OBJECTIVES:
٭general: to enhance more our knowledge in the male ward.
٭specific: to care for a patient with surgical case.

OBSERVATIONS:
٭self: 'though it's not my first time in the area I still got confuse in routine and I still got nervous. It's my first time to render care for a pt., in the surgery ward.
٭group mates: At 1st we 're not so close with each other but later on when we have adjusted ourselves with each other we're getting closer with each other. Also to our new groupmates/ classmates although they are still in the adjustment period I've seen they are also cooperative and responsible in what they are doing.
٭clinical instructor: Ma'am Nhelz thanks for all the knowledge and understanding you've given to us even sometimes you got angry to us because of our because of our attitudes we still give our thenks to you because we know that it is also for our own good.

LEARNINGS:
٭This week I've learned a lot, I've to be more responsible in doing procedures to our patients, and to be careful enough in what we are doing. I've learned to be more patient enough in caring my patient.

OBJECTIVES:
GENERAL- to enhance my skills and apply my knowledge in rendering nursing care to the patients at male ward.

SPECIFIC- To render proper care and give emotional support not only to the patients but also to their family.

OBSERVATIONS:
self- ive done my best to render nursing care to the patients and i have enhanced my skills in doing procedure like wound dressing.

groupmates- new group thats why new adjustments, but i guess we already surpass the adjustment period because weve known each other before except for the the new faces in our block who trasferred from other schools, i know that they were doing their best to adjust to their new environment.

clinical instructor- shes very strict in the area, but i know that it is her way to discipline us so that we can improve ourselves better.

LEARNINGS:
I've learned how to do proper wound dressing, eventhough its not my first time at the male ward i have done some adjustment so that i could enhance my ability, knowledge and so that i could perform my duty as a student nurse so that i could perform my skills well and enhance it at the same time.

Objectives:
General : To conduct our direct duty at male ward for this summer. To gain knowledge about orthopedic and psychiatric nursing.

Specific: To apply our previous knowledge and skills regarding nursing measures and procedures that are related and needed by our patient according to his/her case.

Observations:
Self: During our first day of duty, it was really another adjustment for the three weeks vacation. But I tried my best in doing my responsibilities in caring my patient and made my work well and appropriately.

Group mates: We're all at the adjustment stage specially our two new group mates yet they did their best in rendering care to their patient.We did our responsibilities as a student nurse even though we had mistakes, we learned.

Clinical Instructor: Still Ma'am is very responsible and very concern with us as student. She explained to us once again the importance of becoming a nurse someday. Though, we had still many mistakes, she stayed with us and gave advises and challenged us to improve our skills.

Learnings: I've learned again the process of handling patients in the male ward, the every day routine, having time management, making soapies, computing medications, and being patient in doing our job. I've learned to wake up early and preparing the complete paraphernalia's for the duty.

I. Objectives:

General: Provide nursing care to patients in Quirino Provincial Hospital, make ward.

Specific: Improve skills and broaden our knowledge which will help us in the near future.


II. Observations:

Self: I am a bit careless the fast days of our rotation.. Maybe because I wasn't that prepared for the day's duty.

Groupmates: A lot of improvements were vissible during the first week.. I am much pleased with the performance of Miss Farinas, despite the fact that she's new in the group, she showed to all of us that she herself had a place and that she is a good and a reliable member of the group, too.

Clinical Instructor: You were right when you scolded me that day, ma'am.. And for that reason, I am very humbled to tell you that I'm beginning to idolized you.

III. Learnings: Despite the status of a client, a health care provider should always respect his/her client and give all the necessary support to hasten the health improvement of a certain individual.

Objectives:
General_To gain more experience.
Specific_To care our client.
To expose in surgical ward.

Observations:
Self_I cooperated to my client and i gave my best support in rendering care,also to my groupmates,C.I,and to the staff.

Groupmates_We shared ideas and opinions to each other.And we helped each other sometimes.

C.I._Maam Perez is very nice.She always support us in giving meds.And sometimes she scolded us but its okey.

Learnings:
I performed a wound dressing in ward.And always putting my self on their situations.

I.OBJECTIVES
a. General Objective
> to improve more my skills in giving services to medical and surgical patient
b. Specific Objective
> to make an accurate assessment and interventions with assigned patient

II. OBSERVATIONS
a. Self
> during that week of duty especially the first day of duty I feel unprepared to have the duty because I forgot some of the routines and my mind was still on vacation but thanks God because he gavew me a patient that is not toxic.

b. Groupmates
> Like me, some of my groupmate's minds were still on vacation because we forgot to put our vital signs taken in TPR sheet. Our two new groupmates are still adjusting to the routine.

c. Clinical Instructor
> Our C.I. ma'am Nhelia is doing her best to shape us to become effective nurses someday by disciplining and teaching us what we need to have.

III. LEARNINGS
> My patient that three consecutive days of duty was a V/A and suffering from hypertension and diabetes. During the first time I handled him, he refused IV insertion but I tried my best and I convinced him because of his IV medicines. I assisted ma'am Nhelia in inserting the IV line and lastly, I learned to fill-up X-ray request form.

-To learn and to improve our skills and ability as a Student Nurse in the Hospital Settings. Specially on Male Ward.

>Specific Objectives
-To formulate different techniques in caring of patient's in such way that may improve their health.

Observations:
>Self- I, myself , I learned a lot of things, mostly in charting, hpe to learn more.

>Groupmates- Everyone done their part as a Student Nurse.

>C.I-Ma'am Nelia is very supportive to us, she's a very responsible clinical Instructor.

Learnings-
For my skills, I just hope that it will improve more in Male Ward as it is my second time here, i familiarized a lot of things and procedures, but not all so i jusr hope that i learn more from ma'am Nelia

OBSERVATION:
>self:on my part although it is not my first time assigning at the male ward,i expect that i do the better work,but some of the routine i've lost.
>group mates:all my group mates are good in caring thier own patien.but the two person newly we meet are not yet aware to the routine of the hospital so that our clinical instructor oriented them.
>clinical instructor:thank you ma'am for having fan in our group,you encourage all in continue learned the routine and policy of the hospital,thank you ma'am for mparting your knowledge with love and hapiness.\

LEARNINGS:
>at my 2nd time assigning at the male ward i learned a lot,in caring the pt.and learned also the new term we encountered,thank you ma'am for the knowlede and encouraging all to the good work like a proffetional nurses
