Even before I handled this group, I already have fears that I may not be effective as a CI to them coz other CI's have warned me regarding the attitudes of some members of this group beforehand...
But after 2 weeks of exposure in the Male ward, I found out that most of them can change... if you give them challenging case to handle... but I guess there is always an exception to the rule. Some still commit the same mistakes and still does not remember the routine.
I hope and pray that most of them will; really become licensed nurses someday...

A. Self: During our second week of duty, I was able to hasten my work a little bit. Few errors are made but it's definitely fine coz it is not my procedures but instead it's just my chart. I'm a bit disappointed with myself because despite my effort in doing my job carefully, I always end up stumbling on the ground because of my lame penmanship. Will somebody save me from this fate!..
B. Groupmates: Compared last week, my groupmates who commit numerous errors were in good shape and transformed to a little bit competitive this time. Errors were lessened, and so as our dimerits.
C. Clinical Instructor: Madamme, we always knew how hard it is for you to handle students such us, that is why we always thank you, although we seldom verbalize it, but deep in our hearts we value the time you spent with us. You may seem to notice that a lot of errors are made during our duties but still you are always there for us giving your wisdom that strengthens our hope that someday we may be your colleague and work with you in harmony for the benefit of all. III. Learnings: In my experience, I have never encountered a patient's guardian that went berserk who's reason is just a mere misunderstanding. I was shocked that I had witnessed one myself yesterday, unfortunately it's my patient's wife. But as a student nurse, I always had my composure with me. I explained evrything to the staff and to ma'am Nhelia what was it all about. There has been an error commited by the pharmacist and so as the wife herself, she never checked the medication she received from the pharmacy and hysterically ends up blaming us when I returned the medication to her stating that she bought the wrong one. With the help of our angel, ma'am Nhelia, I was saved from the ferocity of this old woman and helped me explain to the old woman that she and the pharmacist were the one who committed the error. An d at the end of the day, I felt relieved knowing that it was not my fault, indeed, and I just did the right thing.

Self: I think I have improved already my self since our exposure to the male ward and my skills also have also improved. I've so many experiences that I only encountered in the male ward. I have improved my self esteem to do some procedures and I've also learned from my mistakes. So I've told to myself that I should be careful next time in charting because since our first day my mistakes were the same it's only with regards to charting.
Groupmates: As Ma'am Nhelia have said we have improved this week because compared last week I and my groupmates committed lots of errors mostly in charting and in procedures. This week we are slight faster than last week in doing our procedures.
Clinical Instructor: Ma'am thank you for the corrections to the errors we've made and thank you for the patience even some times we are that kind of "makulit" because sometimes we can't remember the lessons you've teach us. We will always remember you Ma'am for your own unique way of handling students we know that it's harder to handle students than a patient.
LEARNINGS: This week I've handled a Pneumonia patient and that patient coincidently one of the Godfather of my mother so I've find it easy to render care for a patient you know already. LEARNINGS
During my two weeks of staying in male ward, I have learned a lot in rendering prper nursing health care such as giving medications using the five rights,in some nursing procedures,and to become alert and always aware what I am doing, in writting with my patie'ns chart with correct data or informations because as long as you created error there is a corresponding demerits.
One thing more that I leaned is to become familiarized with the data of the patien before referring to a physician so that as long as they will ask a question you can answere it correctly.

> in that 3 days of duty, sometimes I felt drowsy because of almost only 3 hours of sleep atnight when making case abstract and NCP, but I toleareted it. Our CI said that we're improving in making SOAPIE, and I can say that also to myself because my errors during our first week of duty were lessen during that second week.

> as our CI said that were improving in making SOAPIE, its true as evidence by not late from endorsing our patient to the next nurse on duty that was happened during the first week of our duty, but some of my groupmates sometimes because of being busy making SAOPIE they forgot to carry out some of orders that need to be carry out that time.

> our CI is so much strict in terms of writing SOAPIE on patients chart. She's kind and considerable because she allowed us to post our LFD eventhough it is late.
Learnings
> I learned ways on how to gain the cooperation of too old patient, because my patient that time was already 90 years old and all we know that sometimes their atttitude is more than attitude of a child. I learned also some details about his disease which is UGIB, actions of his medicines, and nursing interventions of his common problems that I observed like having generalized body weakness and dizziness when standing.

>I learned how important it is to be well prepared and ready during duty. Having complete paraphernalia is not only for my own good but having them complete helps alot for patients whop can't afford to buy some of the materials needed. I'ved learned to adjust myself specially with the smell in male ward. When you are assinged to male ward, you should be physically healthy and at least have some extra vitamins.

LEARNING FEEDBACK DIARY
OBJECTIVES
GENERAL: To enhance my skill in rendering health service and this experience served as a training ground to become an effective nurse someday ( if God permits me to be).
SPECIFIC: To know the different activities, policies, procedures done in the wards specifically the medical ward and to gain more knowledge about the different illnesses with its information specially with the nursing responsibilities.
OBSERVATIONS
SELF:
It was our first day and I was so nervous. We had our orientation and the next day, we had started the so called "routine".
GROUP MATES:
Ever since my group mates were very funny though sometimes we have some misunderstandings, it will only takes a short period. We are sharing our ideas and help each other. They are really friendly and I'm happy to be with them.
C.I.: The first day was a long long orientation that it took us one day to discuss. Ma'am Nhelia oriented us everything, what we should do, the routine, her likes and dislikes, the demerits, merits, procedure usually done, medications, charting and many more. She's kind, I think everyone, including the lasts group, were saying that. She wants us to be oriented well so that we'll know what to do.
LEARNINGS:
The first day duty taught us a lot already even though it was only an orientation. We were all listening for the different do's and dont's and it was also the day of preparing ourselves for the duties we should do as a student nurse. I learned from this experience, being alert, having presence of mind, being patient, being knowledgeable, and having a heart who really cares for the patient. Having this real heart helps me to render service more easily. Though the second day which was the start of doing our duties we had some mistakes, but as we all know, those mistakes will help us grow and mold us to be a better person.

LEARNING FEEDBACK DIARY
OBJECTIVES:
GENERAL: To conduct duty in the male ward and to know more about their policies, regulations and procedures.
Specific: To render service and experience more about procedures done in the hospital.
OBSERVATIONS:
SELF: I felt nervous and excited. I was nervous because this is our first time to handle patient in the male ward.
Group mates: They help each other and they try their best in doing their responsibilities, they are also funny.
C.I.: Ma'am Nhelia is kind in our first day. She discussed the policies, regulations in the male ward and also the requirements. In our second day, She guided us in preparing and giving medications.
Learning:
I learned to be alert and to have a presence of mind in doing the procedures so that I can do the right way.

2nd week
OBJECTIVES:
Gen: to further hasten our knowledge and skills that we have acqiured in the past duties and lectures.
Specific: to render good quality of services for the patients that we care.
OBSERVATIONS:
Self: I think i had already improved my skills in this duty. I did not have mistakes/demerits for the whole week. I had already familiarized myself in the routine that is done in the male ward.
Group: I can see some improvement in our charting. they are very approachable , cooperative, and alert in what they are doing.
C.I: They are all approachable and kind. They always sharing thier knowledge to us, and i hope that may this knowledge they share will lead us to our brighter future. I observed to all of them that they dont want to waste time for nothing. They are always there in preparing and giving our medications.
LEARNINGS: I learn to be more alert in whatever you are doing, because we are dealing with life. I also gain some knowledge how to make pathophysiology.

Objectives:
>To enhance my capability in rendering Health Services
>To promote wellness among patient
Observations:
a. Self- Before, I wasn't serious in doing my task but now i have the initiative to do my responsibilities as a student nurse.
b. Group mates- They are comfortable to be with. You can feel their eagerness to learn.
c. C.I- Ma'am Nhelia is very approachable. She is easy to be with. At first, I was so afraid to be her student because of many feedbacks about her like she is cruel, strict and so on....but now I can say that those humors are wrong, though she is strict,but in the sense that she just want us to learn for our own sake.
Learnings:
I've learned a lot from ma'am Nhelia. She teaches me on how to be organized in doing my responsibilities as a student nurse. She want everything is in order and proper. She is always sharing a jokes for us to smile or even laugh. And I know that I'll be enjoying my stay with her company.

Objectives:
>To render care for the patient
>To improve my skills in computing medicines for our patient
>To apply what I've learned from our Lecture
Observations:
a. I'm so disappointed to myself because i had committed an error in charting that's why I acquired demerits
b. Group mates- They are responsible and also busy doing their SOAPIE chart
c. C.I- Ma'am Nhelia is a nice CI. She is funny and approachable. She let her students to learn and improve their skills with her supervision.
Learnings:
Ma'am Nhelia teaches us on how to do the Gordons 11. I also learned one most important thing that is to have a presence of mind.

Learning Feedback Diary #2
Objectives:
>To render Health care Service
>To care and give right medicine to our patients
Observations:
a. Self- My skills was improved and I'm willing to give my best in doing my duties as a student
b. Group mates- As usual, they are noisy but happy to be with. We are laughing all the time though sometimes we are tired.
c. CI- She is one of a kind, jolly and a instructor wherein you can learn a lot of things
Learnings:
I've learned to be more alert and improve more my skills in applying what I've learned from our SLE. Because of Ma'am Nhelia, I've learned to appreciates small things and give importance to those. I've learned to be more serious in handling patient and do what are my responsibilities as a nursing student.
