Marcus Cesar L. Pineda
I Objectives:
a) General: to enhance skills in handling patients with communicable diseases and to gain knowledge in the manifestations, managements as well as the etiology of communicable disease and to experience actual care in this field.
b) Specific:
to appreciate nursing care for patient with communicable diseases as well as the signs and symptoms of the diseases.
to gain knowledge on how to care for patient with infectious diseases.
to familiarize the clinical set up and management for patients with communicable diseases.
II Observations
To myself: I have observed that I still need to improve more to be an effective nurse in the future. I also need to be resourceful and flexible at all times ion order to be a successful nurse someday.
To my group mates: I really appreciate the way they treat each other they are willing to lend a helping hand and they know if one needs assistance even if they are not asked to do so.
To our C.I. I am thankful that she thought us and handled us well. She attend to our concern and she wanted us to improved well and she treated us equally. I appreciate her eagerness to exposes us to every cases so that we will be able to appreciate them. She corrects our errors and after we are reminded so that we will not commit the same mistake again.
III Learnings
In my exposure at San Lazaro Hospital, I have learned about how they handle patient with different cases of communicable diseases. they were able to avoid cross contamination by placing patient with same cases in one room. I am really amazed with the system they have applied such as placing the patient which is more infectious at the far side of the ward. I have also learned about Herman's sign and the killing power and the strength of its communicability.
I know that affiliation is the highlight of nursing related experiences.I have expected a lot from this and I feel nervous I was never excited in this duty because I was anxious due to lack of information about the area we are assigned to but during our orientation I was relieved.
During the presentation of my case I begun to despise myself for not having done my task and I was not able to deliver the topic.It was the first time that I fail in presenting my case so it was so disappointing to me. My classmate look up to me as their older brother and I feel so ashamed with myself. I feel like I lost their confident in me and that they would no longer look up to me however I took it as a challenge and I swear that I will the best I can and that I will remember the teachings given to me by the Clinical Instructors here and as well as my previous mentors.
I have lot of unforgettable experiences during our affiliation in San Lazaro Hospital. we have encountered lots of gadgets that is new to us. one thing more is that I feel nervous whenever I am with our CI I feel so intimidated and afraid because she is our academic coordinator so i have to do the best that I could then i realized that my fear is not right, I should fear my Clinical Instructors because of respect and not because of the duties they bestowed upon us I know that they only want to bring out the best in us.
Regarding my classmates even if we have different issues in life we still manage to get along well. We settled disputes as early as possible.
I was a resident of Sampaloc for almost seven years but one time I happened to misdirect my classmate in returning to our dormitory. I guess I was preoccupied by the time and since I was familiar with this place I was always asked by my classmates on directions and where to ride.
I love my exposure and I wish that our exposure would be extended.

II. OBSERVATIONS:
a. SELF: I have noticed that I need to be more attentive and knowledgeable regarding on handling patient of San Lazaro, its not easy for me to adjust from the duty I've experienced at province because its a big different here at San Lazaro.
b. GROUP MATES: All of us are willing to learned and have an experience that's fruitful for our future references. They help each other for easy work. In pre-con, post-con, and ward class, they make their best for the presentation.
c. C.I.: She's a kind of woman that's keen observer and joker. In teaching process, she’s one of the best teachers I've ever met. Effective in her strategies in teaching. She’s strict in the sense for our improving as a future nurses someday.
III. LEARNING'S:
I'm lucky because I reach this stage of BSN course. In my exposure in San Lazaro Hospital, it's been a kind of experience that I've never forget. In handling different cases of communicable diseases, I learned on how to care for my self and to my client. I appreciate the system of this hospital, in different pavilion, there's a separation of room of patients of same cases, (like if it is dengue, that's only dengue cases), proper handling of patient by the Doctors and Nurses, the observation of cleanliness all the time. Preventing of cross contamination of infections for the client and health workers safety.
On our first day of duty, our C.I tour us in this hospital,"naloka ako sa laki ng hospital”, it’s just like the same starting at down to up, I thought that I will be lost during our duty, but we thankful because our C.I always there to guide us, but some of my group mates have been lost. We had also our orientation on that day, we sat at the aisle of their AVR.In those pavilion which we had our duty, they always do the brief orientation for us.Different schools we have been met also. There’s a factor that I felt nervous sometimes.
Having a duty at San Lazaro is a big factor on my career, because I observed and learned that you must be an attentive, intelligent and aware in your health and for your client for the effectiveness of it.
I am happy and thankful that I experienced this affiliation here at San Lazaro.

II. OBSERVATIONS:
a. SELF: Since it’s was my first time to have a duty in San Lazaro Hospital, I felt nervous at first, as the time past by, it’s been a reality to become attentive and more knowledgeable in handling patient.
b. GROUP MATES: In term of cooperation, they were all in. In doing our pre con, post con and ward class, we had a time to help each other for the good outcome of our work. In rendering health services to our client we make the best all the time.
c. C.I.: She’s the one of the most honored clinical instructors of Northeastern College. She will guide and corrects you in your duty. She like an orderliness of your work.
III. LEARNINGS:
I found my duty at San Lazaro a very exciting and at the same time rewarding. It was also a good experience for me because my skills are being enhanced especially that which pertains to communicable diseases. Our first day of duty was more on orientation about communicable diseases and being a student nurse, it was my responsibility to know the rules and regulation, history of the hospital, administrative staff and special areas of the hospital.
The next day of duty we render care for our given client, we handled 3 patients in our group, everyday there a 2 member of the group to get the supply on the Central Supply Room. As we go on in our duty we assigned for several tasks, new knowledge again imparted to us in the area. I’ve learned a lot from you mam.

Observation and Learning’s:
During our 1st day, we had an orientation accompanied by our Clinical Instructor with Ma’am Nhelia and Ma’am Luisa, they also tour us inside the Hospital premises. Traversing long stretch of the corridors with the different pavillion’s and their respective locations after our tour we immediately proceed to the hospital amphitheater for a brief orientation about the history of the Hospital the different department and its department heads and the rules and regulations inside the hospital we had our first set of post conference reporters who discussed about the topic of “Dengue Fever”.
On our 2nd day we had started our first duty at Pavillion II catering to adult female pt. ages 20 up “before we started our pre-con. Reporting with the topic of “Pneumonia”.
On 3rd day of duty at San Lazaro Hospital we are in Pav. III catering to adolescent pt. male and female ages 13-19. After we care our pt. we started our pre-con. With the topic of Varicella. I feel nervous when I deliver my report.
During 4th and 5th day of our duty we are assigned in ER and Pedia Extension and in the pedia extension we really see and appreciate the Dengue with Petechiea rash and some of my groupmates they discussed about the overview, sign and symptoms, management, and treatment for the Dengue, my groupmates are very supportive and easy to approached, some of them are “KALOG” they are very cooperative. I am happy that they are my groupmates.
Our duty at the SLH is our lifetime experience because I have learned a lot of things In the help of our Clinical Instructor, Thank You very much ma’am for always giving your ideas, knowledge and skills on how we take good care of our pt. most especially in communicable diseases. To experience hospital setting duty at San Lazaro Hospital.To observe the communicable disease @ San Lazaro hospital,its signs and symptoms, managements, treatments and prevention of a certain diseases.

to see the manifestations of a certain diseases in reality.
to gain knowledge to the proper interventions of the infectious patients.
to expose with the different communicable diseases and able to see the differences.
II.OBSERVATIONS
a.SELF
At first I was very nervous as well as excited. Nervous, because it’s my first time to go at San Lazaro hospital and dealing with the patients who have communicable diseases. Excited because I want to see in reality the manifestations that are seldom to see in our base hospital like the person who has petechiaes and other signs and symptoms of the diseases.
b.GROUPMATES
My group mates are really eager to learn, and enhance our skills in different areas. I can compare them as a collection of ants because where the one goes the other follows that really make the bonding moment of the group. I really appreciate the way they boost the confidence of each other.
c.CLINICAL INSTRUCTOR
At first I was scared because it’s my first time to be with her, the first two days was really an adjustment days but the succeeding days was really full of happiness to be with her. She really made everyone at ease with her. She really supervised us and taught us many things @ San Lazaro. She made sure that we learned a lot from the exposure we had. I'm very thankful that somehow I experienced to be her student because it's an extraordinary moments to be with her, not just the knowledge and skills but also the good deeds, and encouragements that really made us not just a better nurses in the future but a better individuals.
III.LEARNINGS
I really enjoyed a lot and I really appreciate those things I saw at San Lazaro Hospital. There are lots of thing that I learned from San Lazaro as well as our nursing fields.
First, viewing the nursing career as a calling because it’s a gift to have therapeutic skills in order to render a genuine care to those people who really needs care.
Second, dedication and willingness to help, I learned that nursing career have lots of responsibilities. Sacrifices are really connected with the career because it is a patient-centered and sometimes affects our personal lives but on the other hand the heart to help is always still there because we are considering our patients as our own family or relatives.
Third, it is always guided by knowledge because how can you render quality and effective nursing care if you do not know.I am really inspired to the staffs because there health are really at risk, regardless to there situations, they really give importance to the pt. as they are doing the right interventions to them.
There are lots of memorable moments at San Lazaro especially the bonding. It was one of the very exciting parts of being a nursing student. There were times that I'm enjoying the laughing moment of the group even though I'm the subject of the ridicule because we saw a 2 students that are darker than me and my group mates said " dolly pumuputi ka" and i said why, they pointed the two students and I'm really proud that i am lighter than them. They always teasing me but its OK with me at least I made them happy. And when I was assigned to get the supply, i was laughing because we don't know where we will return the supply because the hospital had the same structure so we were very confused to the extent that they went down and saw as arguing where the supply room is until they went with us.
Overall, I really enjoyed a lot and it was one of the memorable parts of my life.

I.OBJECTIVES
a. General objective
· To gain knowledge and new experience in a tertiary hospital or in a hospital with special field like San Lazaro Hospital.
b. Specific objectives
· To know how to manage patient with different communicable diseases.
· To gather new learning about san lazaro setting.
· To determine the big difference between a tertiary hospital in urban place and a secondary hospital in a rural.
II. OBSERVATIONS
a. Self
At first I am very nervous and excited too. Nervous because I don’t know the setting in san lazaro and I don’t know what I should do to help or render and apply my independent nursing skills that I’ve learned in the school. Excited because it was my first time to go in hospital like san lazaro and see some cases that I rarely see in QPH.
b. Group mates
I observed to my other group mates like me they are also nervous. We had same feelings on going in said hospital. Like me they also want to extend our exposure in san lazaro to see cases that seldom we encounter.
c. C.I.
She is very jolly. She knows how to communicate with us even there is boundaries between C.I.s and students she knows how to handle us. She is not toxic C.I.. If we had questions she’s there to help us.
III. LEARNINGS
On the first day of duty we had a tour at san lazaro hospital and an orientation regarding our stay, rules, setting and a brief history of San Lazaro Hospital. On the 2nd day and previous days we had our actual or hands on duty but before we get our patient the staff will orient us regarding our responsibilities in the ward. We had our own patient to handle. Sad to say we didn’t experience giving IV medication because that is the duty of the doctors in the hospital. I saw patient with petechial rash, TB meningitis, a pt. with congenital problems, and pt. with severe TB cases. We had our cases presentation regarding aH1N1.
My unforgettable experience in that hospital was when we’ve been almost lost. We can’t find our group mates and C.I.. so we decided to went out and wait for the van. Suddenly Mr. pineda saw us sitting out side and he guide us in our way to go back.

to conduct our duty at San Lazaro Hospital(SLH) and have a knowledge about the different cases in SLH and how to manage the different king of communicable diseases we will encounter that will also lead in developing our skills as a student nurse.

To give or conduct nursing intervention to help for the recovery of the patient.
II. OBSEREVATION
A. Self
At first I was excited but then nervous also, excited because it was my first time to go there and nervous for the things that we will going to do because I am not familiar with the different routine, but it was also relieved because we were oriented.
B. Groupmates
Like me I think they are also excited. They are also fun to be with and helpful.
C. Clinical Instructor
She is very knowledgeable and shared her knowledge to us. She is a jolly person and fun to be with. She is dedicated to her work and approachable too.
III. LEARNINGS
On our first day at San Lazaro Hospital (SLH) we have a tour and on orientation about the history of SLH, vision, mission, and the different staff. On our second day we have our duty at adult female ward, 3rd day we have our duty at adolescence ward which are ages 13-19 y/o, 4th day in the emergency room, here I found out that if the cases is severe they admitted it but if the cases are only cough, colds and etc. they sent it in the OPD and in the 5th day in the pedia extension which are from birth to 12 y/o. during the days of our duty , in every ward we have our orientation. We encounter different kind of communicable diseases like Varicella, pneumonia, TB meningitis, cases of dengue fever and many more

I felt nervous during our first exposure because its my first time at San Lazaro Hospital but Im excited to render care to patient with communicable diseases. During our duty I’ve become more careful in handling a patient to protect myself as well as them. And of course Ive educated them regarding proper hygiene to minimize the spreading of the disease.
GROUPMATES:
They enjoyed their work, they are all cooperative everybody was working during our duty and they finish their task at a given time. They also share their knowledge regarding the diseases they encountered and I like them for being an open minded coz they accept others idea.
They are happy to be with….I’ve love this company..!

LEARNINGS:
I’ve learned a lot @ SAN LAZARO HOSPITAL, about the different communicable diseases on how it manages and treated properly and on how to mingle with different patient’s. I’ve became more observant especially when it comes to sanitation and became responsible with my task .We also conducted ward class regarding dengue hemorrhagic fever,on how do the disease spread,and the proper treatment and management.We are happy because they understand what we are taking about and some of our patient’s interact with us.All I can say is you should be willing on what you are doing to avoid mistakes and you must love your work so that you can render quality care-… always remember that we are dealing with lives.

b) Specific Objectives: for us to be able to gain knowledge in analyzing different kinds of cases of communicable diseases from its etiologic or predisposing factors , know the signs and symptoms, management, treatment and how to prevents those kinds of diseases. II. Observations:
To myself: I felt that I still need to augment and improve my skills in nursing and be more familiarize regarding proper practices in conducting proper interventions to be an effective nurse someday.
To my group mates: Everybody cooperates and they are all eager to become skilled in this profession. Everybody help each and everyone for the improvement of each presentation. They did their task well.
To my Clinical Instructor: I have been ma’am Nelia’s student before, I have noticed that she was really intelligent and skilled nurse. She has this kind of potential wherein every student will like and I am grateful to be her student once again I really enjoyed. She makes sure that everybody is doing their task well. She taught us things that can help us to improve our abilities in this field of nursing and correct our mistakes either.
III. Learnings:
Thank God I have reached this level as a fourth year student nurse, lucky that even there were problems I have encountered those past years I’m thankful that I passed to be one of the affiliates. I told myself that this will not be the last time to be here. I’m so glad and at the same time nervous at first. We have been to San Lazaro Hospital wherein any kinds of communicable diseases are present. This experience at SLH was a treasuring piece of mine. The first thing that I have learned is that, how am I supposed to do to protect myself, my patient and even my co-student nurse from cross contamination? Of course, by practicing those proper care proper techniques that can help us to prevent contamination; like hand washing as WHO said, it was the first base line for protection.
During our first day duty at the SLH, we have our tour at every pavilion. I noticed that they maintain the cleanliness of every area unlike other hospitals at the province. San Lazaro was one of a kind, even if it is a government hospital it is really wide, clean and good for patients that are suffering with such diseases. I noticed that any patients with same kind of disease are being at one area. I was impressed to all the doctors on duty because they are doing their best to help patients. They were responsible even the stuff nurses.
On those past few days of duty I recognize that everything should be cooperative and has this presence of mind in order to provide a functional nursing care to patients. I have also handled patients suffers from dengue, CNS infection, patients with varicella, and encountered patients with wound infection at the ER. I have learned how to manage those kinds of diseases with the help of our case analysis, case abstract and of course by the help of our CI that explained and gave us further information and rationales in giving care for the patients we have handled.
I really enjoyed this affiliation and thankful to be a part of the group with ate dolly, kuya red and of course to with ma’am nhelia

OBSERVATION:
Self
During our first day of duty at San Lazaro hospital, I felt nervous and excited. We exposed from the different wards. I became observant with the patient. I found out also the importance of time management especially when doing a case analysis.
Group mates
They are fun to be with them, they always make me laugh especially the joker in the group. They are also supportive and taught me on the things, I didn’t know yet.
CI
To our CI, ma’am Nhelia Perez she is kind, approachable and also a joker, she makes us laugh. She made sure that we had a lot of things to acquire before leaving the hospital. She always made the day complete by making us at ease and saying a joke that really removed our tiredness the whole shift.
LEARNINGS
I developed my self confidence and also my communication skills, I gained more knowledge and information about the diseases I encountered at different ward we had made our duty. We are given also the opportunity to saw and handle patient suffered from different kind of communicable diseases such as Pt. with varicella, and TB meningitis. All patients I encountered and handled was kind and cooperative. I learned to become independent and used my time wisely in doing abstract and case analysis. I am thankful for all the experienced and learning I gained.

I am was very nervous and excited on my duty because this duty at San Lazaro is the most exciting part of the affiliation where it risk your life as a care provider from the different communicable diseases.
Group mates
They are all cooperative they help each other especially doing case abstract and etc. I enjoyed being with my group mates.
Clinical Instructor
She’s very knowledgeable, you were not feel any toxic under her… she is willing to help, taught, informed us whatever we don’t know regarding our duty though sometimes I feel aloof because of so many problem we gave to her regarding our Block….. Sorry about, it ma’am.

Learnings.
Our 1st day is more on orientation where they shared us the significant informations about the institution. Though the institution is about 432 years I can say that it was a very clean hospital I ever seen. From there I encountered a lot of diseases where only read in the book. The succeeding days were really enjoyable and memorable because of the simple smile that our pt. showed symbolizes that they are also thankful for what I’ve done to them. In building relationship with the pt. you have the real motives to care for them so that they will cooperate and trust you for the thing you are doing with them. I gained the confidence that I can do the things that can alleviate the sufferings of my pt. I was very lucky to be with them because I learned many things not only for the nursing career but also to be a effective nurse someday.

Objectives:

Observation:
* Self: I felt nervous and was so excited because its my first to have a duty at San Lazaro Hospital.
* Groupmates: They are cooperative, kind, industrious, caring and discipline.
* Clinical Instructor: I observed to my Clinical Instructor that she is very kind and very supportive. She is always their to guide us.
Learnings
There is nothing more jovial than be a part of the Manila Affiliation. Well, On our first day of duty at San Lazaro Hospital we had oriented to the set up of the hospital, to the staff and also the policies , They also oriented us about the history of the hospital. After the orientation we have our hospital tour together with our cute Clinical Instructors Maam Valencia and Maam Perez. On our next day of duty we are already assigned for our responsibilities, at this day we are assigned at Pedia Ward. Most of the cases that we are handled are Dengue Hemorrhagic Fever. On our 3rd day, we are assigned at Emergency Room, Being exposed at E.R. is not easy because you must be a keen observer, active and you should know what are you going to do. On our 4th day of duty, We are assigned at Male Ward, Most of the cases their is also Dengue Hemorrhagic Fever. On that day I assigned to do Ward Class together with my partner Ms Palaming and Ms. Rosete. We discussed about Malaria, the signs and symptoms and how it being prevented, we also rendered Health Teachings. On our last day at San Lazaro Hospital, We are assigned at Adolescent Ward. Some cases their are Meningitis and Leptospirosis but most of the cases we handled are Dengue Hemorrhagic Fever. After our last day of duty at San Lazaro Hospital, We have a picture taking together with our beautiful and very supportive Clinical Instructor Maam Perez.
Manila Hospital are far different as compared to our Local Affiliating Agencies. It made me realize that as Nurses, total patient care is our concern. Being exposed into different ward of San Lazaro Hospital developed my knowledge and skills better. It was a worthy experience and it has fueled my desire for being a better nurse. In every exposure, we showed that NCnians are naturally caring, industrious and well disciplined. Thank you Maam Perez for being a good Clinical Instructor and for guiding us. GOD bless you.

Observations:
Self : I am very excited and confident in handling my patients however, i was oftenly distructed by frequent queries of my groupmates even in the dorm. I wish that i should handle different cases with classical manifestations but my team were only exposed to Dengue & miliary tuberculosis due probably to agency policies. My exposure really was very limited that i did not met my expectations.
Group mates: They are good but some are doubtful of what to do, what to write & what should be done in a routine manner. Sometimes the group was noisy that even other dorm occupants are distructed & some do more roaming around/ shopping rather than doing their paper works, They rush up very often that resulted to ineffective delivery of their cases. In short, case studies are not their priority to learn to some during the affiliation. It is true that " It takes a lot of brain to become a Nurse".
C.I. Very supportive, understanding & a true mentor since she asissted affiliates in accomplishing hospital task very often. She reminded SN's from time to time & supplemented discussions, giving general leads & information - a therapeutic approach on the part of a learning process for the students. She was approachable, jolly but sometimes toxic however, at manageable, tolerable & minimal toxic effects jajaja.
Learning: The 5- day affiliation at SLH enhanced my competencies in the following aspect:
1. Familiarized in the whole ward set-up - confinement of same patients / same cases together to limit or prevent possible cross infection w/ other patients;
2. Equipped w/ some of the policies in medical treatment especially to severe cases of meningitis or any severe infection w/ CNS manifestations;
3. Gained more competencies in handling Dengue & milliary TB patients particularly in the implementation of effective nursing interventions & medication approaches;
4. Acquainted w/ the systematic & strategic & effective traffic flow of patients especially to H1N1 close watch monitoring;
5. Familiarized with the common medicines used in SLH especially in Pedia, adolescence and male ward;
6. Developed sense of responsibility & rendered health services to other patients not assigned to me especially in necessities & those without SO;
7. Able to assist on duty physicians and given outright nursing care services whenever called or whenever necessary;
8. Observe some clinical cases at a distance and understand well the importance of proper recording;
9. Developed my patience w/ my team mates & fellow affiliates thru their frequent queries and asking supplemental discussions about their cases

Comments and Suggestions:
The affiliation was educational, skills oriented but it was a very tiring task & journey especially in case report preparation w/ a very limited time & resources. It is very significant however as a training venue of future nurses who want to expand his/her vision on health promotion especially if collaboration w. other hospital will be needed in his/her job.
It was a very challenging endeavor for SN's since resourcefulness, proper budgeting of time and testing of self capability will be developed, how far my knowledge have reached about the cases. It is really a requirement, that a nurse should be smart, alert, fast thinker, creative, resourceful and should has a good decision making for her/him to accomplish the immediate & necessary needs for his/her patient, and accomplish effectively his/her assigned task and responsibilities. Maniana habit was the main problem of most affiliates, prioritizing less significant things - personal needs, indulging themselves to less important events rather than self empowerment, knowledge acquisition and skills enhancement. For me, it was a very challenging journey, it makes me wise and vigilant anyway.
With all the good benefits of the affiliation, despite of a very short time and a very limited exposure, i appreciate so much every effort done by my fellow affiliates but most importantly our Clinical Instructors who sacrificed their time and effort. There might be unintentional things that somebody did thru words, actions or gestures even myself. We are all prone to commit errors, mistakes & worthless deeds unconsciously -- MY HUMBLE APOLOGY to CI's an my fellow affiliates.
What is important now, we are in the same family NCnian - we are to uphold our institution's name wherever we go. I am very proud to be 1 in the family. As both mentors & students have their own respective responsibilities, very significant importance in the existence of any academic institution that both should not be neglected but rather exist with all the courage, commitment & dedication to work, study together in harmony with mutual respect. General objective:
To strenghten our knowledge, develop skills and proficiencies in CD/ infectious diseases and to render effective, efficient, quality care to patients with proper approach, right attitude coupled with commitment and sense of responsibility in health promotion and care,
Specific objectives:
1. To gain information about the history, vision, mission goals and objectives of SLH;
2. To familiarize with the different facilities, pavillion, policies, guidelines and all the services rendered by SLH pertaining to health care;
3. To stenghten our knowledge on CD / infectious diseases, its distribution,ocurence,supportive therapy ,medication,treatment thru actual observantion & pre & post conferences, interaction w/ patients & observation of cases;

I. Objectives:
a. General
To be able to enhance the experience via encountering the different communicable diseases in the hospital
b. Specific
To apply the knowledge about the specific interventions based from the appreciation of the clinical manifestations of the handled diseases of the patients
To be exposed naturally from the diseases to develop skills and learnings effectively
II. Observations
Myself_ I have observed myself as part of the clinical team to be an effective nurse someday. I'm being more competitive as I encountered a lots of new experiences taking care of those patients with communicable diseases.
Groupmates_ I've obsered them with how are they learning to intervene with those needing patients. They are very participative with our c.i.
C.I_ Ma'am Nhel is very intelligent sharing her knowledge about communicable diseases dring our case analysis. She did very well her function as our mentor and she always help and encourage as to bring- out the best in our selves especially in intervening with our patients. I considered her as an influential motivator.
III. Learnings
I've learned how to intervene effectively to the needs of my patient by initiating t with my clinical instructor.
I will not ever forget my patient who's suffering from TB meningitis because he is nearly 4 months in the hospital of San Lazaro. I've done my part to give him the unconditional care that he needed.
I will not also forget my patient who is suffering from A.G.E because he is a malnourished 7 y/o child. When I've done skin test to him, I felt very sad causing him pain because he was crying silently with small amt of tears flowing fom his eyes...
I am thanking ma'am Nhel for her kindness and patience during our exposure in the San Lazaro Hospital.

-to be aware on the signs and symptoms of different diseases that we encountered in the hospital to know the preventive measures regarding on the cases handled
to give prior intervention on the nursing problems of the patient to render care and give health teaching to our patient
Observation:
self-i was too anxious with the communicable diseases ive handled and encountered,but as i go on in the field, i think so positive because it is a part of a nursing profession to comply obligations
Classmate- they we're all cooperative and participative,we work in hand,we have nice bonding with each other
CI- I'm very thankful for having a mentor like ma'am Nhelia for being our CI. She always there to guide us in times of our weaknesses in the clinical field. Thanks ma'am for imparting your knowledge to us.GODBLESS...........
Learnings-During our stay at San Lazaro Hospital,ive learned a lot of things on the different cases we handled that I seldomly seen or never encountered in provincial hospital,those are unforgettable moment of my life because ive encountered those types of diseases that ive not seen yet before,from the presconference and post-conference we had,ive learned a lot from those studies that might help us in the clinical area which increases our knowledge,those imparted knowledge might help us in our profession for our protection.......

Observation:
Self- i was to exited and nervous, because its my first time to handled and seen different people with the different communicable diseases. I did my best to be able to perform well and to accomplish the task and obligation as a student nurses.
Group mates - they are cooperative, the did there best to established their duty and obligation to their respective patient, we are all willing to learned so that we can able to cope up those responsibilities assigned to us
C.I- our C.I is a keen observer she is always their to facilitate us and observed on what we are doing, she imparted her knowledge and skill to us, so that we are able to established our therapeutic nursing care to our patient. Maam thank you for sharing and giving your advise, we learned a lot from those mistakes it enlighten us to be struggle and study hard and to fulfill those dreams, that may we be a competent nurse someday. Learning’s:
- during my stayed at San Lazaro General Hospital, there are many things that i have learned, it widen my knowledge my skills regarding the different nursing intervention, nursing process that appropriate to the patient having communicable diseases. through the different cases we handled I learned a lot from those, because ive seen a lot of patient having those communicable diseases that ive not seen before. Those signs and symptoms, manifestation that arise to the patient. Through the post con. and pre con,ward class,Ive learned a lot from those because its very important to study those diseases for us to know and understand and appreciate the beneficial aspect of conducting study. thus leading to the restoration of the patient health and vitality. Because prevention is better than cure. that experience serve us our guide to emphasized that the health is very important to everyone.
i enjoyed a lot and i learned a lot, this serve as my stepping stone to my chosen field of career...........
