General objecives:
To apply what I've learned in the school.
Specific objectives:
To perform basic nursing care to people who are sick.

Observations:
Group-mates: I observed that they are willing to learn and they have the patience to learn more.
CI: She is great and also i can say that she is smart enough to produce a smart student.
Self: I was so nervous this time, because i have done many things that i had never did before.

Learning:
I've learned a lot such as to make my patience long, to listen to what others says about my weaknesses and how will i improve it and of course, I've learned the right basic nursing care procedure.

RHU-day 2

General objective:
To be able to help the staff of CHO in rendering care.

Specific objectives:
-to apply my knowledge and learnings
-to gain more experiences
-to know the health problems of different patient in CHO
-to give health teachings appropriate to a patient

Learnings:
I learned to apply the right basic nursing care procedure with my patient. In our level, they only allowed us to do the basic procedure in nursing but if there’s a chance that the staff of CHO ask me to help in such things that’s not belong to our job. I’m not hesitating to help them because that’s one way of learning and improving my knowledge and skills. But take note, with permission of my C.I and it depends if I know that procedure or something. Like, my experienced, mam faith asked me to helped her by admitting the patient whose dog bitten and she allow me also to inject. In that situation, I feel motivated, I feel that’s were really part of the team of CHO. Well actually, we really belong.hehe
In this day also, I’m so lucky to witness/observed the operation “TULI” of many young boys of community. As I know, this would be the last operation “TULI” that will be held in RHU. So, I’m so really lucky, not only me but also my group mates. As I observed, my misconception about circumcision has faded away. Now, I know already how’s the circumcision be done and another learning for me to share. Funny thing is, “natatawa ako sa mga itsura ng mga young boys nun”. You can see that they really scared. And as a nurse, I will be the one who ease their scariness by talking with them. So MASAYA. 
So good to learn!

Observation:
Self- I was able to do my job well at the same time learning a lot of things. Since my duty started, I already gained more things that will help me in my profession. This will also give me more determination to achieve my goal.

Group mates- I observed that they will give their best to render outmost care with their patient. Good for them.

Clinical instructor- although I haven’t so much bonding with my c.i I still thankful for being one of her student because she’s not only make me happy but she’s more motivating me in my dreams same with my group mates.

GENERAL OBJECTIVES:
From 8:00 am to 12:00 pm, in day 2 in RHU, my duty was not that hard and full of pressure like in the admission in day 1, because i was assigned in the Dental area, to apply my knowledge in taking VS.-to be able to master again and again taking the Basic data we must have-VS, and with my short time there at the dental, they taught me about dental specifically, the procedures. Dr. Leo also showed me the injection site for anesthesia, and i was honored. And one of my objective is to learn something that i haven't read yet, but i already saw on actual because experience is different from theory.

SPECIFIC OBJECTIVES:
To be able to perform well the basic data we need to collect from our patients because with these alone,though it's just a basic, it's not simple and these will be their base to diagnose patients.

OBSERVATIONS:

GROUP MATES:
They were not that bad actually especially in the "operation Tuli" during the afternoon. They agreed,some insisted to stay and extend our time just to be their in the circumcision action as one of the programs of the City Health Office and that was a big sacrifice for nursing students like we are.
Their health teachings were very nice, the support they showed to the kids was honored by me, it was great to experienced it.

CI: she supported us and treat as like her own children. she's really jolly and with that alone, she made the group 4 really happy and our determination was always their to perform the best that we can do because she also showed us her determination to teach and instruct us.

SELF:
I was blushing during morning because Dr. Leo is cute,hehe,I am so malandi talaga. i was one of those students who insisted to stay and observe how to circumcise, but i experienced nausea,my stomach function unwell and had a headache. But at least i just focused my attention to the procedure and it made me feel great full.

LEARNING'S
I learned a lot from the circumcision alone by observation and by the mini lecture regarding the equipments that we saw. It was awesome, my words are not enough how to express my happiness to observe it. I always laughed before when i saw guys who circumcised, with all their SAYA'S,hehe, but the observation made me realized i should not laughed at them because it's not that easy to be circumcise.

General Objective: To be able to increase more our knowledge with the responsibilities in the CHO as well as incorporate new concepts learned from the classroom to the actual duty area.

Specific Objective: To be able to be more alert to the community people we are going to get their v/s and render our care and be able to enhance our skills regarding nursing care.

Observation:
As our second day of duty in the CHO, Mrs. Perez already assigned us to our different area. We are divided into 3 groups. The first group were assigned to the Treatment Section, Dental Clinic and Admission Area second and third group respectively. I observed that there were few community people we are going to get their v/s not just like the passed day. Around 10:00am, no more patients were admitting to the area. So we just observed ma’am Rose for here lecture in Animal bites and we assist her for injecting anti-rabies for the patient. After a few minutes, our given task was already done. We took our lunch break early for us to be well prepared for the “Operation Tuli” as the program of the institution. We are lucky because we are given an opportunity to assist and observed a case “Operation Tuli” in which, we can enhance our skills and knowledge regarding nursing practice and giving a care to our patients.
When dealing with my group mates and observing what they are doing, cooperation can be seen to them. Their concentration in observing really focused in the said operation. So much fun together with them.
To my C.I. she is approachable and efficient. Though sometimes I observed she felt a little irritated to what we are doing. I like the way she treated us because she expanded our learning experiences.

Learning:
Along with this experience, I was able to learned another experiences. I have learned the different situations and procedure done in doing the circumcision. I have learned how to get along with different persons and I was able how to adjust to do things and more effective students nurse in the area.

Specific Objective: To be able to be more alert to the people in the CHO and render our care and be able to enhance our skills regarding nursing care.

Observation:
Self:
On the second day in the CHO,Ma'am Perez already assigned us to our different area. I was assigned at the Treatment area. There was alot of people seeking for our help. my first patient got an injury in his left foot and i assisted ma'am mildred in giving him his medication. i also helped in getting the anesthesia into the syringe. After a few minutes, our given task was already done. We took our lunch break early for us to be well prepared for the “Operation Tuli” as the program of the institution. We are lucky because we are given an opportunity to assist and observed a case “Operation Tuli” in which, we can enhance our skills and knowledge regarding nursing practice and giving a care to our patients.

Group mates:
Cooperation can be seen with my
group mates. Their concentration in observing really focused in the said operation. With them i had experience a lot of fun.

C.I.
Ma'am Perez is approachable at all times. I like the way she treated us because she expanded our learning experiences.

Learning:
Along with this experience, I was able to learned another experiences. I have learned the different situations and procedure done in doing the circumcision. I have learned also how to get along with different persons and I was able how to adjust to do things and more effective students nurse in the area. and most of all i had learned to believe in my strength and skills.

General objectives:
>to widen our knowledge about getting vital signs as well as assisting the patients and also how to apply what we had learned about the past return demonstration in the school.
Specific objectives:
>to enhance ourselves by doing such different tasks and some procedures like said in the general objectives.

Observations:
*Group-mates
>As i observed them honestly, the others are doing good and some are not specially in getting the simple vital signs but it is also because we are just a novice in this profession, but then, in the last we made it better than i expected.
*CI
>Maam. Perez gave us our own area of responsibilities, she ie great companion whether he is slightly strict, honestly i thought she is a drag racer but she is not. she is doing well.
*Self
> As a 1st timer in the City Hall felt great, unexplained nervous, maybe because i know that my ci is looking at me..

Learning:
>I learned a lot of things by doing those said procedures that i did and also how to communicate people living in a community.h

General objectives:
to apply what i've learned in the school and for me to enhance my skills and broaden my knowledge.

Specific objectives:
to do my duty as a student nurse like getting the vital signs of the patient,to assist and assess the patient and etc..

Observations:
Groupmates; they are all funny and i observed also that when we are in the field performing our task they are all serious.
CI: shes so intelligent and approachable.
Self: I'm so nervous because theres so many patient

Learning:
i learned how to be more confident about my self.

Observations:
Self: Now, I have a confidence in getting the vital signs of different patients because of the proper instructions given by my C.I. Thanks to her with first three (3) young boys, I was shocked with the processes, but as the patients come and go, I learned to be calm so that I could give the due services and assistance to the patients.
Group mates: They worked hard as I do and wants to learn just like me just the same.
C.I: I can say that my C.I is a cooperative, approachable and gives proper instructions in due time. She is not just a C.I observing students to their works giving instructions and etc. But also a mother, a good friend that gives you jokes, make you smiles in time of stressed and tiredness and troubles too.
Learnings:
I was so excited to faced my second day of my duty to assist circumcision among young children. At first, I feel excited, but ours went by, I am weakened by the blood I had seen and by the cry of children of pain they feel. But I have to brighten up and strengthen them. Giving them a nice top on the shoulder and tell them that its just normal and its gonna be fine. Thank you that I was able to share to my community my simple services. I had learned to give more due respect to the kind of people I have to mingle everyday, to give my “HONEST” and sincere services is the top of all the things I do, showing smiling face even so tired and makes me fully satisfied of my days activities.
