospital with the supervision of our CI.
II. OBSERVATION
Self: Since this is my first exposure here at SLH, I was so scared knowing that this hospital is for communicable diseases and I may acquire on some diseases, also felt nervous. But I was amazed and surprised, it was very clean and organized, each pavilion has its own satellite ICU, all available meds and IVF are provided free to all patients including meals. One of the most systematic hospital, I have visited.
Group mates: As I observed, my group-mates was doing their best in regards in our duty, but sorry to tell that I may not enjoying the company of Mr. Abalos since he always insulted me. I admit that I am not so smarter that others but I felt so dump and down. I’ll never forget the day he insulted me. I think his not the professional to be honored
Clinical Instructor: Our CI closely monitors/supervises the group in good mood and motivation. She guide as in every procedure that we do and observed. A supportive CI that no other than Mrs. Nhelia B. Perez, thank you for being kind, understanding and your so approachable and strict sometimes but in a good way. Thank u Ma’am!
III. LEARNING AND EXPERIENCE
Our first day at San Lazaro Hospital, we attained the orientation and we’ve known that the hospital was established since 1577 as dispensary in Intramuros Manila. Founded by Fray Juan Clemente. In 1784 was named Hacienda Mayhaligue and Father Felix Huerta built chapel and improved the compound. After the orientation, we go for a tour in the hospital equipped with our face masks, we started the tour at 3rd floor Pav3 the adolescence ward,pt. admitted here was 13 years old above. Fallowed by the Pav4 the pediatric ward, ages admitted is 2mons-12years old. Pav7 the Philippine health ward,pt. admitted here varies from their ages,gender and cases. Pav6 was the adult male ward, then the Gavino Hall for student nurses. Pav2, adult for all ward patients ages 20 above. The bahay Lingap which houses the asymptomatic pt. of HIV and the symptomatic pt was put in Pav4 and Bahay Lingap are restricted area. Pav9 houses for PTB pts. There was a separate room for pts. With CTT and there are more wards of different level of the diseases IDCCU for AH1N1 and Sars. The ER was divided into 2 sections, one for the OPD and for pts. need confinement.
Second day, we assigned to a Pedia ward extension on Pav8, most of the patients on that ward having a Pneumonia. I enjoyed my duty together with my cluster. Wednesday, we assigned to ER and we assisted all incoming patients, we need to be alert specially in Triage area and do the good assessment for patients in order to identify where they put in OPD or direct to Pav4 etc. Thursday, we assigned to Pav6 male adult, we handled patients with DHF and I appreciated so much our patients because of having petechial or hermans sign generalized on his body. On last day, that’s my worst day huh, because my partner is no other than Mr. Abalos, I felt uncomfortable with him because I still remember the day he was insulted me. But anyway, I enjoyed so much being with Ma’am Nhelia even this is my first time in her company, I learned lot from her, and got some ideas that added in my knowledge. Of course, I’ve learned lot from our post con and pre-con everyday. Thank you and hope to gain more knowledge and have time to share for us again oh before I forget, we also bought some souvenirs there like t-shirts etc. Thanks again

I. OBJECTIVES

II. OBSERVATIONS:
a. TO MYSELF:
You know,The first day of duty in San Lazaro, the nervousness is always here. Why? because everitime I see Madam Nhelia Perez I got this Vital signs of BP-140/100 PR-120 RR-40 (joke). Hehe I dont know why I am like this, maybe this is my first time to be her student.
b. TO MY GROUPMATES:
I only have limited comments to my groupmates. I only wanted to say this to one or some of them.. I relly dony like your attitude guys, some of you are very "feeling" when it comes to our lectures or topic " nagmamagaling" to make it specific. I hope on the next semester i dony want you to be mt groupmate again. Although some of us has the argumentation before, I am happy that we had the cooperation during our exposure in San Lazaro.
c. TO My CLINICAL INSTRUCTOR:
As past days goes by, I've known madam nhelia for the 5 days she held our group. Some said she's terror. She's not the type of person that is " isnabera, suplada, maldita" but she's only stricto...
My nervousness everytime I saw her was already fades! " Marunong siyang makisama sa mga students niya and she treated us like magkaka age lang kami" Maam Nhelia was also " kalog" because she always made us smiles and laugh thru her jokes..
Although she is stricto, but this is the way I've been liked her..
Bonding with maam Nhelia was reaaly been unforgettable days and a gorgeous one..
III. LEARNINGS and EXPERIENCES:
Through my experiences in San Lazaro Hospital, I might say that I really learned a lot like doing and analyzing pathophysiology of every diseases like Dengue Fever, Rabies, leptospirosis and AGE. Mind you!I only have limited ideas about doing and explaining pathophysiology since we do not have subject like that.
I gained knowledge about many different nursing intervention when it comes to caring a patient. But anyway, I wanted to give my highest thanks to the one who gave me a lot of ideas and shared informations about many different illnesses. Thanks Maam Perez for giving me the opportunity to learn. Through your lectures and informations I will always remember that in order to become an effective nurse. I learned how to respect others. I've known also what are the attitudes of my roommates..

II.OBSERVATIONS:
SELF-I am excited and nervous to had a duty at Manila because of the new institution to be exposed.
GROUPMATES- They also excited and nervous because of the new things to be learned and institution to be exposed.Some of them had annoyed w/ some of our groupmates because of misunderstanding, but the cooperation,helping each other are there.
CI- My first impression to her are very "masungit",terror CI,hard to approach but i was wrong w/ that she's not hard to approach, very understanding CI,nice,very cool and joker CI,a joyful CI,she always make us laugh for us to be comfortable in what our doing,advising and reminding us ,as our second mom,and she's a smart and good CI.
III.LEARNINGS
On the first day of our duty at San Lazaro Hospital in Manila I felt excited all of us are fisrt time to be exposed/affiliate to that institution. Many schools you were encounter and different cases and people, to be familiarize from the different Pavillion at San Lazaro Hospital,they have department or ward for the cases of HIV an to the infectios disease citical unit.We discussed the different cases through giving our idea regarding to the cases we assigned to us(pre and post conference) for us to be aware and familiarize,to have some knowlegde in a certain disease,to know the possible nursing management,the distinctive sign of the disease.I experienced also to had a ward nursing assessment in San Lazaro Hospital, discussed clearly the aseptic isolation techniques.I encountered also a 6yr old patient who had a CTT.On the seond of our duty we assigned at the ER,I observed the daily routines doing there,the proper documentation,and records who are the patient to be admitted or considered regarding their cases or complaints.I experienced to endorsed our patient, we allowed to do the endoresment in our area or Pavillion we assigned.I learned that we need to be alert, awake and confident enough ad have presence of mind in what we are doing for us to know the proper nursing interventions to the patients or the health teaching to be given, on how to assessed and assist the patients w/ communicable disease.Even in a 1 week duty at San Lazaro Hospital I really learned enjoyed and appreciated the new things w/ the help and support of our CI, for me i considered that as my unforgettable experienced a different experienced for me and even our affiliation was not good enough because of some misunderstanding or not obeying I think but I know that all of us are learned and enjoyed during our affiliation at Manila even some of us are very tired and "Puyat" because of the requirements to be pass but that is part of our studies, the field of nursing course we taken ,for us to study hard and gain knowledge for us to use it in the future as a proffesional nurses someday.

GENERAL OBJECTIVES:
To render service to our patients and to have a better learning exposure with different communicable diseases.

To give therapeautic care to my patient with regards to their specific diseases.

DAY 1- July 13, 2009
On our first day of duty, we had our orientation held at San Lazaro orientation room and it was started at around 10:00 am. The discusser was Mrs. Elma Caluntad, Supervising Officer. Afterwards, they explain the history of SLH followed by the rules of the hospital.During our orientation, I have learned that san Lazaro Hospital was not accepting patient who have cases on alcohol problem and drug users but patient with contagious disease.
Then we visit the different pavilions as a part of the orientation for us to know the rooms assigned to us. After that, we had our post con on Dengue Fever and it was discussed by Mrs. Miguel and Ms. Puruganan.
DAY 2- Jul y 14, 2009
On the second day, I was a little bit nervous because I was thinking what I should do first since it was my first time to be exposed on this kind of case. As our duty goes on, I felt relieve because I tried to gain self-confidence for me to be able to work well. And they both explained very well.
After the discussion, our C.I., assigned us to our patient to handle. Four patients are handled by three student-nurses. We had also task to do which helps us to acquire basic skills. One student-nurse (I) for the procedure,another for the medicine (Mr. Alimbaba) and one for the charting (Mr. Abalos). We’re assigned at Pedia Main Pavilion III and my partners were Mr. Abalos and Mr. Alimbaba. Our patient was suffering from Miliary Tubercolosis Cat. II.
We finished our duty at 12 noon and that night of Tuesday, the post con was postponed and it was moved on to Wednesday.
I have learned that patient should not be neglected, given proper care and assistance, and most importantly communicating with the patient to be able to establish rapport.
DAY 3- July 15, 2009
On the third day of our duty, we were assigned at the Emergency Room. First, we had our orientation which took for 5-10 minutes for the rules inside the E.R. Afterwards, our C.I. divided us into 3 groups so I and my group mates were the first to assist. After that, we had patient who was suspected for leptospirosis. I observed that the patient was gasping for air to breath, has cold skin, and cyanotic.
I heard the doctor asking for the cardiac rate and the nurse replied that she can’t hear any cardiac rate of the patient. After that, the C.I. told us to look over for the patient.
I observed that my group mates felt pity on the patient’s condition.
I have learned that being observant and alert helps me to be a responsible student- nurse most especially if patient is in serious situation.
DAY 4- July 16 2009
This day was our pre con. I explained the overview of the Leptospirosis. My partner was Mr. Zabala, explained the pathophysiology of leptospirosis. After our explanation, I and my partner received questions from our group mates and our C.I. Some questions we did not answer was answered by our C.I. after the question and answer we proceeded in our duty.
During the duty, I and my partner was assigned to our patient who had dengue fever. That night we also had our post con.
I’ve learned that to be able to prevent diseases one must to take care of his or her self like proper hygiene.
DAY 5- July 17, 2009
This day, I and my partner was assigned to another patient who had dengue fever. We got his vital signs and we did also the charting.
That noon, we had our pre con about rabies. The overview was explained by Ms. Dumalanta, student-nurse while the pathophysiology was explained by Mr. Navarro, student-nurse.
I’ve learned that being observable helps me to get accurate information on my patient’s status.

OBSERVATION:
a)SELF
-it was my first time to exposed at San Lazaro Hospital. I was too excited on the said place. I also felt nervous as a first time always do.I enjoyed much at this area because I learned a lot of management use to be an effective nurse when time comes.
b)GROUPMATES
-like me, they were also their first time to expose in this institution, they also felt nervous just as I am. They were too busy in their own client when duty starts on the second day.
c)CLINICAL INSTRUCTOR
-she always monitor every students under her supervision and she always support us especially in applying the proper technicques of being a nurse in the field. She gives an overview before handling patient to be able to know how to manage and to be precautious especially if it is a communicable desease one. She treat us like her own siblings, she has also a good sense of humor as I always experience.
LEARNINGS:
I learned much on this duty, I was been one of those student who received punishment from the two clinical coordinators, We are grounded for almost four days after the said side trip at mall of asia. I want to apologize to what had happen last july 11, 2009. I just want to include my apologize through incident repport but some says "do not put any words to apologize on that letter because that letter need only the whole story of what you've done for the whole day". I sincerely want to apologize Ma'am to what I've done.Honestly I have no knowledge about the instructions given. I've learned a lesson on that part, Im sorry Ma'am! For being grounded, I utelized my time in studying my case analysis and also I saved money for three consecutive days.
On the field, i experienced to visualize those different diseases not just only assuming that there is a case like that but in reality.
I am very happy to be part of this duty because I know this part is the most highlight in the field of nursing.Before handling patient Ma'am Nhelia gives a quite overview of the disease of a patient and reminds also to be precautious because we might also acquire those diseases just in case.Thank you very much Ma'am for being patient to me even I was also a "PASAWAY" too as a trade name of our block.

(July 13-17, 2009)
I General Objective:
To develop and enhance our capability as we handle a patient with communicable disease.
II. Specific Objective:
To acquire knowledge about communicable disease.
To develop our skills on how to manage, prevent and formulate a specific nursing intervention to a patient with communicable disease.
III. Observation:
a. To myself- I enjoyed my duty at San Lazaro Hospital from the day of orientation up to last day our duty. I worked with my group mates cooperatively.
b. Group mates- Some of them were a little bit anxious to have heir duty at San Lazaro Hospital. But as day goes by, they were adjusted to their new environment.
c. Clinical Instructor- I think all of my group mates were very happy including me with Ma’am Nhelia as our clinical instructor because she makes us laugh that relieves our anxiety and stress during our duty. Ma’am Nhelia is a kind CI as long as you are not a “pasaway.” We enjoyed our duty with ma’am Nhelia at San Lazaro Hospital.

IV. Learning/Experiences:

Being a student nurse is to give all the best that you have. Putting your faith in everything you do will help you to maintain a good person because it also contributes to your intellectual aspect of life. But not only the faith we should have, we should also use our emotion in proper way especially when dealing with patient care. All aspects including spiritual, physical, emotional and mental play an important role in our life in order to become a good person.

On July 13, 2009, Monday, at San Lazaro Hospital, we were oriented at their AVR about the history of the hospital and its leaders and officials. The orientation took about 30 minutes. Then ma’am Nhelia and ma’am Louisa toured us to different part of the hospital including Pav. 8 and 4 at the 4th floor, Pav. 3 and 7 at the 3rd floor, Pav. 6 and 2 at the 2nd floor, Emergency room at the 1st floor and central supply room at the 2bd floor near the pay ward. We were not able to see other area such as TB, HIV and AIDS because of bad whether condition. Then after the orientation, we went back to the dorm and make a draw lots about the topic that we will be reporting. At night, Ms Manuel and Ms. Puruganan discussed about Dengue.

On July 14, 2009, Tuesday, we were scheduled at the Pedia ward. But before we handle patient, ms Dias and Ms. Casares discussed about Acute Gastro-Enteritis. Of course, we learned from their report and the supplements off our CI. After the reporting, Ma’am Nhelia assigned us to our patient. I did the charting and my group mates also did their function too like medicine nurse and procedure nurse. Then, on July 15, it was Wednesday, we were scheduled to have our duty at Emergency Room. Sir Lito Bagu oriented us inside the ER about the proper admission of patients. We were divided into two groups. The first group started their duty and on my observation they admitted almost nothing. Then we substituted them. On our time of duty, we admitted many patients. I think we have admitted 8 patients at that time. Although it was a little bit exhausted, at least we have acquired another big experience that enhances our skills in doing assessment and patient care. After the duty at E.R., at the dorm, Ms. Ortega amd Mr. Orias discussed about pneumonia then after a couple of hours Mr. Abalos and Ms. Obiena discussed about Typhoid fever. We also did our first quiz on that day.

Thursday, July 16, 2009, I and Ms. Baliwang discussed about leptospirosis before we start to handle a patient at 2nd floor Pav. 6. Successfully, we accomplished our part. Ma’am Nhelia grouped us again by 3’s. I worked together with Ms. Ortega and Ms. Baliwang. I did my part as medicine nurse. Ms Ortega did the procedure and Ms. Baliwang did the charting.

Finally, last Friday, July 17, 2009, we were assigned to have our duty at Pav. 3, 3rd floor. Ms. Dumalanta and I worked together with our patient. But before we receive our patient, Mr. Navarro and Ms. Dumalanta discussed about Rabies. From orientation and the start of our duty up to the last day of our duty, we have learned a lot at San Lazaro Hospital with the supervision of our clinical instructor Ma’am Nhelia.
These are all my experiences and learning.

OBSERVATIONS:
A. SELF:
I've felt excited, nervous and challenged during our duty in San Lazaro Hospital because many schools used to take their affiliation in this Hospital and the presence of different pt. which has different diseases and diagnosis that is seldom to encounter in province.
B. GROUP MATES:
I observed that they felt the same way as me, because it is their first time to have their duty in San Lazaro Hospital. they do their job as a nurse in this area and do not think that they are still student nurses.
C. CLINICAL INSTRUCTOR
She taught us and guide us in our duty at this Hospital. she correct us with our errors by humbly scolding us and put demerits with our performances.
LEARNING'S
I've learned a lot including the order of patho-physiology, the charting techniques in this hospital, preventive measures against droplet, health teaching with interventions, and new terminologies in medical terms.

OBJECTIVES:
A. General Objectives:
*To be able to aquire more knowledge about the different commnicable diseases that are present at San Lazaro Hospital.
*To be able to enhance our skills with regards in hospital setting.
B. Specific Objective:
*To widen our knowledge about communicable diseases and to understand how it was aqure, manifested and treated, underlying signs and symptoms and able to share to other people.
OBSERVATIONS:
A. Self: At first, i felt nervous because it was my first time to have a duty at San LAzaro Hospital. But then, i enjoyed it because our C.I gave some jokes to make us laugh. I was shocked seeing some diseases that are not common in Hospitals where we are affiliating in Isabela. And I am lucky enough to see a rabies patient at San Lazaro. During my duty in the said hospital, i learned a lot who mold me to be a better nursing student.
B.Groupmates: Some of them are cooperative, they've done their best in finishing their daily task, as they have shared to me, they really enjoyed our duty in San Lazaro in a way that they like the way our C.I holds our group. And they also exclaimed that at first they also felt nervous like me.
C. Clinical Instructor: I like the way she holds our group even though some of us are hardheaded sometimes in making our tasks. She guides us in everything we do and she shared a lot of things unto us, like, some tips in preparing and giving medications accurately.
thank you ma'am for the patient you've given to our group, for the things you've taught to us and we will applied it in future in our chosen career.
LEARNINGS: During my duty at San Lazaro Hospital, it molds me to be a better nursing students, new settings, new learnings, and new experiences.
In doing our responsibility as a student nurses, i found out to set aside nervousness to be able to perform our task accurately. I also found out how important the group cooperation in all our activity that we have done.
All those experiences and learnings during our affiliation at San Lazaro will contribute to us especially to be a more competitive nurse someday.

II. OBSERVATIONS:
a. Self
My first exposure to San Lazaro Hospital, I felt mixed emotions. I am excited yet a part of me was nervous. I am curious but a part of me was afraid that I could acquire a disease once I've entered the hospital. Also I felt happy that I made it to the level where I will now have my affiliation.
b. Group mates
I could see the excitement on their faces but I know they were also nervous inside like me. They were fun to be with and very cooperative in everything that we do during our duty.
c. Clinical Instructor
She was very supportive and jolly to be with. All through out our duty, she was there to assist us in formal teaching through our pre-conferences and supervise us in our clinical practice. Being with her makes my duty very enjoyable and memorable one.
III. LEARNINGS:
As we enter the institution, my first reactions was.. whooaah! Is there an outbreak in here? What I can see all around were people wearing those laboratory gowns and face masks. Then I realized, this is a hospital of communicable diseases., silly of me.. I almost forgot.
Although I already have foundations on communicable diseases from our past lectures, I cannot say that I am ready and confident enough to face patient's with communicable diseases. Listening have a big difference from experiencing. That's why my exposure to S.L.H. was very important 'coz I believe that clinical experience is the core of the nurse education.
Our five days experience to the institution, I have learned and observe a lot of things. I was so glad that the patient's and the S.O. were so very kind and cooperative. Some patient's still manage to smile despite on their conditions. They were open to answer our queries so we did not encounter any problems regarding with our data gathering.
I have learned that nurses exposed to this kind of institution was not that very easy for they are in contact each day with patients who are in incubation period of development of communicable diseases and were closely associated with people who are carriers of disease-producing-microorganisms. For this they have to protect themselves and safeguard other patients by observing principles of aseptic technique in practically all nursing procedures.
This learning experience is something that I would carry wherever God will send me to apply my chosen career in the future.

To my clininical instructor she is very kind and understanding to their student nurse, she impart their knowledge about the communicable diseases, she taught us how to care our pt with PTB and TB meningitis. and my groupmates they are all busy doing their works or responsibilities as a student nurse. that time i've felt nervous because its my first time to under maam Nelia Perez as my CI at San Lazaro Hospital
and ive learned how to mingle and communicate with other people, during our first post con im so, shy to my other class mate because i asked maam Nelia what is cilia during i discuss patho of our case i cant answer it immidiately because ive did not search to my dictionary. so this is a challenged for me. and ive leared how to care pt. have PTB, TBM and DHF. this is a great experienced to us as a student nurse. i enjoyed our duty at San Lazaro Hospital.

learnings:
i have learned a lot in San Lazaro Hospital even though our exposure there is too short.
i have learned how to make SOAPIE in a short period of time, i have learned from the diseases that i have encountered, i have learned from the pt's for their kindness and cooperation, and lastly i have learned from my mistakes but most of all for all the things that i have accomplished with this kind of experienced, all i can say is: that my skills is being developed and enhanced my knowledge is widen.

Learnings:
First day of our exposure are we have a short orientation at San Lazaro HOspital, after the orientation we are all saw they different parts of the hospital. At 12am our CI give to us the requirements must be passed to them. My learnings in the one week of duties in San Lazaro Hospital is exciting and challenging especially where we in Emergency Area because we have seen a different communicable diseases tha was admitted in the ER area. On how to give them proper rendering care.Ansd specially i learned on hoe to do my tasked with the help of my CI because she told to us not just only to present a kind of cases in front to read only, but to give little imformation regarding to the cases was presented.I thankful again to my CI, in that kind of policy she was been told to us i improved my self-confidence in front of our patient to discuss a cases and on how to prevent the diseases.!!
