-one day miss Somera was ask to give a medicine to a patient and the doctor ordered a cocaine instead of procaine and she did not question it and the patient died, because of following the order she was sentenced in prison and fined a certain amount.

- the mistake that is done by the doctor was when he/she gave a wrong medicine to a patient and the mistake that is done by miss Somera was when he follows the order of the doctor and did not question about it.

-if I were in the place of miss Somera, I will do the right thing and i will not follow the order of the doctor because the responsibility of a nurse is to promote health care not to hurt the patient.

-> upon reading and understanding the case i can say that the persons involved were the CHIEF NURSE LORENZA SOMERA who was the one being accused and the OPERATING SURGEON who had asked for the medicine which was to be administered to the patient.

-> lorenza somera was the chief nurse then when she have assisted in a operation which is tonsillectomy, where she was asked by the operating surgeon to prepare 10% of cocaine with adrenalin which is to be administered to the patient under operation,after the said medicine was given to the patient the initial reaction of the patient to the medicine given was terrible the patient had convulsion and after sometime the patient died unexpectedly. Only Lorenza Somera was found guilty for the case of manslaughter due to negligence the doctor who have asked the medicine was set free.

-> she should have asked the doctor again if she is not sure with it,if the medicine the doctor have said was correct because if you are not sure you should asked first. There is nothing wrong with asking.(but there were notes in the net where in it was stated there that Lorenza Somera asked and verified the medicine to the doctor befor administering it...hmmm i just think there is something much deeper with the said case.)
4. If you were in the place of Miss Somera, how would you act in the situation @ hand
-> if i where in Lorenza Somera's shoes at that certain moment i will asked the doctors order again it is life that is in my hand not a toy, nurses should be very specific with medicines most especially one droplet of med can either make the person or the patient live or die, be particular with your actions especially in the hospital a simple mistake can cause a huge damage.
5. If faced with the same situation, what would you do in order to defend yourself?
-> for me if the doctor still insists me to administer the medicine which i know in myself is not correct ill just asked him to give the med by himself so that the blame would not be in my shoulders anymore so watever happens with the patient the blame will not be on my shoulders.and after his action let him note that he administered it. :)

- in the somera case although the physician was negligent, the cause of the death was the nurse's negligence. For me its hard to defend because we know that there was the negligence of the person involved but it is the physician who gave order and nurse are following so to defend I'LL just go to a attorney that knows the nurse profession that will include the ethics of a nurse and include investigation.

A surgeon is about to perform tonsillectomy. Being assisted by Miss Lorenza Somera, head nurse,the surgeon asked the her to prepare and administer 10% of cocaine with adrenalin. Miss Somera repeatedly asked the surgeon what drugs are to be administered but still the surgeon kept on ordering cocaine instead of procaine. Upon administration of the drug, a few minutes later, the patient started to have convulsions which resulted to death.

On the surgeon's part, he might as well mispronounced the name of the drug and did not paid too much attention on the nurse while she repeatedly asked about the drug to be administered.
On the nurse's part, she should have been responsible and consistent enough with regards to her training and experince that the drug being ordered is not the right one.

Being a skilled and well-trained nurse, it is my responsibility to take care and promote life to mankind. Being such in a situation where the life of a person is at risk because of a preventive medical negligence I will take the risk. I'd rather be fired because of not following a wrong order than being haunted by my conscience by letting a person be killed. I have to take my part in any situation and fight for what I believe is true and must.
5. If faced with the same situation, what would you do in order to defend yourself?
Being in the same ordeal I might as well present evidences that it was not only me who irresponsibly committed a mistake but also the surgeon. My silence have brought me in to prison and I believe that it's also just humane to speak out what really happened.

The persons involved in the case were the Surgeon and the attending nurse named Lorenza Somera who assisted the operation.
2. In your own word, relate what happened to the case?
The case was simply to perform "tonsilectomy" by a Surgeon, assisted by Lorenza Somera. The Surgeon prescribed cocaine and the nurse injected the said drug and finally the patient died.

1.The Surgeon prescribed a wrong drug unintentionally since i believed that the surgeon is knowlegeable about pharmacology.
2. The Nurse who injected the wrong prescribed drug and failed to make any further referral to the surgeon or failed to verify, clarify the prescribed drug. or review any reference or the literature enclosed in the drug packing.
4. If you were in place of Lozana Somera, how would you act in the situation at hand?
I will always first confirm the drug to the surgeon prior to administration. In casae i was already injected the wrong drug and the patient diet, i will not blame anybody or the surgeon. It is better for me to accept my mistakes since the situation is the result of my carelessness.
5. If ever you face the same situation, what would you do to defend yourself?
I will not defend myself but rather i will accept the case as my mistake. I will ask humble apology to .the family of the patien. In this way , their anger might be lighten and probably they will forgive me .

2. One day, Mr. Pedro Clemente brought her daughter Anastacia Clemente to Dr. Gregorio Favis for consultation. Meanwhile, Dr. Favis have ordered his patient to go at St. Paul's Hospital on May 26, 1929, 7am for HE(the doctor) were going to do an operation (TONSILLECTOMY). Upon arrival in the hospital, Dr. Favis together with Dr. Bartholome(assistant surgeon) performed to operation. At that time Lorenza Somera, the Head Nurse, assisted the operation. I evenly accept that I'd prepare the drug but I'm just following the order of the doctor for if I didn't do so the patient may die.... If I'd face the same situation like with Somera, I would evenly tell everything in the court that Dr. Favis have evenly ordered the certain drug to be prepared with out my based knowledge about what operation it would suits to be used for. At that time, a nursing student named Consolacion Montinola was working in the operating room under Somera and was assigned to be the sterilizing nurse. Before the operation, Dr. Favis asked for a 10 per cent cocaine solution with adrenalin. When the operation got started, Dr. Favis asked his assistant Dr. Bartolome, for the novocaine solution. Montinola, the student nurse handed to Dr. Bartolome the syringe filled with solution, Dr. Favis injected it to Clemente. There were two injections done, and after the second injection, Dr. Bartolome noticed that the patient became pale and acted as if she was dizzy and so he called up Dr. Favis' attention. Dr. Favis said that it was not unusual so he asked for the third syringe of the solution which showed symptoms of convulsions on the patient after injection so Dr. Bartolome called Dr. Favis' attention again. After that Dr. Favis ordered adrenalin and injected it to the patient. The patient was given 2 shots and still showed signs of convulsions and after few minutes, the patient died. It was then that Dr. Favis asked if the novocaine solution prepared was fresh and Somera replied that it was not novocaine but a 10 per cent solution of cocaine.
Montinola, being the witness was questioned by the prosecution and affirmed that she did not know who prepared the drug and that she heard Dr. Favis ordered cocaine with adrenalin for injection, and that, Somera verified the order.
Dr. Favis and Dr. Bartolome were absolved of the crime and declared Somera guilty of the crime charged for homicide through reckless imprudence.
3. One of the errors i have noticed committed by the persons involved in the case was the order of Dr. Favis for cocaine instead of novocaine to be used in the operation. Another error was the negligence of the nurse Somera to check the drug which in the first place must be sensitive enough on the drugs ordered by the physician, like the cocaine which was supposed to be novocaine. It must be on her discretion that upon hearing the drug 'cocaine' to be administered, she should have asked the doctor and made sure if the ordered drug since it is very evident that such drug could endanger the patient.
4. My initial reaction would be, of course not to put the blame on my self but on the doctor who ordered the said drug. In the lorenza’s Somera case, Dr. Favis ordered a cocaine with adrenalin to Lorenza Somera, the head nurse, for the tonsilectomy patient. After administering the medicine, Dr. Bartolome noticed that the patient becoming pale, they were all alarmed why does it happened. Ms. Montenola verified the medicine and she learned that the medicine given was wrong. The patient died because of the wrong medicine that was administered. - Lorenza somera was directed by the operating surgeon to prepare 10% cocaine with adrenalin for administration to the patient 4 tonsillectomy. Ms. Somera repeated and verified the ordered. A few moment after the injection was given the patient showed symptoms of convulsion and the patient died. The operating surgeon meant to say was found guilty of manslaughter due to negligence. The negligence consisted of fulfilling an order that 3 nurses should have know by reason of her training and experiences was an incorrect one. Although the physician was negligent, the cause of death was the nurse negligence.

The persons in the case are: Lorenza Somera- the head nurse: Anastacia clemente, the patient who was died.

Loenza Somera is a head nurse she was accused of homicide throught reckless imprudence and was condemned in may 26, 1929 in manila to one year imprisonment in connection with the death of young girl name Anastasia Clemente in the operating room.
Lorenza was found guilty because when the doctor said cocaine instead of procaine, she did not question his orders.

Assisting Lorenza Somera, a head nurse, were student nurses Valentina Andaya and consolations Montinola. The assistance surgeon was Dr. Bartolome. During the operation, Dr. Favis asked Dr. Bartolome for Novocain solution. Ms. Montinola handed Dr. Bartolome a syringe of solution which was handed in turn to Dr. Favis who injected the same to the patient. After a few minutes Dr. Baartolome noticed that the patient was becoming pale and acting as if dying. He called the attention of Dr. Favis to this but the latter said it was not unusual. A third syringe of solution was injected and a few minutes late, the patient died in a few minutes. Dr. asked if the Novocain was fresh. Ms. Somera replied hat the solution was not Novocain but 10% cocaine.

2.A surgeon is about to prepare an operation of Tonsillectomy,Upon the operation,The doctor ordered the attending nurse Miss Somera(head nurse) to administer a 10%coccaine with adrenalin,miss Somera asked the surgeon for the second time for the name of the drug but he insisted and keeps on repeating the name coccaine instead of procaine which she knew will cause bad to the patients health,after few minutes of administrating the drug the patient had convulsion and eventually died.
3.The errors comitted by them are firstly its the surgeons fault of not clarifying and verifying his orders and not paying attention to what he is giving,secondly to the nurse who has knowledge about the the drug's effect to the patient but did not raised question about the drug which has harmful effect to the patient.
4.If i where in miss someras place as the attending nurse i would not have second thoughts on raising questions about the drug because i know the drug will have bad effect on the patient.
5.I as nurses we believe that we are just a doctors "alalay" in what so ever aspect inside the hospital, obeying their rules and orders they are giving that's what we do but i also believe that we as medical proffesionals,our role is to save and prolonge lives, i will not follow the doctors order and tell them what i really know about the said drug because big things came from small ones, and a wrong action might heal or kill a persons life.
