Fourth Year B LFD (2nd and final week)

Thanks for the "not-so-toxic" 2 weeks duty guys... You made my last rotation worth remembering.... OF the four groups that I handled in your block, i enjoyed your group the most!!!

"2ND WEEK"

GENERAL OBJECTIVE:
> To deliver my skills in Good Quality care necessary to my pt.
> To enriche my knowledge, to become effective nurse as well credible person in the industry of nursing feild.
> To imposed good attitude toward to my CI, groupmates, staff, especially in my patients, just to act well manner and never doubt on my effectiveness.

SPECIFIC OBJECTIVE:
> Make the most out of it,
> Observe ideal technique must be apply to a procedure on my patients, so will never mess up on rationalizing on prior on exam for board.
> To evaluate my self if Iam ready to face the challenge as a full pledge nurse someday.
> Establishing good relationship to my groupmate, CI, and friend to come, as they face fast approaching graduation, as they bring a bacon for our school and to there families later on.

OBSERVATION:

TO MYSELF:
> Its a such whoa feeling or you never might think we are getting closer to the end of our college year, as we approach for seriously stumble strive for our live to work hard for ourself and to pay vice versa to our parents who show passion sopporting to us sending in school to become nurses, i admit to myself that im being greatfull what is being given to me and feel contented, in my age its huge oppurtunity to come back in school and to work hard my ass, even sometime im being so slow in absorbing knowledge due to my capability, but still im burnout myself just to evolve person as our CI or parents wanted us to be.
> one time i ask myself that why i should be to this or that, producing toxic on my self on what im doing and feel bad after all, but later on,i rationalize or reflect that it will pay off that afterward and will give a good reward in to my skills and give good effect on my self-being as a nurse someday.

TO MY GROUPMATE:

> My groupmate have different walk of life and have good differences, but they have the capacity and capable to be to do such a thing that will unite us in giving quality care to our patients that will leave a good mark to those who touch there lives.

TO MY CI:
> You will not repent and surely she give the best in her, as they want for us to have flying colors.
> even sometime we are toxic in what procedure we may do, she always there to make you smile, not only to smile but to laugh.
> after all its very rewarding that shes always make a point that she will surely give sufficient learning to our empty coconut shell and bring out the best in us and manage any circumstances that may arises.


LEARNING:
> I learned this week , if you want to be a nurse locally or internationally we student should embibed the KSA( knowledge, skills, attitude)just to prove ourselve that we are right the one to be nurse that have COMPETENCE, COMMITMENT and COMPASSION in giving care.

GENERAL OBJECTIVES:
> To rendered or to provide proper health to the patient
>To gain more knowledge,skills and exposure at the hospital

SPECIFIC OBJECTIVE:
>TO observe the high risk of the disease
>To observe the complication of the disease

OBSERVATION:
SELF:
> "It is hard to do work if you are not ready in doing it"

GROUP MATES:
> I find enjoyment having with them, they are cooperative, friendly and supportive

RESPECTIVE CI:
>To our respective CI,you are knowledgeable person,approachable,and very understanding CI

LEARNINGS:
>On the day of duty at QPH, I felt really great.Its our first time to be assigned in the pedia ward.Our CI oriented us the rule and policies. She also reminded us what will be the requirement to be passed.After that,She group us into four cluster in every 2 cluster their is a two supervisor then I supervised my co-members.On the next day,I was assigned as a procedure nurse and i handled the taking of vital sign and regulated the IVF.On the last day,I am now the medication nurse.I gave the proper medicine to our patient

I. OBJECTIVES
> To develop and enhance more my skills and abilities.
> To experience the actual duty of a Pedia Nurse.
> To know how to render service and care for a pediatric client.

II. LEARNINGS/OBSERVATIONS:

This was during my first week!!!!We start our first day with orientation at Pediatric Station.And just after the orientation given to us, we proceed and start on our duty. I was assigned as the head nurse of one cluster. To be a leader is a great challenge for me. I learned how to lead my group. So far, I did noy commit errors on my charting.
Day 2
>I was assigned as a Procedure Nurse. Likewise, I just followed the routine procedures, taking vital signs at 6pm and 10pm and regulating IV fluids. But then, I have 1 patient monitored his vital signs every 2 hours.And this day, I have no special procedures made.
Day 3
>I was the Medicine Nurse where I computed IV meds for a right dose to be given to my patient. This day, I learned that chloramphenicol is painful drug when it is administered intravenously, that's why I have been instructed by our Instructor to aspirate first and push it very slowly. This was enculcated to my mind so that every time I encountered this drug, I know what I'm going to do.
----QPH Pediatric station has 4 wards-1 for with medicare patient, non med pt, isolation and NICU. Cleanliness and orderliness are being observed and many colorful posters on the wall that it really helps the condition of the pt.

>ON THE SECOND WEEK-I learned how to supervise the 2 clusters that i have handled and I got no demerits from them. On my second day, I was assigned again as the head nurse of one cluster and i learned again to lead and manage my group and my knowledge in doing the SOAPIE was enhanced. I did well my task that's why I have no demerits.On the last day of our duty, I was the procedure nurse and I felt so toxic because I handled patient whose vital signs should be taken every 2 hours and her temperature be monitored every 30 minutes because the patient experienced seizure. I almost spent my time rendering TSB on my patient. Because it was our last day, we conducted a ward class at pedia medical ward about "rabies". we delivered the message well and the S.O. of our patients participated in the said lecture. We gave toys to pedia patients as a sign of our thanksgiving and as a remembrance from us. After we accomplished all our tasks, we had our evaluation exam. After that, we did rationalization of the answers and I got new informations.

III. INSIGHTS AND IMPRESSIONS:
To myself:
I observed that I have a better improvement on myself in doing my part. I did my task well and so far I have no personal demerits.
To my groupmates:
>They are fun, cooperative, and nice guys. But some of my groupmates are not careful in plotting vital signs which i got 10demerits from they did.
To my CI:
>Since even before,I felt nervous to under her supervision, its because I met her in first place as terror. But when the day comes to be with her again, All I thoughts found wrong. To be with her, you feel comfortable and better. Because she is a very knowledgeable and responsible in guiding us well. She is also nice and with sense of humor that made the group alive all the time. Thanks Ma'am!I salute you and made me proud of you.

" 2nd Week/ Last Duty "
I. Objectives:

General Objectives
> To be able to provide high- quality patient care.
> To be able to know and understand the functions and responsibilities of a Head Nurse, Medication Nurse, Procedure Nurse, and a Supervisor.

Specific Objectives
> To be able to apply what I have learned in Nursing Leadership and Management in the clinical setting.
> To be able to render care to my patients empathetically, properly, and correctly.

II. Observations:
During this last week of our duty, I observed that my skills was improved because I do not have any demerits. I also observed that I need to be meticulous in rendering care to my patients. Lastly, I observed that as a nurse, we need to provide the total care, not only doing some procedures and giving medications to our patients but to sympathize and empathize to our patients.

III. Insights and Impressions
A. To myself
As a student nurse, I tried my best to do my duty and responsibility to my patients, groupmates and to my CI.

B. To my groupmates
My groupmates were so fun to be with, they do their responsibility as a student nurse to their patients.

C. To my Clinical Instructor
I'm so thankful to be her student because she's so responsible, intelligent, kind, considerate, and affable. I learned a lot from her, my knowledge and skills was enhanced, and also my attitudes towards my patients and groupmates.
Actually, this was my first time to be her student, and I felt so nervous and afraid whenever I talked to her because I remember when I was a 3rd year student, during our practice for our capping and pinning when she called my attention and scolded me for what I did, until before my duty to her, I still felt so nervous. But now, as I go along with her, my perception about her was changed because she shared/ imparted all her knowledge to me and my groupmates of what we need and she was so fun to be with her.

IV. Learnings:
I learned how to be a Head Nurse, Medication Nurse, and Procedure Nurse to my patients and a Supervisor to my groupmates.
I also learned that a student nurse should empathize to their patients and to be alert and systematic during their provision of care.
Lastly, I learned that a nurse should not only have the knowledge and skills but most importantly the attitude because patients need the total care(TLC).

2nd Week

GENERAL OBJECTIVE:

OBSERVATION
>On the last week of our duty i observed that almost/all of us are very much happy bec. its the last week of our duty. Many of us shared much their skills and knowledge to care for our pt. some of the S.O. are give their comment to us these are the weaknessess that we neeed to improve more through our duty when we become a professional nurse someday.

>SELF
>I,m so very much happy because i tried again to become a head nurse in our group. At first its hard but enjoyfull and pleasurable. During the last week of our duty I assigned as the supervisor and i'm very much glad as a supervisor I feel like the chief nurse, I'm the who is responsilble to my group and I observed to my self that I can stand now with my own self, but then we cannot deny the fact that there are still some doubt in doing such procedure specially toxic pt.

>GROUPMATES
>To my groupmates I observed that all ogf us are ready to work in hospital without the supervision of our c.i. I observed that even though withiut aur c.i. we can such procedure with good determination. I hope that those things we in the ward will be the same when we work to other hospital and this will be improve more.

> C.I.
>I think our c.i. has akready the confident or trust in our capabilities bec. she does't assist us in administering drugs to our pt.Thank You Guiding Us Ma'am

>LEARNING
>I learned to depend on my own bec. of the experience that I encounter in our duty.I experienced the things that I havent experienced before.

"2ND WEEK"

GENERAL OBJECTIVE:
>To be able to gain more knowlwdged and apply what we have learned from the school.

SPECIFIC OBJECTION:
>To be able to enhance ourselves on how to deal with pediatric pt.
>To be able to practice ourselves on the responsibilities in a Pedia Ward.

OBSERVATIONS:
>It was my first time to have a duty on a Pedia Ward and I had observed that the STAFF there are very patient to do their own task and deal with their pt.
On a Pedia Ward,you must gain the trust of the S.O for you to gather data and perfome different procedure excellently.

INSIGHTS AND IMPRESSION:

>TO MYSELF: I know I have done my best to do my own responsibility and cooperate with my own cluster.

>TO MY GROUPMATES:
Thank you for the cooperation. Just always fight your fears for you to perform hospital procedures correctly and be alert always and have more patience.

>TO MY C.I.
Thank you for sharing us your knowledge. You are very responsible and intelegent nurse and instructor as wel. I know you are strict in demerits, but its fine with us because I know you are just doing it for us to improve our work.

LEARNINGS:
>I have learned many things on my rotation on Pedua Ward this week, like how to deal with the pt. correctly and gaining trustly relationship with the S.O. I have also learned that a nurse knowledgable, because you won't perform different procedure excellently if you have no knowledge on it. Be carefull on computing and giving medication.

Objectives:
To be able to widen our knowledge and skills in caring our pts. specifically in pediatric ward.
To apply our skills and learnings in caring pedia pts. and to be able to render them enough care they need.

Observations:
It is our last rotation of duties for our 4th level.We are blessed because we are assigned at QPH, pedia ward. We are assigned before at CGH pediaward, but comparing to QPH pediaward, its really a big difference...
As to myself, I see to it that im giving the best and enough care needed by our pts.
Our group is a cooperative one.Despite the fact that for the past days we are busy and tired, we are giving our best to care our pts.We enjoyed our stay here. We are on hands-on,in caring our pts. We expereienced the actual task of a staff nurse.We are on functional task,being a supervisor nurse,headnurse,medication nurse and procedure nurse.
To our clinical instruuctress. thank you for the knowledge you shared with us.They said that your a "terror" one but the truth is that you a good mother to us.Its really my first time to be with you as my CI, but is very worthfull,because my knowledge regarding to our field widen....thank you ma'am.

Learnings:
Its a big,big responsibility.... Yes,being a student nurse is requires a big task.You must see to it that all the necessary needs of your pts. specially the care that they are seeking for will be properlly given. You must put yourself first to their situatoin and consider their family to be able to render them the care they need....
Above all, I thank GOD for all the blessings HE gave and for all the learnings HE showered upon us,during our duties...

OBJECTIVE:

OBSERVATION:

-This is our last rotation and last of our RLE DUTY at Pediatric ward,Quirino Provincial hospital.....
Two weeks after,i remember my first step in this hospital back when i was in my third year level,,
i felt so many things and things to be happen to me maybe they are negative or positive,,i feel so nervous and afraid,and also i expect for disappointment and discouragement from my CI or my Patient,,but as i walk through and through this expectations of mine is not enough to hold and hindrance my learnings as a nursing student and a future nurse someday.......this last duty is my last step for my RLE in this institution at pedia ward, all the things that i felt before,hatred,being afraid,and nervous is just only a feeling but it will serve as a tool and weapon to face challenges for a succesful nurse someday............

I learned on how to be cooperative,independent,and active through a strong temwork...


Classmates:

I thank and appreciate the skills,ability,and knowledge of my groupmates even thou there is a lot of misunderstanding and hatred to each everyone of us but still they are my friends and classmate...... 4ever!!!!


To my CI:

mam nhelz,your the most important CI in the nursing department,and that's really true....Many of us admired you most it is because of your benevolent inspiration,consideration,and support to all of us....And remember,,we're not able to step forward to our last level of our studying if there is no mam "PEREZ" that will help us and always giving inspiration to pursue and finished what we've started......And i really really appreciate your discussion about medical surgical nursing,coz i've learned a lot from "YOU"......THAnKS MAAM AND GODBLESS....abt. the review,,promize,i take it seriously for you....hekhekhekhek............
