General objectives:
To be able to widen my knowledge and to be more confident of my self by doing such task.

Specific objectives:
To perform what I've learned and to apply it correctly.

Observations:

*Group-mates
> i observed that some of them are not that good in taking the vital signs of a patient and some are nervous to do it but i see to them that they are willing to learn and they are striving harder to be a good student nurse....I enjoyed being with them.

*Ci
> im so lucky that she is my ci, all i can say is she's so good...

* self
> i think i had a lot of improvement: to my self, my skills, and knowledge...

Learning:
> I've learned a lot, not only how to improve my skills and knowledge but also how to deal with people and how to mingle with them in a good way...

I. General Objective
To improve my skills, knowledge and practice. For i am to provide appropriate nursing care to the client having diseases.

II. Specific Objectives
To establish a nice and good relationship with our clients. To make the best and do the task easily for the last rotation.

III. Observations:
Self:
More confident and more efficient in doing the job. I was assigned in the admission area. I assisted the Staff to find the records of the patients, helped in taking vital signs, report of the illness of the patient, taking the temperature, height and even the weight. I enjoyed my last day in the CHO.
Groupmates:
There is two of our classmates just attended our duty on the last day. But despite of that matter they were easy to teach whaht to do. And of course maybe we have the same feeling, tired.. Toxic!
CI:
Ma'am Nelia is encouraging us to do our best.. She seems toxic too.. hehe I appreciates her attitude, understanding and easy to be with..

IV: Learnings:
I have learned that it is important to take care of ourselves against the Rabies that can give us by our pets. Also I gain more confidence to socialize to the people around us. Having this last duty of ours is a memorable memories.

Observation:
.Group-mates
> All were doing there best in doing the task that our clinical instructor given to them.

> She's doing her best to share her knowledge to her student.

.Self
> I'm quite nervous but also excited because i know there so much knowledge or skills i can get from my instructor.

Learning:
> I learned to communicate to others and to mingle with other people having different attitude and of course i learned how to improve my skills.

I.OBJECTIVE:
General
To be able to increase our knowledge and To be able to appreciate the things that are done within the CHO.

II.Specific:
To identify learning opportunities and to be able to increase knowledge as well as skill
observations When dealing with my groupmates,they seem to be a little wobbly,Although some of them are good.still there are lot of things for us to learn from our RLE as a student and a future nurse.
observations:

still they are doing good even though exhausted in doing some task.

b.C.I
she always assisted us in doing activity in the CHO,, she is respOnsible and approachable.

c.
all i can say that being a first timer in this field,maybe i need more practice to call me a competent one

General objectives:
To be more equipped health care provider by mastering or practicing the basic nursing procedure.

Specific objectives:
-to help the staff of CHO in giving outmost care for the patient.
-to apply my learnings and knowledge related to my profession.
-to gain more experience at the same time learnings.
-to render care and give teachings to my patient.

Observation:

Self- I am willing to learn more things about my profession. So, if there’s a chance to come I’ll grab it immediately because that’s one way of learning something specially I’m in the field of medical which needed a continuous education. This will be our last day in RHU/CHO, since I entered in this institution I already feel that I’m part of their team maybe not only me, same with my group mates and with this place I encountered a lot of people which having a different attitude and behavior. But because of that “mas lalo tumibay ang loob ko harapin ang mga susunod pang magiging pasyente ko”.i also improved my self confidence. With regards of my skills in basic nursing procedure, I say that I’m already improving and mastering the getting of vital signs. I have some also knowledge in other procedure like how circumcision done, how wound dressing done, how administered vaccine esp. anti-rabies and many more. I feel so honored to have a duty in RHU/CHO although it is more different from the hospital; it’s still part of a nursing field and still important for us to experience. But in all that, I still say that I need to learn and experience more in order for me to become an efficient and competent nurse someday.

Group mates- they are willing to experience and learn more things. Having an actual duty is their first time to experience so you would see them having a nervous thing in their face. But in spite of that they are still confident to do their task and to do it better as they can. Although some of them are still not so good in taking vital signs, they don’t give up instead of giving more patience in practicing it. I enjoy being with them, we have the same attitude and behavior in other such thing. We actually help each other when it comes of difficulties. I hope as time goes by; all of us become an efficient/effective nurse someday.

Clinical instructor- without her, were not able to make our self improving. Were so lucky to have a clinical instructor like her, she encouraging us to do our best in all things, she imparted knowledge to us which we need to learn, she always supporting us. Not only that, she also make us laugh/happy, in the middle of our difficulties.  So easy to be with! I’m so thankful to you mam for being part of our journey. Hope we meet you again in our next level! Love lots.

Learnings:
I’ve learned a lot of things such as, be patience in dealing with the different kinds of people, mastering the basic nursing procedure like taking vital signs, knowing the programs and rules and regulation of RHU/CHO at the same time the staffs and many more. A lot of knowledge and learnings related to my profession is all I’ve got for having a duty in that institution.

OBJECTIVES:
General:
To be able to know important how assess for own duty. And how to perform in others care.
Specific:
To be able to know the important about health.
To be able to give quality nursing care to those community people and attain good health
Observations:
Myself:
The last day of our duty I am also boring in my area at TREATMENT SECTION.
Groupmates:
I think they are not boring that day. But all are good follower.
Clinical Instructor:
In my C.I is a good leader to all of us and will to teach about health assessment. And give information’s about communicable disease.
Learning:
In the duration of our duty at the RHU I’ve learned a lot.

General Objectives: To be able to enhance my skills and knowledge with regards to be more alert in doing the task given to me.

Specific Objectives: To render health care to the community people/patient and enhance my learning in nursing profession.
>To be perform well the task for the last day in staying at CHO.

Observations: Though it was our last day in CHO, I did my duty well and made it as a memorable day. As I was assigned to he Treatment Area, I feel more confident in getting the vital signs to the patients due to the reasons that I was already master in doing it and improved what I did last time. About my group mates I enjoyed being with them because so much fun and they are jolly. Though some of them are not just good in getting the vital signs, they made it as a challenge to make their work better. And really they did their task focused to it. And with regards to my C.I, I enjoyed being with her because she enrich our learning experiences whenever we made some mistakes to our work.

Learnings: Since it was our last day, there are lots of fun and learning experiences dotted and planted in my mind which are essential in dealing with other people. I have learned to improved my skills and enrich my knowledge to be a good and responsible student nurse. I enjoyed how to communicate to the community people/patient.

Observations:
Self: My experiences are great to me. They added confidence to myself. I have found out that giving services to my fellowmen is one of the fulfillment of my dreams in life, so may God continue to blessed me to be successful in my chosen course.
Group mates: I felt self – fulfillment of having a group mates that are supportive and cooperative. They are all willing to learn, to gain more knowledge and accept their mistakes.
C.I: My appreciation each day is deepened of all the concerns she had given to us. She work like a pro, working with painted smiles on her face, and with encouragement of the things we do.
Learnings:
I experienced being criticized, so tired to and pro getting records, following commands, giving instructions and lectures to my patients are my additional learnings that I should handled properly and think that they are all duties and giving due services and I appreciated and welcome more learnings and experiences to be successful in my chosen course.

GENERAL OBJECTIVES:
To learn more things that we didn't yet encounter especially in terms of RHU settings, to be able to identify different kinds of procedures in giving care to our patients and i found it very hard, all i know is to get the VS of our patients and some knowledge regarding with it(health teachings). and to gain more experiences and the reality inside the medical area.

SPECIFIC OBJECTIVES:
to be able to apply all those knowledge that i brought in the three-day duty in RHU and the things that i learned inside the said medical area and keep in mind and heart because i chose this as a guide in my life to live.

OBSERVATIONS:

GROUP MATES:
I saw their hardships and dedications in this field,though we are still in our first step of nursing i can already see future nurses with them. Funny isn't it? We, the KALOGS suddenly became hard working students for this field and that what makes nursing difficult,just one mistake, little mistake, it can affect our client's health that's why i know why they,we became like this.

CI:
i can also see the determination of ma'am nelia to give the best of her for us to earn knowledge that we can use in our duties. Having four children is not that easy, being our CI and for the others is such a tremendous activity,but one thing i noticed was that, she can simply balance it, sometimes i wanted to ask her if this routine(house-school,school-house) stresses her.

SELF:
My gosh, during our last duty for this summer in RHU, I'm quite bored,huhu. we have only lesser number of patients in the treatment section, and its very boring! honestly speaking, i don't want it to be like that,it's not that i want more people to become ill,it's just that my whole day became "MATAMLAY",but on the other hand I'm glad that after lunch break we didn't do anything,hehe. (ma"am tinamad tuloy akong mag duty dahil dun,sori po.)But i didn't trashed my responsibility as a student nurse, when a lady asked me to take her BP because she feels dizzy, i did it.

LEARNING'S:
I learned to act more mature and give myself in this field truly, though the last day was quite boring, i also learned to smile to patients asking me to perform health care. and lastly' i learned some tips to give care more effectively.

General Objectives:
To have a relationship among the patients, staff, head nurses and clinical instructor in which learning is facilitated toward attainment.
to have a quality care to all patients.


Specific Objectives:

* To Further enhance our Knowledge and our skills in Nursing.
* To developed our confidence to cooperate with other people.
* To motivate us though a reward-system bases leadership style.
* To Promote an individualized primary care to attend the needs of patients.

Observations:

Well. Since my first day of Duty. My Co-mates help me to have a confidence to face any patients. even though i was little bit scared because i don't know what im going to do. But then, their always support me.

~ Clinical Instructor

She was so great and helpful and She always guide us. She always appreciate everything.

~ Self

Like i said a while a go, it was my first duty and that day i was feeling nervous. but i know that i will accomplish everything. so when my Clinical Instructor Assigned me at Admission. My other Co-mates said to me that it was a toxic area compare to others. when i was there in Admission Area. i was told to myself that i can do it and just enjoy and i was so glad because i made it! ;)

Learnings:

My First Duty and Last Duty at City Hall. I learned to be more open minded and confidence and to face everything challenges while working.

I. GENERAL OBJECTIVE:
-To be able to apply my knowledge and skills by doing one particular intervention for a given diseases, and by sharing our knowledge as a health care provider.

II. SPECIFIC OBJECTIVE:
-To render care and proper health assessment to the patient.

III. OBSERVATIONS:

GROUP-MATES:I have observed that everyone of us has its own weaknesses and strengths.Some have much knowledge in doing what is needed like taking vital signs and some is not that confident that they can do it. some of us lacks in the power of talking and explaining. But as a whole we are happy and looking forward to learning and helping more..

C.I- though i just came to duty the last day i still enjoyed the company of my group-mates especially my clinical instructor. she is very overwhelming,helpful and approachable. She always find ways on how to assist us and to share her golden knowledge to us nursing starters..

SELF- I'm very confident that i have given and shared my knowledge to the patients who needed my help.

IV. LEARNING:
-I have so much experiences in this duty. though late i enjoyed observing the kids having them circumcise. i also enjoyed talking to people and explaining them one particular question they've been asking. all in all a total of happiness and contentment and added with a spice of knowledge and skills thought by our most humored CLINICAL INSTRUCTOR.. love it..=)
