Third Year Block A Rotation 2 LFD

This space is for the LFD of 3A Rotation2...

I saw an improvement in all of you. Your skills were all rough during the first week but truly I can say, that you have improved as a Student Nurse. How I wish, grading you is as simple as winking my eye. But you know that it is not that easy.

Our lives are not determined by what happens to us but by how we react to what happens, not by what life brings to us, but by the attitude we bring to life. A positive attitude causes a chain reaction of positive thoughts, events, and outcomes. It is a catalyst, a spark that creates extraordinary results.

Whatever happens, I am confident that you will be able to see the reason behind your failure.

As I always say, God does not always answer YES or NO. Sometimes he answers WAIT...

General Objectives:

to enchane my knowledge to the different diseases in the hospital

Specific Objectives:

to experience and to apply care to the patient

II. Observation:
Self:
I feel more confident now because with the help of people around me we are taking all the consequences to face the future. To face all the challenges in life. I always believe on my strength. Self-confidence is one of my treasures. Another chapter another experiences, Education has enriched me so much gift to make me a better person to share all of this to people that I met.

Group mates:
I really enjoy with my group maybe because of the bonding. We help with each other, share of what we have know.

Clinical Instructor:
To my clinical instructor Ma’am thank you for a lot of experience and for making us a good and responsible student. Those demerits are one of your keys to make us better student. I must have to thankful to God for those low points we wound not have appreciated the high points better. Ma’am Nhelia is one of my C.I that I really like because of his own good way and maybe because she is so approachable interms of everything, sometimes joking to make us happy. Thanks ma’am for one of my inspiration if weren’t you I wouldn’t be here.

Learning:
I’ve learn lot in every duty especially to male ward, another duty is the beginning of a new challenged the journey for further learning. I obviously realized that the path to success is hard but through courage and determination, we can get through everything we also experience cherished the times we smiled together passing quizzes and examination despite all the difficulties that we had and experience we still made it successfully. We know that without ma’am nhelia we are nothing you been there for us all the time. Thanks ma’am for sharing, giving, and understanding us and for the quality time you have spent in our duty.

Observation:

Groupmates Observation:
>my groupmates has an individual strategies on how to give thier care to yhe patient andalso my groupmates are cooperative and has an unity to one another.

Self Observation:
>for me as an student nurse i comit mistakes. on my first day of exposure i felt nervouse but at the last two weeks of exposuring on the male ward i do my best to be allert at all times.

Clinical Instructor:
>mam nhelia perez as our clinical instructor,she impart his knowlege and challenging everyone,

Learning:
>on the whole day of rotation untill the first up to the last exposure of the hospital i learned a lot sometimes i felt nervouse,but i realize that those mistakes i've comitted i face them just to improve my skill and kwoledge.

> to give services to sick people in the accordance with my knowledge and past experiences

Observations:

Self
> I enjoyed the company of my groupmates especially when we're sharing our funny stories that we know.on that last week of our duty at male ward, I enjoyed also making preparations for our case presentation with my groupmates and I'm happy that the clinical instructors present on that presentation verbalized that they are impressed with our presentation because that evidenced that we're improving.

Groupmates
> they are all very kind, concern with there groupmates and cooperative because eventhough we have a lot of expences during the case presentation they are all comply from it without murmuring and I appreciate the cooperation of sir Daquioag, my partner with our patient during our last day of handling patient.

Clinical Instructor
> to our CI ma'am Nhelia, I really appreciate her kindness and concern with her students not only because she make friends with us but because she wants us to become a better with our chosen career by giving us punishments when we commit faults through demerits and sometimes she scolded us. She gave us also a lot of encouragements to motivate us to persue our chosen career eventhough sometimes our experiences discouraged us.

Learnings:
> I learned additional things about our case that we presented because that disease was discussed during our lecture, I learned also how to render services to patients through ward class and lastly, I learned to dischaged patient.

I. Objectives:

A. General: Hone skills and add learnings through clinical experiences.

B. Specific: Render care to individuals infirmed at Quirino Provincial Hospital.


II. Observations:

A. Self: I felt good at myself during this last week of duty, it is because our numerous errors and dimerits has come to a an end... he he he

B. Groupmates: When ma'am Nhelia meets us at our last post-conference, I am relieved that she's the one who voiced out the things I hate most about my groupmates. Sometimes they almost rely to their groupmates whenever they have to do something. It has been their manner ever since our rotation began which made the entire group to overwork for them. This is in fact the reason that slows our work as we will have to wait for the others before proceeding to something else.

C. Clinical Instructor: As always, our ever dearest ma'am Nhelia ends up "intoxicated" after our shift. As our mentor, you had shared and provided us all the knowledge we need which will help us in the near future and we appreciate that a lot. Although the time we had spent together is such a short time, we appreciate your good deeds to us and your endless love to our group. We thank you so much and may God bless you always.


III. Learnings: The recent rotation enabled us to be more confident and be competitive in our chosen career in life. Although some errors are made, this may be my guide to become a good health care provider to render good and quality service, bounded with love to my fellowmen and to myself. May God bless us all.

OBJECTIVES:

General:
☺To develop more our knowledge and skills in rendering care to the patients and to know all the importance of the punishments given to our mistakes.

Specific:
☺To render care and to enhance more our knowledge regarding case presentation and to perform well physical assessments interview patient regarding the 11 Gordon’s Functional patterns, to do NCP well, and to do all the parts of case presentation.

OBSERVATIONS:

Self:
☺To our last duty in the male ward we I think I did my best not to do same mistakes I’ve done during the last few days in the area, I will always remember the demerits I’ve done and I will always remember all the learning’s I’ve learned from Ma’am Nhelia.

Group mates:
☺During our case presentation I appreciate the cooperation in our group and we did our best to perform well in the case presentation but there are still some errors we’ve committed that we can’t avoid in all things we are doing because we all know that nobody is perfect.

Clinical Instructor:
☺Ma’am thanks for all the knowledge and demerits. I know that all demerits can help me in the near future and I will appreciate those because if you didn’t correct my mistakes I will not know they are not good and I will continue to do it.

LEARNINGS:
☺in the male ward even they are saying that the area is very toxic because in that area there are lot’s of patient with different cases and there are some instances that we encounter communicable diseases and very complicated cases, I have many learning in this area like doing well our responsibilities as students and to be patience enough to our patients and render care with confidence and with all our hearts. During our last day we have rendered care to approximately 30 patients it is so hard but it is enjoyable to render care to our patients.

OBJECTIVE
i: GENERAL OBJECTIVE:
To acquaint my knowledge regarding routines in the area.

II:SPECIFIC OBJECTIVE:
To render nursing health care services with our respective patient.

OBSERVATIONS
a.) SELF:
During my three weeks of staying in male ward,it helps me so much to develop my ability physically and mentlly,It helps me also to develop my self confident and selfdicipline although I had created some error andI had learned from it.
b.) GROUPMATES:
Like me,my groupmates also have a lot of demerits done in the area due to frequent error in charting,because as they become " toxic" the more they created error but beside of that they leaned alot.We as one group worked as one by helping each other.
c.C.I:
Our clinical instructor is very supportive and responsible enough, she has an organized plan of activities done in the area which is strictly implemented.She tried her best to trained us to become competent nurse someday.She wanted us always to be responsible,aware in what we were doing,to always allert in giving such medications and health care services and also to avoid any errasure in writing data in our respective patient because as long as you created mistakes there is always a coresponding equivalent. But theough the demerits we learned a lot and almost of us developed our self dicipline. THANK YOU VERY MUCH MAAM! WE LEARNED A LOT FROM YOU!

LEARNINGS
During my whole rotation in male ward, I had learn a lot not only in computation,in administration of medicines and in all the daily routine done in the area.Honestly, I had improved in making " SOAPIE" chart not just like before.I had learn also on how tomanage my time in making my requirements and my daily activities in the area.
During our case presentation, I had learn a lot from our mistakes in making the pathophysiology, nursing care plan, physical assessment and the gordons eleven functional pattern,hopefully,by next case presentation we will be able to make as good as one and fully prepared.

General objective
To be able to increase our knowledge with the procedures and routine done within the hospitals well as incorporate new concepts learned from the classroom the actual duty area,

Specific objective
To apply my knowledge that I had learned and to be able to give the best care to the patient.

OBSERVATION

Self
Well, at this time I am proud because I learned a lot, and I can do a lot of things independently.


Groupmates
My groupmates is very kind, most of them are friendly, for almost two months that were been together as a group I know the attitude they have.

CI
I am very thankfull because I learned a lot from you. Thank you for sharing knowledge and skills, and for those challenges to study hard and most of all thank you for teaching how to discipline ourselves. You are very responsible and approachable, you are always there to guide in every activities we do I honestly and frankly speaking at first I really hate you because you always scolded me infront of my classmates but now I realized this is your challenges to me to study hard.

Learnings
I learn to mingle with different kinds of patients and I also learn to interact with them.

Observation

Self
I was able to make my work better and I think I am starting to improve my self.

Groupmates
Each and everyone of us has a cooperation. We helped each other and bonding is getting better and deeper.

Clinical Instructor
She is such an observant C.I. She saw our strength and even our weaknesses. She gave such advice to make our work more effective.

Learnings
We have a presence of mind to perform an accurate procedure or steps must be followed and everything should be in order.

A profession rendering care to client by tender loving care.Treating them as what they need as what they deserve.We must put ourselves in the situation just to understand what they feel.Always smile at them and give hope.Caring with whole life by soul and heart.Enhance their feeling for them to recover.

Basic Human Needs
1.Oxygen/air,it gives life to us.It is our primary need
2.Cost,we need money to buy food,water,shelter and other thing what we need.
3.Fluid,we need fluid or water in our body for the circulation of blood.
4.Food,we need food to gain energy.
5.Rest and Sleep,we need to relax our mind and body.
6.Elimination,we need to eliminate those toxic in our body.
7.Worship,be faithful and always pray to GOD.
8.Physical Hygiene,take a bath everyday and clean our body every time.
9.Maintenance of body figure,be presentable and always exercise for good health and circulation.
10.Safety and Security,always take care ourselves.

Objectives:
>To render care for the patient
>To improve my skills in computing medicines for our patient
>To apply what I've learned from our Lecture

Observations:
a. I'm so disappointed to myself because i had committed an error in charting that's why I acquired demerits

b. Group mates- They are responsible and also busy doing their SOAPIE chart

c. C.I- Ma'am Nhelia is a nice CI. She is funny and approachable. She let her students to learn and improve their skills with her supervision.

Learnings:
Ma'am Nhelia teaches us on how to do the Gordons 11. I also learned one most important thing that is to have a presence of mind.
