ACTIVITY NO. 1
ORIENTATION

I- GENERAL OBJECTIVE :

To be able to orient ourselves about the situation in our RHU duty and as well as familiarize ourselves with the rules and regulation that we should observed.

II- SPECIFIC OBJECTIVE

To be able to widen my knowledge and to practise all the learning that i have aquired during our lecture.

III- OBSERVATION

A- GROUPMATES
All of my groupmates showed cooperation and camaradery.
they have given their best during our first day RHU duty.

B. CLINICAL INSTRUCTOR
My clinical instructor has guided us properly. She always checked us with our performance and willingly give her advise when we needed one.

C. SELF
I was really nervous because it was our first day in RHU.
I have done my best and i am thankful for the given opportunity.

IV- LEARNINGS
I have learned a lot.
Not just on terms of additional knowledge but more on additional experience.
Experience in assisting the nurse while doing the POPSMEAR examination.

ACTIVITY NO. 2

I- GENERAL OBJECTIVE
To be able to learn and experience more in taking the vital sign.

II- SPECIFIC OBJECTIVE
To be able to apply my knowledge and practice taking the vital sign.

III- OBSERVATION

A. GROUPMATES
Some of my groupmates are afraid in taking the vital sign of walk-in clients.
however some are confident in taking the vital signs.

B. CLINICAL INSTRUCTOR
She is a very jolly person. She guided us properly and scold us subtly.

C. SELF
At first i am afraid to take the vital sign of our walk-in clients because i'm afraid that i might get the wrong output. But later that day i feel more confident.

IV- LEARNINGS
I've learned that practice makes perfect. That in taking vital sign i should not be afraid rather try to do all my best.

ORIENTATION

I-GENERAL OBJECTIVE:
To be able to apply skills and knowledge learned in our school.And to be able to render services and give care to individuals of any status.

II-SPECIFIC OBJECTIVE:
To assist the patient need like getting their body temperature and blood pressure as well.

III-OBSERVATION:

A.GROUPMATES
I observe that some of us are still nervous in getting our patient vital sign, but we are doing our best to make our performance greatly.

B. CLINICAL INSTRUCTOR
She is always their to monitor us to the task that assign to us and she help us to understand more things that were not yet familiar with.

C. SELF
At first, i was nervous but as we moved from one patient to another,I gained some confidence and i was able to improve my skills.

IV-LEARNINGS
On our first day of duty in the RHU, i learn to assist our client that is admitted to treatment area and do some ways of making the patient more comfortable by giving appropriate activities.

I-GENERAL OBJECTIVE:
To be able to show dedication and love in caring the elderly patient.

II-SPECIFIC OBJECTIVE:
To assist the medical staff to get the records of our patient accurately to enable the doctor to monitor patient exact condition.

III-OBSERVATION:

A- GROUPMATES
I observe to my groupmate that they have their will to learn.

B- CLINICAL INSTRUCTOR
She is always guide us whenever we dont know what we were going to do.

C- SELF
i observe to my self that as assigned to the admitting area, i was able to mingle with our patient.

IV-LEARNINGS
I learn to become flexible and alert to the patient needs and to comfort and also to give advise as to what problems regarding their health they are experiencing.

ACTIVITY I

I-GENERAL OBJECTIVE :

Being comfortable in taking vital signs and mingle to other people specially to the sex workers.

II-SPECIFIC OBJECTIVE :

Burden our knowledge and apply our skills.

III-OBSERVATION :

A.GROUP MATES

As first day of duty we are excited, but their are thoughts that came in our mind that what if we can't do our responsibilities right.

B.CLINICAL INSTRUCTOR

She's so nice and jolly,she help us in our duty and giving us moral support.

C.SELF

CHILL OUT!! I'm so excited in this duty because as the first rotation said that here in CHO have lots of patient and that's what I want,seeing lots of people in talking with them.

IV-LEARNING'S :

I've learned a lot and the ff.
are :

*proper taking of vital signs(temperature and Blood pressure)
*mingle with different kind of people.
*how dentist removing tooth that is aching.
*how pop-smear was done.

Showing our talent and knowledge in taking vitals.

To be a good care provider someday.

III-OBSERVATION :

A.GROUPMATES

Now as the second day of duty they are more comfortable to do their task and responsibilities.

B.CLINICAL INSTRUCTOR

She is very attentive in term of assisting others vital signs. She's easy to hang up with and very approachable when time comes that we really don't know what to do.

C.SELF

I'm little bit aware in doing our task in our second day of duty.

IV-LEARNING

I've learned were when is the first to hear in taking the BP .

ACTIVITY IADMISSION

GENERAL OBJECTIVES;
TO enhance our skills and adequate to our patients.

SPECIFIC OBJECTIVES;
TO be able to gain knowledge about rho programs and services.
To be able to get the right vital signs of our patients CORRECTLY.
To be able to mingle to our patients.

OBSERVATIONS;

GROUPMATES;
all of my group mates were afraid to be assigned at admission,because of the mistakes they might get especially in taking the blood pressure and they were thinking that patients will complains of the results.huhuhu

C.I.
she was always their to help and accompanied us .Also,she shared additional knowledge regarding our field.

SELF;
I am afraid in taking the vital signs of the patient but as time goes by my fear was gone.

LEARNINGS;
I'VE learned the services and programs of rho.ALSO,i applied what i have learned in our ncm100 (taking vital signs)I knew that most of the patients are children most of their illness occur during summer season.

activity 02(treatment section)

general objectives:
to know different kinds of procedure in this section,all i saw and observed and to be able to apply it in my duties soon. to know things i haven't seen and read before in actual my actual duty at rho.

specific objectives:
to assist the staff nurse in the procedures that she had in our patients during my stay in this section like the suture proper of wounds and taking off the moles.

observations:
group mates:
they were very cooperative though some we're really nervous to have mistakes they just did their best as a nursing students and apply the knowledge that they have that the school have thought.

CI
she never surrendered on us, though i observed that her mood changed and got dis appointed on us, but one thing i liked about ma'am nelia, she gave us lots of discussions and lectures about the things that we were really don't know or sometimes the lack of confidents that we showed, she made us feel that we can do everything, all we have to do is practice more and more.

self:
i gain more confident. staying at rho lessens my fear and shyness to comminicate and mingle to our patients and other health staff.

learnings:
i learned the procedures on how to remove moles especially when in the treatment room had a case like this and removed the mole of the patient in his nose, it was exciting. i learned how to assist properly,kahit na nangangapa-ngapa lang ako nung una, atleast parang nasanay na ako habang tumatagal, and made me an efficient nursing student.yehey!!!

ACTIVITY no.1

A.OBJECTIVES:

>GENERAL: To provide health care services to the patient. And share with them our skills and knowledge.

>SPECIFIC: To get the vital signs of the patient. And assist the nurse staff.


B.OBSERVATIONS:

>MYSELF: At 1st, i feel nervous and didn't know what to do. But i do well what ma'am assign for me.

>GROUPMATES: My groupmates are very cooperative. They do their best and they work good as well.

>C.I: Ma'am is very kind and funny. She guide us properly. and she help us if we did'nt know what to do.

C.LEARNINGS:

>It's our 1st duty in CHO, but i've learned a lot. Like how to assist the patient well. Learned to get the weight of the patient espicially the baby. And i learned also the proper taking of blood pressure.

some of my group mates are afraid to do their task but as time goes by they can handle their fear.

B. CLINICAL INSTRUCTOR

She always helping us, guiding us and specially teaching us the right thing to do in our duty. And we are so thankful to that ma'am.

C. SELF

it is my first time in RHU/CHO so I'm nervous but as we go along to our activities in our areas, i filled with confidence to do such task.

IV- LEARNINGS

I learned a lot of things and experience to this duty. specially, mastering the nursing procedure like taking the vital sign of the patient.

A. GROUPMATES

In our 2nd duty, some of my group mates are not doing their task because they always saying that they are afraid to do such task like taking vital sign.

B. CLINICAL INSTRUCTOR

she is very awesome. she always teach us the best and no one is being hurt when she gives opinion to each and everyone of us.

C. SELF

In my 2nd duty in RHU/CHO I'm more confident to do suck task, like taking the vital sign of the patient.

IV- LEARNINGS

I've learned a lot of things such as,be patience and be kind to the patients, and also I've learned how to work with higher class of person like the staff of the RHU/CHO and specially to the doctors at the DENTAL AREA.

I. General objective:

- To be able to orient ourselves on what are the thing that we need to do.
- To be able to know the rules and regulations in CHO.
- To be able apply the things that we've learned to school.

II. Specific Objective:

-to be able to assist the staff nurses in C.H.O.
- To take vital signs like blood pressure and body temp.
- To observe the different characteristics of sex workers.

III. Observation:

a. Groupmates- Some of my groupmates are nervous. others are very excited. They cooperate well and give their best in our tasks.

b. Clinical Instructor- Shes very supportive in at all time. she always help us to understand things that are not familiar with us.

c. Self- I felt very excited because I know that in our duty i will more and experience a lot of thing regarding health Care. I felt very happy because even in a simplest way I helped many people.


IV. Learnings - In our duty i leared a lot, I've learned the different programs of government regarding health. I learned how to deal with my patients. I also learned how take the vital signs properly. and I learned that being a student nurse is not that easy and you need give your best to do your tasks.

Activity II

I. General Objective:

To get the vital signs of our patient especially Blood pressure.

III. Observation:

a. Groupmates- In the 2nd day of our duty I observed that all of my groupmates are very comfortable. And their doing their tasks very well.

b. C.I - She is very nice instructor. and she's always there to correct whatever mistakes that we've done.

c. Self- I'm a little bit nervous but its not like the first day we entered the community.

IV. Learnings:
I learned how to give advice to our patients. I laerned how to interact with them in a nice way.
I also learned how to manage my time. and I learned how to be more comfortable everytime were in duty.

Activity I.

I. General Objective:

First day,orientation day, to be able to familiarize ourselves on what would probably happen during our community duty and for us to be aware to the different rules, regulations and policies that C.H.O imposing

II: Specific Objective:

To be able to widen my knowledge. and to gain further experience for me to become more competent.

III. Observation:

a. Groupmates- all my grooupmates are attentively listening during the orientation, and some of them felt nervous, and some are excited to start.

b. Clinical instructor- our instructor is very serious person. she explain things in simplest way that she could think of. She is very supportive. She so very good C.I. she's done very well for our first community duty.

c. Self- I am very confident during our orientation and i listen attentively to the CHO rules and regulation. i wanted to be aware of what is allowed and not. i do my best to give my very best.

IV: Learnings

I've leaned a lot, I've learned that our patient/client is our top priority.
I've learned how to be patient, how to interact with our patient.

Activity II.

I. General Objective:

To be acquainted with the proper conduct of our training as a student nurse.

II: Specific Objective:

To be able to do our tasks. to enhance our self in different activities during our community duty.

III. Observation

a. Groupmates- For our second day, my groupmates they are very attentive of what are C.I. informed us. they are friendly in every patient, CHO staff and Every people we encountered.

b. C.I.
To our C.I, She's done a good job explaining some details. She also made effort for us to be able to more familiarize with the CHO Vicinity.

c. Self- i think I've done a very good job for our second day. I'm quite irritated because I'm not knowledgeable enough to perform well.

IV. Learnings.
Be confident enough. be patient always, always take a lot of care of what we are doing. Give a very good service.

2ND ROTATION
FIRST DAY



I. GENERAL OBJECTIVE:


To be able to know more on about the rules and regulations of the CITY HEALTH OFFICE (CHO) and to practice more confidence in taking vital signs.


II. SPECIFIC OBJECTIVE:


To be able to apply the skills and the knowledge i know just like taking the vital signs.


III. OBSERVATION:


a. Groupmates

During the first day of our duty in the CHO, my groupmates showed that they are quit nervous, just like what i feel during that day, but as the time goes by on our duty, it seems that we overcome the nervousness we`ve felt.

b. Clinical Instructor:

Our clinical instructor was very nice to us in a way that she support us, and she`s always at our side and explain when there were things that was new to us.


c. Myself:

Quit nervous just like some of my groupmates because it was my first time to have duty in the CHO, unlike in our first rotation in the hospital we encounter different environment but we enjoy a lot and we learned a lot from it, especially when i was one of the nursing student who assist the 2 staff to do the popsmear exam. in some ladies came in the CHO.


IV. LEARNINGS

Me and my other groupmates where assigned by our C.I. in the administraton area, wherein we assist some of the staff of CHO to admit the patient, we get vital signs and communicate some of the patient as well as observed the CHO setting.

2ND ROTATION
SECOND DAY


I. GENERAL OBJECTIVE:

To be able to practice more in getting vital signs and observed, to have more knowledge in working in the CHO.


II. SPECIFIC OBJECTIVE:

To be able to do my job correctly and with confident.


III. OBSERVATION:


a. Groupmates:

They`re more confident this time and
i`ve obserbed that they are enjoying in what they are doing.


b. Clinical instructor:

Very supportive and kind, she observed and watch us time by time and ask us if we`re able to do our job assigned.


c. Myself:


I was assigned in the doctor`s section this time and i enjoy what i was doing this day, i felt more confident in communicating to the patient and tries to miggle with them.


IV. LEARNINGS:

More confident!

I learned a lot and i enjoy what im doing this day!

ORIENTATION

I-GENERAL OBJECTIVE

-To acquire health information,to enhance our knowledge in Nursing Practice and to be able to promote health most especially in the Community Setting.

II-SPECIFIC OBJECTIVE

-To be able to appreciate community Health Nursing and its essential and to be able to know new diseases,and practice Nursing skills such as taking vital signs and assessing patients for their check up and other purposes.

III-OBSERVATION

A-GROUPMATES

They are very cooperated to each other,they give their best and knowledge and most of all they are very happy despite of the Toxic Duty that we encounter in the RHU.

B-CLINICAL INSTRUCTOR

She is very Humble,and she always guide and support us in our duty.she always share her knowledge about the things that we don't know in our duty and she is a good adviser.

C-MYSELF

I feel nervous when I took the BLood Pressuer of my patient because it is my first time to have a duty in the City Hall,but most of all I give my best and to do my duty become good.

IV-LEARNINGS

I've proven to myself that "Experience is the Best Teacher"because you will learned a lot of it and it teaches you to become a responsible student.
And you should know how to socialized with your patient.

I-GENERAL OBJECTIVE

-To be able to render Nursing Care to our patient especially in the Community Setting.

II-SPECIFIC OBJECTIVE

-To perform Health Teachings and perform some assessment regarding our patient illness and discuss topics regarding the activities that we are going to perform.

III-OBSERVATION

A-GROUPMATES
I observed the some of them are afraid to get the vital signs of the patient may be because they are nervous,but despite of that they are willing to try their best to face their nervousness for the sake of their patient and to their self.

B-CLINICAL INSTRUCTOR
-She is very professional and responsible in her job,because she give her best in teaching us about the things that we didn't know in our duty.

C-MYSELF
During our 2nd Duty in the RHU-I feel more excited to myself because I know that the things that I'm going to do like taking vital signs,etc.are one way to become myself more Competent in my chosen profession.

IV-LEARNING'S
I learned that you should "Learn from you're mistake" so that you will become a better person and you should believe in yourself and give your best in whatever task that you will give them to you because it is the one way to reach your goal in your chosen profession or in to your life.

GENERAL OBJECTIVE:
To be able to Familiarize and be oriented and be able to learn the different rules and regulations at City health office.And also to know their main focus as a community health worker.and also as a Nursing student.
SPECIFIC OBJECTIVE:
be able to learn and apply what i have learned during the duty hours.
OBSERVATIONS:
A.GROUPMATES
My group mates was given a task some where assigned at treatment area,others where at admission,and others at the dental and laboratory area.but i know eveyone felt a little nervous.
B.CLINICAL INSTRUCTOR
She guided us so well specially me coz Im so nervous when Im taking up vital signs and she teaches me
how to take it correctly.
C.SELF
I felt a little anxiety at first but after taking up some vital signs at some patients my fear disappeared. and I'd gained self confidence.
D.LEARNINGS
I've learned a lot. now I can take up vital signs with my whole self confidence Because I can now clearly hear the heart sound. when taking the blood pressure.and I'm so happy because i can apply what i have learned during the Duty.

As i observed to them they are now more confident to do their part as well that they are doing great on this activity.

She is always being a great leader to us in order to make us more compatible.

As i asign again at the doctors office,i feel better confidence in my self.

I- GENERAL OBJECTIVE:
To be able to help the CHO staff in terms of different health services.

II-SPECIFIC OBJECTIVE:
To be able to assess the patient health status by knowing their situation and illnesses that they are suffering and how long it has been started.

III-OBSERVATION:

A- GROUPMATES
As we go on they also learn to become alert and work hard to the things assigned to them.

B- CI
She is slightly strict to us but it is her way to disciplined us.

C- SELF
I felt nervous on how to assess the patient health condition maybe because it is my first time to have this kind of task.

IV- LEARNINGS
On the 4th day of our duty i learned on how to assist the doctor and nurses in the circumcision among the children.

I. GENERAL OBJECTIVES:
To get the health status of the patient.

II. SPECIFIC OBJECTIVES:
To help the patient by admitting and by taking vital signs.

III. OBSERVATION:

A. GROUPMATES
I observed to them that every patient they're assisting, they are now more alert on it like taking their blood pressure.

B. CLNICAL INSTRUCTOR
She is always being jolly,but then in terms of our activity she is always guide us on what to do.

C. SELF
i was excited because we had assist more cases in this activity.

IV. LEARNINGS:

I was successfully great feeling because we gain and we treasure more experiences like assisting those patient to admit what illness they have, also in assisting circumsition and in mass removal operation.

I. General objective:

- To be able to help the stuff nurses in Community health Office regarding health.

II. Specific Objective:

- To be able to assist the stuff nurses of CHO espcially in getting the vital signs of the patient.
-To be able to observe the procedures on how the Pop-smear was done.

III.Observation:
a. Groupmates
My groupmates showed excitement when our C.I. give us another chance to observe pop-smear.

b.C.I
In terms of getting vital signs she's to srict but when our lunch break come she's very jolly thats why we enjoy being with her.

c. Self
Like my groupmates i was also very excited because in the last 2 days of our duty i know that we will learn andexperience a lot.

IV. Learnings:
In the third day of our duty my knowledge in getting vital signs was improved. I also learn to give more advice regarding health espcially to those who are member of sweet club in CHO. I also experience observing how pop-smear was done. and I also learned how to clean open wound because one of the stuff nurse of CHO entrusted me to clean open wound of their patient.

Activity IV.

I. General Objective:
To be able to practice more in getting vital signs.

II. Specific Objective:
To be able to observe and assist the doctor in doing circumcision of a children.

III. Observation:
a.GROUPMATES:
In the last day of our duty in CHO my groupmates was very excited. Because our group gained a lot of experience and learnings.

b. C.I
She always guide us in all thing we've done in our duty. She's always there to give us knowledge whenever we encounterd unfamiliarize things.
She's also always there to explain things before we do it. And I learned a lot when she was our C.I.

C. Self
I was very excited that day because of another experience and learnings will be done.

IV. Learnings.
Having a duty in CHO was one of the unforgettable duty we had. We learned a lot,like how to get proper vital signs,assisting the stuff nurse whatever they do, dealing with different kind of poeple espcially to those who are sex workers. we also learned how the procedure of circuncision was done. And also a little bit knowledgein removing mass.
